Abstract:
Objective:To investigate the effects of anti-reflux sleeve gastrectomy and sleeve gastrectomy on circulating adipokines and inflammation related cell levels.Method:A retrospective cohort study was conducted to analyze the data of 45 obese patients undergoing laparoscopic sleeve gastrectomy plus fundoplication (LSGFD) and laparoscopic sleeve gastrectomy (LSG) at Xinjiang Uygur Autonomous Region People's Hospital from Apr to Sep 2023. Patients were followed-up for 6-month .Body weight, fasting plasma glucose (FPG), 2-hour postprandial glucose (2-h PG), GERD questionnaire score (GERD-Q), circulating leptin, adiponectin, peripheral white blood cell count (WBC), neutrophils (NE), lymphocytes (LY), and neutrophil-to-lymphocyte ratio (NLR) between two groups of patients before surgery and 6 months after surgery were conducted. Results:The operative times for the LSGFD group and the LSG group were 180 (105-240) minutes and 125 (85-180) minutes, respectively ( Z=-3.358, P<0.01). There were no statistically significant differences in intraoperative blood loss, total hospital stays, postoperative hospital stays, use of robotic assistance, or readmission within 30 days postoperatively (all P>0.05). Six months after LSG, patient's weight, BMI, 2-h PG decreased ( all P<0.05), Six months after LSGFD, patient's circulating leptin, peripheral blood WBC, NE, and NLR levels decreased ( Z=-3.808, -3.173, -3.435, -2.389, -3.173, all P<0.05). Six months after LSG, patient's circulating leptin, peripheral blood WBC, NE, and NLR levels decreased ( Z=-4.292, -3.606, -3.431, -2.220, -3.619, all P<0.05). Conclusion:LSGFD and LSG not only significantly reduce the weight of obese patients but also improve the levels of circulating adipokines and inflammatory status.
