The application status of cognitive behavioral therapy techniques for generalized anxiety disorder: a Delphi study

Abstract:
Objective:To investigate the current application status of Cognitive Behavioral Therapy (CBT) techniques for Generalized Anxiety Disorder (GAD) by Chinese domestic experts using the Delphi method. The goal is to promote the standardization of CBT for GAD.Methods:Based on a review of relevant domestic and international literature, an expert consultation form including 59 CBT techniques for GAD (categorized into basic, cognitive, behavioral, and other techniques) was developed. The Delphi method was used to conduct two rounds of consultations with 24 qualified CBT experts. The application status and importance were ranked according to the statistical data.Results:(1) In the two rounds of consultation, the positivity index and the authority coefficient of the experts were both above 0.8. (2) After the second round of consultation, Kendall′s W coefficients of expert opinions on the frequency of use, contribution to treatment, and acceptance by clients were 0.397, 0.291, and 0.336, respectively, which are acceptable. Among the 59 CBT techniques for GAD, except for four behavioral techniques (music relaxation training,deliberately increasing anxiety experiments, systematic desensitization, and worry time), all other techniques received unanimous agreement from the experts in the three dimensions. (3) The top five techniques in terms of comprehensive application were data collection, establishing a therapeutic alliance, clinical psychological assessment, psychoeducation, and goal setting. Conclusion:In CBT for GAD, domestic experts unanimously recognize the importance and applicability of basic, cognitive, and traditional behavioral CBT techniques. However, they are relatively cautious about the clinical application of emerging techniques from the third generation of CBT, except for mindfulness and sleep management.
