Abstract:
Objective To investigate the expression and prognostic correlation of serum lipoprotein phospholipase A2(Lp-PLA2),pentamerin 3(PTX3)and high mobility group protein B1(HMGB1)in patients with acute cerebral infarction(ACI)after thrombolytic therapy.Methods A retrospective study was conduct on 190 ACI patients admitted to the Affiliated Hospital of Tangzhishan Vocational and Technical College from May 2020 to October 2024(observation group),and another 190 healthy individuals taking health checkup in the hospital during same period(control group).According to the severity of ACI,the observation group were divided into mild,moderate and severe subgroups(66,80 and 44 cases,respectively).According to the clinical outcomes after follow-up,the observation group were also divided into a good prognosis group(120 cases)and a poor prognosis group(70 cases).The levels of serum Lp-PLA2,PTX3,and HMGB1 were compared in the groups.Spearman correlation analysis was used to analyze the cor-relation between the levels and disease severity.The factors affecting the prognosis of ACI pa-tients were analyzed,and the prognostic value of the three indicators for ACI after thrombolytic therapy was evaluate.Results The serum levels of Lp-PLA2,PTX3 and HMGB1 were signifi-cantly higher in the observation group than the control group(P<0.01),and were gradually decreased in the mild,moderate and severe subgroups in turn(P<0.05).Spearman correlation analysis showed that the levels of the three indicators were positively correlated with the severity of disease in ACI thrombolytic patients(r=0.579,r=0.689,r=0.592,P<0.01).The poor prog-nosis group exhibited significantly more lesions,larger cerebral infarcts,longer time from onset to thrombolysis,higher NHISS score at admission and elevated levels of serum Lp-PLA2,PTX3 and HMGB1 than the good prognosis group(P<0.01).Multivariate logistic regression results indica-ted that number of lesions,volume of cerebral infarcts,time from onset to thrombolysis,NIHSS score at admission and high serum Lp-PLA2,PTX3 and HMGB1 levels were risk factors for poor prognosis in patients with ACI thrombolysis(P<0.05).ROC curve analysis revealed that serum Lp-PLA2,PTX3 and HMGB1 levels in predicting poor prognosis of ACI patients after thrombo-lytic therapy,had an AUC value of 0.928,0.971 and 0.860,respectively.Conclusion In ACI patients treated with thrombolytic therapy,serum Lp-PLA2,PTX3 and HMGB1 levels are posi-tively correlated with the severity of the disease.PTX3 has a certain predictive value for prognosis in the patients.
