Abstract:
Objective:To summarize and analyze the clinical characteristics of patients with inflammatory bowel disease (IBD) complicated with primary biliary cholangitis (PBC) .Methods:Clinical data of patients with IBD complicated with PBC treated at Peking Union Medical College Hospital from January 2010 to December 2021 were collected and analyzed, including general conditions, clinical manifestations, laboratory tests, endoscopic and pathological manifestations, treatment and prognosis. The cases of IBD complicated with PBC reported in previous literature were also systematically reviewed and analyzed.Results:There were 5 patients with IBD complicated with PBC in our hospital, who were all ulcerative colitis (UC) , including 4 females and 1 male. Four cases of UC were diagnosed before PBC, 5 cases were all with extensive colonic type (E3) , and 3 patients complicated symultaneously with other autoimmune diseases, including one with psoriasis and autoimmune pancreatitis, one with Sjogren syndrome, and one with autoimmune enteritis. A total of 40 cases were included in the systematic review, mainly female (28 cases, 70.0%) and UC (34 cases, 85.0%) patients. More patients were diagnosed with IBD before PBC (31 cases, 77.5%) . The positive rate of AMA was 89.2%, 11 patients (27.5%) complicated symultaneously with other autoimmune diseases, and 5 patients (12.5%) progressed to hepatic cirrhosis. Four IBD patients underwent surgery (10.0%) , and 1 patient developed colon carcinoma (2.5%) .Conclusion:IBD complicated with PBC is found mostly in female UC patients, IBD is predominantly diagnosed before PBC, and complicates symultaneously with other autoimmune diseases.
