Abstract:
[Objective]To explore the traditional Chinese medicine(TCM)pathogenesis and treatment strategies for renal involvement in primary Sj?gren's syndrome(pSS)by using the"Sanjiao-Moyuan-Xuanfu"system.[Methods]By analyzing the structural and functional connotations of Sanjiao(triple energizer),Moyuan(membranous source)and Xuanfu(subtle pores),combined with the renal pathological mechanisms of renal involvement in pSS,it summarized the diagnostic and therapeutic approach for pSS renal damage based on the Sanjiao-Moyuan-Xuanfu system.A validated case was included for illustration.[Results]The Sanjiao-Moyuan-Xuanfu system is characterized by membranous structures taken as its physical basis and the circulation of Qi and fluids as its functional core.The kidney can be analogized as a microcosmic Sanjiao-Moyuan-Xuanfu system.The study proposes that spleen-kidney deficiency and impaired Qi transformation in Sanjiao constitute the core pathogenesis of the disease.Macroscopically,it manifests as endogenous dampness,heat,phlegm and blood stasis;microscopically,it is characterized by stagnation in Moyuan and dysfunction of Xuanfu,which serve as critical turning points in the disruption of kidney Qi movement and the degeneration of renal Xuanfu.The treatment strategy emphasizes harmonizing dryness and dampness,regulating the spleen and kidney to restore Sanjiao Qi dynamics,while simultaneously dispersing latent pathogens in renal Moyuan and promoting blood circulation to resolve stasis and unblock Xuanfu.The attached case involved a complication of dryness-Bi syndrome with blood stasis and water retention,treated with blood-activating and water-draining methods,spleen-strengthening and dampness-resolving therapy,as well as liver-kidney nourishing and heat-clearing approaches,yielding favorable outcomes.[Conclusion]The Sanjiao-Moyuan-Xuanfu system is involved in the distribution of Qi and body fluids.Under pathological conditions,dysfunction of this system provides a novel perspective for elucidating the pathological features of renal involvement in pSS,offering theoretical and methodological insights for its clinical diagnosis and treatment.
