Effects of remimazolam and propofol on hemodynamics and serum inflammatory factors in patients undergoing general anesthesia

Abstract:
Objective To evaluate the effects of remimazolam and propofol on hemodynamics and serum inflammatory factors during general anesthesia.Methods A total of 100 patients undergoing general anesthesia at Nantong Second People's Hospital from January 2022 to June 2023 were selected and randomly divided into the remimazolam group(n=50;initial dose:7 mg,maintained at 0.1~0.2 mg/(kg·h)and the propofol group(n=50;initial dose:1.5~2.5 mg/kg,maintained at 4-6 mg/(kg·h).Standardized anesthesia induction was performed in both groups.Hemodynamic parameters(heart rate,systolic blood pressure,diastolic blood pressure,bispectral index)were recorded at baseline(T0),immediately after intubation(T1),during surgery(T2),and upon awakening(T3).Serum levels of interleukin-6(IL-6),C-reactive protein(CRP),adrenaline,cortisol,and T-lymphocyte subsets(CD3+,CD4+/CD8+)were measured 24 h postoperatively.Anesthesia depth,observer's assessment of alertness/sedation(OAA/S)scores,and adverse events were assessed.Results Compared with the remimazolam group,the propofol group exhibited significantly higher heart rate and systolic blood pressure at T1 and T2,elevated CRP and IL-6 levels at the end of surgery and 24 h postoperatively,and increased CD3+and CD4+/CD8+ratio at 24 h postoperatively(P<0.05).The time to loss of consciousness in the remimazolam group was longer than that in the propofol group,whereas the recovery time and extubation time in the remimazolam group were shorter than those in the propofol group(P<0.05).The OAA/S scores at the beginning of surgery and one hour during surgery in the remimazolam group were lower than those in the propofol group(P<0.05).The incidence of adverse events in the propofol group was higher than that in the remimazolam group(P<0.05).Conclusion Remimazolam provides more stable hemodynamics,attenuated inflammatory responses,and better stress modulation than propofol in general anesthesia.
