External fixation with locking plates in tibiofibular fracture: biomechanical analysis and its clinical efficacy

Abstract:
Objective To conduct biomechanical analysis and study clinical efficacy of external fixation with locking plates in open tibiofibular fracture.Methods Twenty-three cases of open tibiofibular fracture admitted into the hospital from March 2013 to April 2016 for treatment by using external locking plates were recruited as research subjects.All the patients were treated with debridement,fracture reduction and external fixation with locking plates,and had medical follow-ups for at least 1 year after the removal of steel plates.Biomechanical finite element analysis was made in the treatment of tibiofibular fracture by using locking plates,and overall structural stability of the steel plates at different distances between the plates and the tibia was investigated.Results All the patients gained bony healing.Results indicated that average time of plate removal was 6.3 months.Due to severe blood flow disorder,there were 4 cases of delayed healing and 1 unhealed case,which was later cured by debridement of the fractured end of the bone and internal fixation.Finite element analysis indicated that fractured bone gap tended to increase,when the distance between the bone and locking plates increased,and whole rigidity tended to decrease.At the same time,stress concentrated at the roots of the bolts,and it aggravated when the distance between the bone and bolts increased.Conclusion External fixation with locking plates could effectively fix the fractured ends of the bone,be used to treat open tibiofibular fracture,shorten healing time,reduce damage to the soft tissues,as well as lower the incidence of osteomyelitis.Therefore,it might be a new approach to the treatment of lower limb fractures.
