Incidence and influencing factors of low-level viraemia in people living with HIVAIDS of different ages in Zhengzhou

Abstract:
Objective To analyze the incidence of low-level viraemia (LLV) in HIV/AIDS patients of different ages, and to explore the influencing factors of LLV. Methods Relevant information was collected from HIV/AIDS patients undergoing anti-retroviral therapy (ART) in Zhengzhou from January 1, 2004 to December 31, 2023. The patients were categorized into ≥50 years old and <50 years old groups based on their age at the initiation of ART. The Cox proportional hazards regression model was applied to analyze the influencing factors of LLV. Results A total of 2 981 cases were included in this study, comprising 2 460 cases in the group aged <50 years and 521 cases in the group aged ≥50 years. In the group aged <50 years, 15.5% (382/2 460) developed LLV, with an incidence density of 25.8/1 000 person-years (95% CI: 23.2-28.3). In the group aged ≥50 years, 22.6% (118/521) developed LLV, with an incidence density of 38.3/1 000 person-years (95% CI: 31.5-45.0). The incidence of LLV was higher in the group aged ≥50 years compared to those aged <50 years (χ2=14.353, P<0.001). Multivariate Cox proportional hazards regression analysis showed that being age ≥50 years, having a baseline CD4 cell count ≤200 cells/μL, and missing medication were the risk factors for LLV. Conclusions Elderly HIV/AIDS patients have a higher incidence of LLV, and age is an independent risk factor for the occurrence of LLV in HIV/AIDS .
