Predictive value of cytokeratin combined with carbohydrate antigen 125 detections for prognosis of patients with bladder cancer

Abstract:
Objective:To investigate the predictive value of cytokeratin 19 fragment 21-1 (CYFRA21-1) combined with carbohydrate antigen 125 (CA125) detections for prognosis of patients with bladder cancer.Methods:A retrospective case series study was conducted. The clinical data of 97 patients with bladder cancer in Yancheng First People's Hospital from January 2016 to December 2020 were retrospectively analyzed, and they were divided into survival group (82 cases) and death group (15 cases) according to the prognosis of patients. The clinical data and CYFRA21-1 and CA125 levels of patients were compared between the two groups, and the influencing factors of prognosis and the predictive value of CYFRA21-1 combined with CA125 for prognosis of patients were analyzed.Results:Among 97 patients with bladder cancer, 48 were male and 49 were female, and the age was (65±5) years old; 15 patients (15.46%) died 2 years after operation. The proportions of patients with stage T 2-T 4 [46.67% (7/15)], muscle-invasive bladder cancer [60.00% (9/15)], lymph node metastasis [46.67% (7/15)], low differentiation [40.00% (6/15)] and CYFRA21-1 level [(8.5±2.2) ng/ml] and CA125 level [(36.6±6.5) U/ml] in the death group were higher than those in the survival group [21.95% (18/82), 31.71% (26/82), 20.73% (17/82), 15.85% (13/82), (5.3±1.7) ng/ml, and (18.6± 2.9) U/ml], and the differences were statistically significant (all P < 0.05). Logistic regression analysis showed that clinical stage, lymph node metastasis, degree of tumor differentiation, CYFRA21-1 level and CA125 level were the influencing factors of prognosis (all P < 0.05). The analysis results of receiver operating characteristic curve showed that the optimal cut-off values of CYFRA21-1 and CA125 for predicting prognosis of bladder cancer patients were 6.89 ng/ml and 23.47 U/ml, the area under the curve (AUC) was 0.868 and 0.848, and the AUC of the combination of the two was 0.962. Conclusions:CYFRA21-1 and CA125 are effective biological indicators for predicting the prognosis of bladder cancer patients, and the predictive value of the combined detection of the two is high.
