Pathogenic bacterium and risk factors of wound infection in patients with pressure injury in intensive care unit

- Abstract: Objective To investigate the pathogenic bacterium and risk factors of wound infection in patients with pressure injury(PI)in intensive care unit.Methods The clinical data of 189 PI patients during hospitalization in intensive care unit were retrospectively analyzed,the incidence of wound infection and the types of pathogenic bacterium were counted,and the risk factors leading to wound infec-tion were analyzed.Results Of 189 PI patients in intensive care unit,32 cases(16.93%)developed wound infection.A total of 48 strains of pathogenic bacterium were isolated from the wound secretions,including 19 strains(39.58%)of gram-positive bacterium,27 strains(56.25%)of gram-negative bacterium and 2 strains(4.17%)of fungi.The age≥70 years,wound stages of 3 to 4,combined with tissue edema,combined with diabetes,plasma albumin level<30 g/L,PI duration≥1 month,and urinary and fecal incontinence were the risk factors for wound infection in PI patients(OR>1,P<0.05),while the addition of traditional Chinese medicine dressing and the ability of patients to turn over spontaneously were the protective factors against wound infection(OR<1,P<0.05).Conclusion The detection rate of gram-negative bacterium is higher in the wound infection of PI patients in intensive care unit,and the older patients,or patients with the wound stages of 3 to 4,tissue edema,diabetes,low plasma albumin level,long duration of PI,and urinary and fecal incontinence are more likely to develop PI wound infection.Therefore,using targeted antibiotics and traditional Chinese medicine dressing,and assisting patients to turn over frequently can reduce PI wound infection.
