Analysis on the changing trend of parturient delivery in a ClassⅢ Grade A hospital in Beijing from 2013 to 2021 based on the circular distribution method

Abstract:
Objective:To explore if there is a central trend in the change of delivery information of parturient and delivery time after the change of China's fertility policy, and to provide a basis for the allocation of midwife human resources.Methods:In this descriptive study, totally 18 306 parturient who gave birth in a ClassⅢ Grade A hospital in Beijing from January 2013 to December 2021 were selected by convenience sampling. The delivery information of the parturient was collected, including age, parity, whether it was premature, whether it was a low birth weight baby, etc., and the circular distribution method was used to analyze if there was a central trend in the delivery time of the parturient underwent vaginal delivery.Results:Among the 18 306 parturient, 12 222 (66.76%, 12 222/18 306) had vaginal delivery and 6 084 (33.24%, 6 084/18 306) had cesarean delivery; there were 4 688 (25.61%, 4 688/1 8306) multiparae and 3 751 (20.49%, 3751/18 306) advanced maternal age woman. From 2013 to 2021, the proportion of multipara, advanced maternal age woman and premature infant showed an increasing trend, among which the advanced maternal age woman accounted for 11.34% (235/2 072) in 2013 and reached a peak in 2020, accounting for 30.22% (475/1 572) ; in 2021, it slightly decreased to 27.84% (324/1 164) . The proportion of multipara in 2013 was 9.27% (192/2 072) , and reached a peak of 34.74% (740/2 130) in 2017, with an average annual rate of more than 30.00% from 2018 to 2021. The total proportion of premature infant was 7.84% (1 435/18 306) , and the highest rate was 9.79% (114/1 164) in 2021. From 2013 to 2021, there were significant differences in the proportion of multipara, advanced maternal age woman and premature infant ( P<0.05) . The results of circular distribution method showed that the delivery time of the women who delivered vaginally had a centralized trend, mainly from 14∶00 to 15∶00 ( P<0.01) . Conclusions:With the adjustment of fertility policy, from 2013 to 2021 the proportion of advanced maternal age woman, multipara and premature infant has increased year by year, and the delivery time of vaginal delivery women has a tendency to concentrate. Therefore, it is necessary to dynamically adjust the manpower of midwives to ensure the safety of mothers and babies and provide a basis for the deployment of midwife human resources.
