1.Penile fracture: experiences with conservative and surgical treatment.

We experienced with 3 cases of penile fractures for recent 2 years, one occurred during masturbation and the others occurred during coitus(one case was associated with urethral injury). While the former was treated with conservative management, the latters were treated with surgical intervention that was immediate removal of hematoma and primary closure of ruptured corpus cavernosum. Penile deformity end decrease in rigidity occurred in conservatively treated case. However, two cases of immediate surgical treatment had a excellent result without complication. Even though we describe only 3 cases and could not compare the degree of injury. our short experiences suggest that immediate surgical intervention is better than conservative management in initial treatment of penile fracture.

3.A Foreign Body in the Urethra: A Case Report.

Most foreign bodies in the lower genitourinary tract are self-inserted through the urethra for masturbation. It can be diagnosed by clinical histories and physical examinations of the patient, endoscopy and radiologic evaluations. Extraction should be tailored as the surgery, endoscopic manipulation and dissolution according to the nature of the foreign body and should minimize bladder and urethral injury. We report a case of a long wire in the bladder and urethra with brief review of literatures.

5.Penile Injury During Erection: The Clinical Manifestations and Results of Operative Treatment.

This study was designed to evaluate the clinical manifestations and results of operative treatment in penile injury during erection. We reviewed 18 cases of penile injury during erection from December 1992 to June 1996. Of 18 patients, 14 patients were treated with early operation, 3 patients with conservative treatment and followup was 1 to 31 months (mean: 14.1 months). Patients age ranged from 23-76 years old (mean: 39.6 years), common cause of the injury was sexual intercourse (8 cases), masturbation (3 cases) and finger-pressure (2 cases) in order. Of 8 patients who occurred during sexual intercourse, 7 patients were married. The injury developed at midnight in 8 cases (44%), early morning in 7 cases (39%) and the other times in 3 cases (~7%). Of 14 patients who received with early operation, 13 patients revealed rapture of the corpus cavernosum and 1 patient revealed rupture of superficial dorsal vein. The site of penile fracture showed proximal shaft in 8 cases (62%), midshaft in 3 cases (23%) and distal shaft in 2 cases (15%) in order. The overall complication rate of early operative treatment was 15% (2 of 13 cases) compared to a complication rate of 100% (2 of 2 cases) for conservative treatment. In conclusion, our experience demonstrates that the most common cause and site of penile injury are sexual intercourse and proximal shaft, early operative treatment would be an effective treatment of penile injury to prevent complications.

6.Role of Reactive Oxygen Species in Motility of Human Spermatozoa.

PURPOSE: To determine whether seminal malondialdehyde and carbonyl group have any relation to sperm motility. MATERIALS AND METHODS: Human semen samples were obtained by masturbation after 3 days of abstinence from patients consulting an infertility clinic. Using conventional semen analysis, samples were divided into two groups according to sperm motility (group 1: motility > OR =50%, group 2: motility <50%). Malondialdehyde and carbonyl group were measured in whole semen. RESULTS: The amount of malondialdehyde and carbonyl group was slightly lower in group 1 than group 2. CONCLUSIONS: Although the difference between the groups was not statistically significant, it seems possible that malondialdehyde and carbonyl group, which are produced from reactive oxygen, are negatively correlated with sperm motility.

7.Long-Term Results of Immediate Surgical Treatment of Penile Fracture.

PURPOSE: Penile fracture is a rare but serious urological condition. Immediate surgical repair is widely accepted as the treatment of choice in penile fracture. The aim of this study is to investigate the long-term outcome of immediate surgical treatment of penile fracture. MATERIALS AND METHODS: This is a retrospective study of 12 men with penile fracture who were treated in the Department of Urology, Sanggyepaik Hospital, Seoul, Korea from January 2000 to June 2005. Diagnosis was made clinically and was confirmed by cavernosography in all our patients. All patients underwent immediate surgical repair, within two days after trauma, using a degloving incision. The long term results of the immediate surgical repair were evaluated using questionnaire on outpatient department visiting or telephoning. RESULTS: The median patient age was 43 years (range, 18 to 57 years). The median follow-up was 32 months (range, 14 to 60 months). Of these patients, 8 (66.7%) patients were injured during sexual intercourse, whereas 4 (33.3%) patients were injured during masturbation. All patients were treated by immediate surgery. All patients reported satisfactory, painless erectile function; two developed penile curvature and one had a penile nodule. CONCLUSIONS: Immediate surgical repair of penile fracture is effective, restores erectile function, and the incidence of complications is low.

8.Masturbation and its relationship to sexual activities of young males in Korean military service.

This study examined the masturbatory experiences and other sexual activities of young Korean males in military service. The actual status of masturbation and its relationship to sexual activity questionnaire. A total of 1,212 young males among military personnel in Korea were interviewed with sexuality questionnaires on masturbation, sexual intercourse and personal characteristics. We divided these subjects into four groups according to the age of initiation of masturbation and analyzed the relationship between masturbation and other sexual activities. The mean age of subjects was 22.03 +/- 1.22 (19-27) years. The percentage of men who reported ever having masturbated was 98.1% (1189/1212) and the average age of initiation of masturbation was 14.26 +/- 1.66 years. So we divided 1,212 males into four groups on the basis of the average age below and above one standard deviation. Overall, 67.7% (821/1212) had experienced sexual intercourse, and significantly, the earlier the initiation age of masturbation, the higher the coitus rate in each group (p < 0.05). About 21.5% of the men who were not virgins had experienced their first sexual intercourse with prostitutes. The mean age of first coitus, the incidence of sexually transmitted disease (STD) and the frequency of masturbation were closely linked to the initiation age of masturbation, respectively (p < 0.05). A masturbatory guilt feeling was seen in about 10.9% (132/1212) and there was no significant difference according to the types of religious worship (p = 0.227). On the basis of this study, sexual activities generally increased accordingly as the beginning of masturbation was earlier. Coital incidence in this study was 67.7% for young males in Korean military service, and 21.5% of them had their first sexual intercourse with prostitutes. Prostitution still plays an important role in the sexual lives of males in Korea. The incidence of STD was over 10% and homosexual manifestation was seen in 1.07% of subjects. Thus a proper and sound sex education at school during childhood is needed in Korea. A large scale survey of sexual behavior in various populations is essential for the improvement of correct sexual concepts.

9.Is the Index Finger and Ring Finger Ratio (2D:4D) Reliable Predictor of Semen Quality?.

PURPOSE: The purpose of this paper is to test the validity of hypothesis that the index finger and ring finger ratio (2D:4D) is related with men's semen quality. MATERIALS AND METHODS: We set two groups of healthy young male student (mean age 23.9) who have different 2D:4D ratio from different departments of Chosun University. One group consists of 26 men whose 2D:4D ratio is higher than 1 (mean 1.06), and the other group consists of 33 men whose 2D:4D ratio is lower than 1 (mean 0.94). Their semen was collected by masturbation and examined. RESULTS: A comparative analysis revealed that there is no relation between semen quality and the 2D:4D ratio. The group of 2D:4D> or =1 showed a semen volume of 3.66+/-1.64 ml, on the other hand, the group of 2D:4D<1 showed a semen volume of 3.73+/-1.40 ml. No statistical correlation was found (p=0.82). The view of sperm count and motile sperm in single ejaculated semen also showed no statistically significance with 2D:4D ratio (p=0.84, p=0.43, respectively). CONCLUSIONS: The 2D:4D ratio has no statistically significant correlation with the semen quality of health young male. Thus, measurements of finger length cannot be a reliable indicator of semen quality and testicular function.

10.Operative Treatment with a Laparotomy for Anorectal Problems Arising from a Self-Inserted Foreign Body.

An anorectal foreign body can cause serious complications such as incontinence, rectal perforation, peritonitis, or pelvic abscess, so it should be managed immediately. We experienced two cases of operative treatment for a self-inserted anorectal foreign body. In one, the foreign body could not be removed as it was completely impacted in the anal canal. We failed to remove it through the anus. A laparotomy and removal of the foreign body was performed by using an incision on the rectum. Primary colsure and a sigmoid loop colostomy were done. A colostomy take-down was done after three months. The other was a rectal perforation from anal masturbation with a plastic device. We performed primary repair of the perforated rectosigmoid colon, and we didea sigmoid loop colostom. A colostomy take-down was done three months later. Immediate and proper treatment for a self-inserted anorectal foreign body is important to prevent severe complications, and we report successful surgical treatments for problems caused by anorectal foreign bodies.
