2.Pain on Injection of Rocuronium: Influence of Two Methods of Lidocaine Injection.

BACKGROUND: Many trials have been conducted on the prevention of pain induced by intravenous rocuronium. The aim of this study was to assess the effectiveness of lidocaine preinjection compared with rocuronium premixed with lidocaine in minimizing pain on injection. METHODS: Seventy five patients, ASA physical status I or II, were randomly allocated to one of three groups. In the control group (n = 25), patients received saline 3 ml, followed 20 slater by rocuronium (0.6 mg/kg) only. In the lidocaine premixture group (PM, n = 25), normal saline 3 ml and rocuronium(0.6 mg/kg) premixed with lidocaine 30 mg were given intravenously, and in the lidocaine preinjection group (PI, n = 25), lidocaine 30 mg was administered before injection of rocuronium (0.6 mg/kg). Pain was assessed verbally as none, mild, moderate, and severe by the same observer. RESULTS: There was a significant reduction in the incidence and severity of pain in the PI group. Only 6 patients (24%) in the PI group experienced pain as compared to 16 patients (64%) in the PM group and 22 patients (88%) in the control group. CONCLUSIONS: Lidocaine 30 mg given before the administration of rocuronium significantly reduced the incidence and severity of rocuronium induced pain. But, lidocaine rocuronium premixtures did not significantly reduce the incidence of pain.

The magistral formula and processing procedures have been studied and tested to reach the VNP II - 1994 and BP-1993 standards. The lethal dosage (LD50), the anti - ventricular fibrillation and anti-arrhythmic effects, the blood concentration of lidocaine taken orally at different times as well as the dissolution of rythmocardyl in the human body have been tested experimentally. The research have proven that lidocaine, taken orally has few side effects and is safe and effective to treat the cardiac arrhythmia, notably the ventricular extrasystoles

Background: Transrectal ultrasound guided prostate biopsy has placed a role in the urologist armamentarium. Considered as a minor procedure, TRUS guided prostate biopsies has currently been performed without any anesthesia. Recent studies have observed that prostate biopsy is perceived as painfulObjective: A. To compare the effect of intrarectal lubricant gel application, intrarectal lidocaine gel application and periprostatic lidocaine injection on the pain scores of patients undergoing transrectal ultrasound guided prostate biopsy. B. To determine the differences in morbidity after the procedureMaterials and Methods: From January 2004 to August 2004, 100 men underwent prostate biopsy at a tertiary hospital. Patients were distributed into 3 groups (control, lidocaine gel, lidocaine injection). A visual analog scale was used to assess the pain score. The Shapiro-Wilk test was performed on all epidemiologic data as well as on the patients pain scores. Statistical analysis used includes analysis of variance for age and Kruskal-Wallis test for PSA level, prostate volume and pain score. Tukey and Mann Whitney U test were subsequently doneResults: Ultrasound guided prostate biopsy was done in 100 cases. There were no statistical difference as to age, PSA level and prostate volume between the 3 groups. There was no statistical difference in the pain scores of patients after intrarectal lubricant gel application and intrarectal lidocaine application. (4.933 versus 4.250, p 0.1375). However, there was a statistical difference in the pain scores of patients after intrarectal lidocaine gel application and periprostatic injection (4.250 versus 2.158, p 0.0001) and intrarectal lubricant gel application and periprostatic injection (4.933 versus 2.158, p 0.0001)Conclusions: Periprostatic lidocaine injection effectively lowers the pain scores of prostate biopsy than those who received lidocaine gel or lubricant gel. Improvement in patient tolerance permits the number of biopsy cores to be increased as necessary without increasing patient distress. Routine use of local anesthesia in the formed of periprostatic lidocaine injection is highly recommended in future biopsies

10.Evaluation on the therapeutic results of anal fissure by point-injection procedure combined with pressing on musculus teres internus

175 patients with anal fissure have received 1% lidocaine point – injection therapy combined with pressing on musculus teres internus. Results: 19.4% (n=34) of patients with high level and 80.6% (n=141) with low level of anal spasm, experienced no longer spasm after one day and 10 days of treatment, respectively. 16.6% (n=29) of patients who had a pain in the anus at high level and 68.6% (n=120) at medium level, feeled no more pain after one day and 10 days of treatment, respectively. Anal ulcer occurred in all participants. The number of patients who no longer suffered from ulcer after 10, 15 and 20 days of treatment was 61 (n=34.9%), 136 (77.7%) and 164 (93.7%) respectively. The treatment provided remarkable results, while the practice was rather simple, no high technique equipment was required
