Suicidal attempts using an overdose ingestion of izoniazid has become an increasing health problem. In our own institution, majority of suicidal cases took INH, being an over the counter drug. Hence, it is recommended that the drug be dispensed only with prescription. Early diagnosis and proper management can lead to high survival rate. (Conclusion)

Background: Rifampicin, isoniazid, and pyrazinamide are oral essential anti-tuberculosis drugs on single or combined preparations. Worldwide research has shown that the plasma concentration of anti-tuberculosis drugs with daily therapeutic doses were seen significant lower than permitted in tuberculosis patients, especially for rifampicin and isoniazid. Objective: To investigate plasma concentration of rifampicin, isoniazid, and pyrazinamide in pulmonary tuberculosis and pleural tuberculosis patients. Methods: Determine plasma concentration of rifampicin, isoniazid, and pyrazinamide at 2 hours after administration in 168 tuberculosis patients by the HPLC method. Identify prevalence of low plasma concentrations of anti-tuberculosis drugs. Results: There was a wide range of plasma concentration of rifampicin, isoniazid, and pyrazinamide in the tuberculosis patients. The mean plasma concentration of rifampicin was 6.13 \xb1 4.66 microgram/ml, of isoniazid was 2.99 \xb1 1.94 microgram/ml, pyrazinamide was 38.98 \xb1 18.39 microgram/ml. There was no significant differences in the plasma concentration of rifampicin, isoniazid, and pyrazinamide in groups of pulmonary tuberculosis and pleural tuberculosis patients. Percentage of patients with plasma concentration below therapeutic concentration was 76.83% of rifampicin, 51.85% of isoniazid, 10.13% of pyrazinamide. There were 12.03% of patients who had pyrazinamide concentration higher than the therapeutic range. Conclusions: There was a wide range of plasma concentration in rifampicin, isoniazid, and pyrazinamide of tuberculosis patients. Low plasma concentration of rifampicin and isoniazid are common. It may be necessary to optimize the drug dose by therapeutic drug monitoring, especially in patients with an inadequate clinical response to chemotherapy.

Thus, with the improvement in diagnostic methods, the concept of tuberculosis control has shifted from the care and treatment of the actually ill to the case finding surveys among the apparently healthy. The treatment of the primary infection which is the root source of the tuberculosis problem is being increasingly emphasized. Since the development of improved methods of therapy, death rate from tuberculosis has declined, but this has not necessarily meant a lowered incidence. From a killing disease, TB has now become a chronic wasting illness with sources of contagion to the community. The vigorous and effective treatment of primary tuberculosis is an important link in the chain for ultimate control of the disease. (Conclusion)

Thirty-six cases of leprosy-19 lepromatous, 16 tuberculoids, and neural-were divided into three groups. Dihydrostreptomycin was administered exclusively in the DS group, in combination with promin in the DS-promin group, and with PAS in the DS-PAS groupThe average amount of dihydrostreptomycin given to each patient was 60 grams in the DS group, 66 grams in the DS-promin group, and 71.2 grams in the DS-PAS group. The daily dosage was 0.50 gram for an average period of 4 months to 6 months. An average of 103 ampules of promin and 437 grams of PAS were administered to each patientOut of the 19 lepromatous, 8 acquired the status of CRS (complete regression of skin lesions, bacteriologically negative); two (2) MRS (marked regression skin lesions); two (2) MMRS (moderate to marked regression skin lesions); and one (1) WS (worsening of skin lesions). Out of the 16 tuberculoids, 13 became CRS , one (1) SMRS (slight to moderate regression of skin lesions); two (2) SIS, (slight improvement skin lesions); and the neural case showed no changeA follow-up of some of the cases revealed that they had remained under control even 6 months to 1 year after they had been discarged from the hospital, with the exception of 1 tuberculoid case which showed recurrence of skin lesionsFurther experimentation on the leprotic action of dihydrostreptomycin-either alone or as an adjunct to the sulfones, PAS, or izoniazid-should be carried out to confirm or disprove the encouraging results obtained in this study. A 42-year-old woman without a prior psychiatric history presented with psychotic symptoms that were suspected as related to treatment of tuberculosis with isoniazid. The psychotic symptoms resolved after isoniazid was discontinued. Clinicians should be aware of the possibility of psychotic symptoms with use of this agent.
