BACKGROUND: The visible cutaneous pigmentary response to ultraviolet A(UVA) is immediate, whereas ultraviolet B(UVB)-induced pigmentation appears after a delay of several days. However, some investigators reported that UVA also can induce delayed taniing. OBJECTIVE: In order to determine whether the pigmentation induceil by UVA irradiation is accompanied by melanocytes responses which are the same or different from those following a UVB-indueed tan, we irraiated malignant melanoma cells to UVA or UVB. METHOD: On the 7th day after irradiation of UVA or UVB, we exa nined the cell counts and the melanin content of control and experimental groups. RESULTS: Although a dose of 10 J/cm decreased the number of manignant melanoma cells, there was no significant difference between the control and UVA-exposure griiups. But there was a significant decrease after UVB-exposun. UVB-exposure groups showed a significant increase of melanin cortent. A dose of 10 J/cm of UVA also induced a significant increase of melanin content. CONCLUSIONS: The data suggest that UVA at a clinically relevant dose has a significant effect on human melanocytes. So, UVA very likely plays a role in the increased, melanization associated with delayed tanning.

BACKGROUND: Kerationcytes make up the vast majority of cells within the epidermis. Recent attention has focused on the role keratinocytes may play in the induction of T cell mediated inflammatory responses in skin, particularily because keratinocytes, when activated by immunologic stimuli, express MHC class llAg and secrete cytokines. But in experimentally induced lichenoid tissue reaction by interferon-nu, MHC class ll Ag was not essential for the enhanced T cell trafficking. There is growing evidence that keratinocyte intercellular adhesion molecule-1 (ICAM-1) expression is involved in the epidermal trafficking of T lymphocytes. OBJECT: To investigate the kinetics of expression of the HLA-DR and ICAM-1 on cultured human keratinocytes by recombinant-interferon-nu(IFN) and phorbol myristate acetate(PMA), and the influence of the HLA-DR and ICAM-1 and the stimulation of peripheral blood mononuclear leukocytes(PBML). RESULTS: 1. 1 U/ml of IFN can induce HLA-DR and ICAM-1 on keratinocytes. Expression of both antigens were increased in a dose and exporsure time dependent fashion. But expression of HLA-DR was less sensitive to IFN than ICAM-1 2. ICAM-1 induction was more rapid than HLA-DR. Keratinocytes expressed HLA-DR 6hours after IFN treatment amd increased rapidly after 12 hours. 3. HLA-DR positive keratinocytes were decreased more rapidly that ICAM-1 positive kerationcytes. 4. Proliferations of PBML were slightly inhibited when cultured with keratinocytes which were treated or not treated with IFN. But IFN treated keratinocytes stimulated the PBML more than untreated keratinocytes. Proliferaton of PBML by IFN treated keratinocytes were inhibited by anti-ICAM monoclonal antibody 5. PMA treated keratinocytes stimulated the PBML more than untreated keratinocytes. Proliferation of PBML by PMA treated keratinocytes was inhibited by anti-ICAM monoclonal antibodies and anti-LFA-1 monoclonal antibodies. CONCLUSION: These results suggest that keratinocytes can express not only HLA-DR but also ICAMP1 may play a important role in initiating immunologic response. BACKGROUND: Because of concern about the infrequent occurre icc of tinea pedis tinea manus, we studied the incidence of them. OBJECTIVE: The purpose of the study was to investigate clinical and mycological features of tinea pedis tinea manus. MEHTODS: We reviewed outpatients with tinea pedis tinea minus at the Catholic Skin Disease Clinic from 1976 to 1991. RESULTS: The incidence was 0.08% out of 1,106,246 outpatients in tinea pedis, 0.02% in tinea manus. Interdigital lesions were the most common in tinea pedis. The ratio of male to female patients was 1.46: 1 in tinea pedis, 1.34:1 in tinea manus. The seasonaI pevalance was highest in summer. The species isolated were, in order of decreasing frequency, Trihohyton(T.) rubrum(89.9%), T. mentagrophytes(8.5%), Microgorum(M.) canis (1.6%) in tinea pedis, T. rubrum(77.8%), T. mentagrophytes(18.2%), M. Canis(2.0%), Epidermophyton(E.) floccosam(2.0% in tinea manus. CONCLUSION: We find that, the incidence of tinea pedis has been inc easing in recent years.

BACKGROUND: A definition of granuloma is a focal chronic inflammatory response to tissue injury evolved by a poorly soluble substwice characterized by the accumulation and proliferation of the mono-nuclear histiocytic cells. The accuracy with which rnononuclear cells may be identified in skir. is much improved by the use of both heteroantisera and monoclonal antibodies directed against selected cellular antigens, OBJECTIVE: Our purpose was to examine the staining patterns of anti-lysozyme, anti-a-1-antitrypsin, anti-S-100 protein antibodies, and MAC-387 monoclonal anibody in granulomatous skin diseases. METHOD: We performed imminoperoxidase staining(the labelled str prvidin-biotin peroxidase complex method on the formalin-fixed, paraffin-embedded tissue specimens of granulomatous skin diseases. RESULTS: S-100 protein positive dendritic cells were demonstrated in the granulomatous infiltrates as scattered pattern and MAC-387 positive cells were predominantly found in the center of granulomas, The staining pattern and percentage of positively stained cells of a--antitrypsin were similar to those of lysozyme. A1Pha-1-antitrypsin and lysozyme positive cells w re present in the center as well as lymphohistiocytic infiltrates of granulomas. CONCLUSION: These data sugget that histiocytes are composed of heter igeneous groups of cells such as the mononuclear-phagocyte system and dendritic cell system.

BACKGROUND: Recent clinial observations have suggested that colchicine, which is in frequent use in gout can affect the conneciive tissue metabolism in skin and other ti.ues. OBJECTIVE: This study suggest that further development of colchiine might provide a novel means of modulating collagen gen expression in patients with fibrotic disease. METHOD: We examed the effect of colchicine on the expres, on of collagen, fibronectin and collagenase gene by skin filroblast culture hy Nort.hern and dot-blot iybridization. Result : The rate of transcription of genomic DNA corresponding o type I collagen and libvonectin was reduced in colchicine-treated cultures but collagenase was not. reduced. Canclusion : The microti.ikile disruptive agent, colchicine, reduced the expression of type I collagen and fibronectin in a dose-(lependent manner. This st.udy suppor that colchicine is one of the promising antifibrogenic drugs curvently being tested as a treatment, of hun an fibrotic disease.

BACKGROUND: Althogh drug euption refers to a common dermatological condition, there are numerous holdbacks on the diagnosis. OBJECTIVE: This study atternpts to define the clinical features and causavite drug of the patients with drug eruptions, and to evalarate the diagnostic value of various skin tests. METHOD: 130 patients with drug eruptions were reviewed clinically. In 54 patients, intradermal test, patch test, and scratch patch test were performed using suspected drugs. Result : The exanthematous eruption(53.0%), fixed drug eruptior.(29.2%), and urticaria(6.2%) were the common types of the drug eruption. The common causative agenst of drug eruptions were antibiotics/antimicrobials, antipyretics/NSAIDs, and CNS depressants. 28 paitients out of the 54 patients on skin tests with suspected drugs had positive results in at least one slinest. CONCLUSION: In our study, we were able to obtain somewhat meaningfi 1 results based on data from the combination of various skin tests.

BACKGROUND: Hydrofluoric acid burns are occuring with ever-increiisir g frequency due to the wide use of this acid in industrial fields, such as electronics and semiconductor i idustries. OBJECTIVE: The purpose of thie study was to evaluate the clinical charateristics of hydrofluoric acid burn. This study will give basic information of hydrofluoric acid burn to the dermatologists who are not familiar with hydrofluoric acid. METHOD: Twenty-two patient who were diagnosed as having hydroiluiiric acid burns at koryo General Hospital(1986-1993) were irraluated clinical)y. Forteen were malc ar d eight were female, most of them were in their third decade. RESULTS: The results are sumrnurised as follows 1. Latent periods were variabie from 30 min. to 3 days. Most patients recognized their chemical burn several hours after exposure. 2. Foot & sole (9 cases) and fingers (7 cases) were most frequently in olved. In 7 cases, nails were also involved. 3. Pain was the most. commori iuobjective symptom. 4. Skin lesions showed erytherna, erythematous swelling, erosion, bulla., necrosis, pigmentation and crust, formation, progressii ely. 5. Treatment Wet dressing with Hyami-solution and topical application of ai tibiotics or steroidointment were sufficient in mild case. In severe cases, intralesional injection of calcium glucoante was perfirmed. Debridement was also performed after the progn ssion of tissue damage stopped. Nails were extracted in cases involving nails. CONCLUSION: Hydrofluoric acid burn is characterized clinically by severc progessive tissue destruction and excruciating pain. Dermatologists need to be aware of the clinical features of hydroclucric acid burn and its proper treatment.

BACKGROUND: Vitiligo is a relatively common acquired depigmentating disorder that affects at least 1 % of the population. The mode of transmission has not been clearly,stablished, but either polygenic or autosomal dominant with incomplete penetrance and variable expresion has been proposed. OBJECTIVE: Our purpose was to evaluate the causative factor, variable clinical features and current teratment of generalized vitiligo patients. METHOD: This clinical in vestigation was made with 225 outpatiens of generalized type vitiligo who had visited the Department of Dermatology, Keirnyung Universitv Hospital from January 1987 till December 1991. Results & CONCLUSION: 1. There were 125 female, (55.5%) and 100 males(44.4%). 2. The mean age of onset was 21.4 years(male . 24.3, female: 19.1). 3. The mean age at the first visit was 27.5 years (male : 28.8, female : 26.5). 4. The mean duration of the disease was 10.7 years(male : 9.9, female : 10.9). 5. The mean interval between onset and visiting was 6.1 years (male : 4.5, female : 7.4). 6. The most common sites of involvernent at the first visit was tie face 11.7% (male : 11.4%, female : 11.8%). 7. The most common precipitating factor was trauma(8.8%). 8. The most common previous treatment was sunlight irradiaion after application of topical oxoralene(33.2%). 9. The most common degree of depigmented lesion was 5-10%(64.4%). 10. The most common occupations of the patients were students mals, and housewife females.

BACKGROUND: Vitiligo is riot quite a rare diseasep; it has about 1% prevalence. The cause of vitiligo is not clear, however, in recent studies an autoimmune origin is freqluei itily mentioned. OBJECT: We tried to an lyze some clinical features of vitiligo and relate them with the presence of autoantibodies. MEHTODS: A total of 381 vitiligo patients was included for the analysis of clinical features. A laboratory study included rhumatoid factor, antinuclear antibocoly, antithyroglobulin antibody and antimicrosome antibody. Some 62 patients were examined for opl thmologic changes. RESULTS: One hundred and one(26.5%) of 381 patients exam ned showed at least one of the autoantibodies tested. Twenty nine pateints showed 2 different aitintibodies. The age at aonset of vitiligo in the autoantibody positive group was 6.6 years later than that of the autoantibody negative group. Autoimmune and/or endocrine diseases were more frequinty found among aut.oantibody positive patients. These diseas s included hyperthyroidism, diabetes me litus and alopecia areata. One patient revealed retinal hypoigmentation and showed no autoantibcidics. CONCLUSION: About 9% of vitiligo patients who were autoantiocyte positive had clinical evidence of diseases associated with the autoantibody. However, it is prudent. to xpect that more patients with t.he autoantibody may develop later systemic autoimmune diseases or endocrinopathies. A long term follow-up of these patients seem:, to be very important.

The present study was performed to evaluate the function of the mandible according to the pattern of fracture and treatment methods of condylar fractures of the mandible and help operators in making a treatment plan. Sixty patients (average follow-up period was 7.8+/-9.4 months) who were treated for condylar fracture from June, 2002 to May, 2006 at the Department of Oral and Maxillofacial surgery, Chonnam National University Hospital were reviewed. The common causes of the condylar fracture were traffic accident and fall-down (35.0%). In concomitant injuries, laceration was 46.7% and the fracture of the mandibular symphysis was highest incidence (60.0%). The common site of the fracture was the condylar head (47.8%), followed by subcondyle (36.2%) and condylar neck (15.9%). Under 15 years old patients, the closed reduction was performed in 87.5% out of the patients. All of the condylar fragments were fixed to the mandible with titanium miniplates in cases of open reduction. The mean period of intermaxillary fixation (IMF) was 14.2+/-6.5 days in closed reduction and 10.0+/-4.2 days in open reduction. The old patient with bilateral condylar head fractures, who were treated by closed reduction with IMF for 3 weeks, showed the limitation of mandibular movements. But, there was no significant different results between open reduction and closed reduction with the respect of the Helkimo's mandibular mobility index and clinical dysfunction index (DI). Complications, such as fibrous ankylosis and resorption of the mandibular condyle, were not observed in all patients. These results suggest that the good results can be obtained by closed reduction with proper IMF periods and functional exercise in most condylar fractures of the mandible except severely displaced extracapsular fractures.
