Objective To study the early diagnosis and management of the patients with acute respiratory distress syndrome (ARDS) caused by cytomegalovirus (CMV) pneumonia after liver transplantation.Methods The clinical data of 8 patients with ARDS caused by CMV pneumonia after liver transplantation in our hospital from April 2001 to May 2004 was retrospectively analyzed. All cases were treated with intravenous infusion of gancyclovir, reduced dosage of cyclosporine A or tacrolimus to 1/3~1/2 of baseline and withdrawal of MMF and prednisone. The patients were subjected to breathing machine assist ventilation and nutrition supply.Results Five patients recovered and 3 died. No one developed acute rejection. Conclusions The key of early diagnosis lies in combining chest X-ray or CT scan with clinical presentation. Administration with anti-viral drugs, adjustment of immunosuppressive agents, management with breathing machine assist ventilation and effective nutrition supply are important for the treatment of patients with ARDS caused by CMV pneumonia after liver transplantation.

BACKGROUND:Compared with the conventional composite resin, the 3M Z350 nano-resin has good wear
resistance, physical mechanical properties, and polishing, and exerts a lower irritation to the dental pulp. Besides fil ing materials, a reliable tooth-prosthesis bonding interface is necessary for resin bonded repairs.
OBJECTIVE:To compare the clinical effects of self-etch bonding Adper Easy One and total-etch bonding Single Bond 2 on nano-resin bonding restoration of the anterior teeth.
METHODS:120 anterior teeth with vital pulp, which had defects at the incisal ends and were to be restored with nano resins, were divided into two groups randomly. Two kinds of adhesives, self-etch adhesive and total-etch adhesive, combined with nano-resin were used to restore the teeth. The patients were re-examined immediately, 6 months, 1 year and 2 years after the treatment. The fil ings, teeth and pulps of patients were examined,
including whether the prosthesis and tooth color were coordinated, whether the gap between the prosthesis and the teeth were sealed, whether the surface of the prosthesis was intact with no loose, whether the prosthesis and teeth had no staining and secondary caries, whether the condition of the tooth pulp had hot or cold
stimulation-induced pain.
RESULTS AND CONCLUSION:No significant difference in the fil ing effects was found between the two groups when the patients were re-examined immediately, 6 months and 1 year after the treatment (P>0.05). The pulp
lesions of the self-etching group were fewer than those of the total-etch group 2 years after the treatment (P<0.05). Self-etching group had 1, 6, 0, 2 cases and total-etch group had 0, 2, 1, 2 cases in uncoordinated color, edge seal,
incomplete restoration and secondary caries, respectively. No statistical y significant differences were found in these four aspects between the two groups (P>0.05). The 2-year fol ow up showed a low incidence of pulp lesions and satisfactory clinical performance after 3M Z350 nano-resin working with self-etching bonding system in the nano-resin fil ing of
anterior teeth with vital pulp.

Chemical disinfectants are generally used for virus inactivation and environment disinfection in biosafety laboratory, and the efficacy and evaluation of the disinfection are critical to ensure the laboratory biosafety.However, there is a current lack of applied standard to evaluate the virucidal efficacy of chemical disinfectants in our country.In this paper, a European Union standard“Method and Requirements of Virucidal Quantitative Suspension Test Method for Chemical Disinfectants Used in Human Medicine” was analyzed and a standard transformation scheme has been proposed.It is suggested that the model viruses should be increased from 3 to 6, including the surrogate viruses to substitute highly pathogenic viruses, and that the method to remove the residual chemical disinfectant and the calculation of 95%confidence interval should be incorporated into the standard.The suggestion will improve the scientific and operational standards related to disinfection and sterilization in biosafety laboratory in China.

The pathogenesy of diabetic peripheral neuropathy(DPN) is approached according to collaterals diseases theory in this study,indicated that defi ciency of both vital energy and yin is the chief pathologic foundation and obstruction of collaterals by blood stasis and phlegm is the critical element in DPN.Furthermore,highlight of differentiation of symptoms and signs,therapeutic principle and diagnosis and treatment based on differentiation are illuminated.This study has supplied a new idea for precaution and treatment of DPN.

Objective:To investigate the cellular immunological regulation mechanism of Zhongjiling tablet on the myasthenia gravis(MG) patients.Methods:The myasthenia gravis patients were randomly divided into 2 groups, the curing group and the control group, 30 cases per group. Patients of curing group administered Zhongjiling tablet and prednisone placebo, patients of control group administered prednisone tablet and Zhongjiling tablet placebo. Course of treatment was 12 weeks. The distribution of T lymphocyte subgroup of myasthenia gravis patients was detected by flow cytometry. The content of IFN-?, IL-4 and TGF-? of the patients’ peripheral blood mononuclear in vitro were detected by ELISA kits.Results:After treatment, CD4+T cell percentage and the ratio of CD4+/CD8+ decreased significantly(P0.05). Purpose To investigate the imaging features of lumbar spinal epidural angiolipoma, and to improve the imaging diagnostic capability of the disease. Materials and Methods Four patients with lumbar spinal epidural angiolipoma confirmed by pathology were recruited in the study. CT and MRI images were reviewed and the imaging characteristics including the shape, size, location, density, and signal intensity were analyzed. Results The lesions located at the right front of the spinal canal in two cases, at the left front in one case, and the residual one case located directly behind the spinal canal. The longitudinal axis of the lesions paralleled to the longitudinal axis of the spine. The dura mater spinalis was compressed and inward shifted. All of the four cases showed homogeneous iso- or hypo-density on CT without calcification or necrosis in the lesions. The maximum diameter was 3 to 5 cm. The boundary was clear and smooth. Three lesions showed dumbbell-shaped and crossed foramen, but the adjacent bone were not absorbed or destructed. One lesion showed scallop-like. On MRI, four cases displayed slightly hypointense on T1WI and hyperintense on T2WI. On STIR images, the lesions showed hyperintense with patchy low signal intensity in it. The boundary of the lesions was clear. After administration of contrast media, two lesions presented remarkably homogeneous enhancement, one lesion showed dual tail sign, and one lesion displayed pen-tip-like at the both ends. Conclusion MRI plays an important role in locating the lesion and distinguishing internal tissue components of spinal angiolipoma, which is the gold standard for the diagnosis of the tumor. CT provides excellent supplement. The Combination of CT and MRI will improve the diagnostic accuracy of the spinal angiolipoma.

Objective To establish the extraction process of Zhechong Chuangyu Capsule. Methods The difference of analgesic effect of water extraction and alcohol extraction in mice was observed by body-torsion test to determine the extract solvent. With the rate of aqueous extraction, n-butyl alcohol extraction and asperosaponin Ⅵ as evaluating indicator, the influencing factors including solvent volume, time and times of extraction were investegated to evaluate extracting procedure by orthogonal experiment. Results There was no obvious difference in analgesic effect between water extraction and alcohol extraction. Given the requirement of produce, aqueous extraction was a better choice. The optimum extracting condition was extracted 3 times with 20 folds volume of solvent, and extraction time was 150 minutes. Conclusion The extraction process is feasible to be applied into production.

BACKGROUND:Wnt/β-catenin signaling pathway plays an important role in tooth development and especially in the root development,including the initiation of root development,Hertwig's epithelial root sheath formation,root odontoblast and cementoblast proliferation and differentiation process.OBJECTIVE:To explain the effect of Wnt/β-catenin signaling pathway on tooth root development.METHODS:CNKI and PubMed databases were searched for the literatures concerning odontoblast and cementoblast proliferation and differentiation as well as Hertwig's epithelial root sheath formation mediated by Wnt/β-catenin signaling pathway during root development.Finally 41 eligible articles were enrolled according to the inclusion and exclusion criteria.RESULTS AND CONCLUSION:During the tooth root development,Wnt/β-catenin signaling pathway can regulate Hertwig's epithelial root sheath integrity,so the inhibition of signaling pathway will block Hertwig's epithelial root sheath formation,further inducing the failure in root odontoblasts and cementoblast differentiation.Loss or overexpression of the signal transduction pathway will inhibit the proliferation and differentiation of odontoblasts,and appear with root dentin formation disorder.Effect of β-catenin signal on cementum differentiation can be regulated by cell origin or developmental stage,surrounding microenvironment and Wnt-related activator and inhibitor concentration,thereby promoting or inhibiting cementoblasts to different exents.

Objective To evaluate the curative effect of stress urinary incontinence (SUI) with Tconsling suspension surgery. Methods From May 2010 to September 2011,we treated 9 cases SUI with T-consling suspension surgery.The average age was 51 years (range 45 -63 years).They all suffered from stress incontinence 1 -5 years. Urodynamic test were performed preoperatively and stress urinary incontinence was confirmed.Abdominal leak point pressures is between 75 - 103 cm H2O (average 87 cm H2O),and no obvious cystocele vaginalis and others pelvic organ prolapsed disease were detected.Operation was done by transvaginal front wall incision to place T-consling suspension tape beneath the middle urethra.Results Operation time were between 15 to 25 mins,with 17 minutes on average.EBL was 5 - 30 ml.Urinary catheters were drawn 3 -4 d postoperatively.In 9 cases,8 were cured,one improved.The improved case had leakage when abdominal pressure increases,but became less significant,and the other cases had no leakage when abdominal pressure increases.The short-term cure rate was 89％. Conclusions The Tconsling suspension is safe,effective and easy to perform for the treatment of SUI,however,longer follow-up and more cases are necessary to confirm the long-term effects of this procedure.
