2.Analyzing the risk factors and related psychological factors in patients with gastroesophageal reflux disease and surveying the curative effect

Objective To analyze, the risk factors and related psychological factors in patients with gastroesophageal reflux disease (GERD)and survey the curative effect. Methods All of 200 outpatients with GERD(GERD group) and 200 healthy persons (control group)from January 2006 to January 2008 were randomly selected, and the personal data were investigated. The GERD questionnaire (RDQ) and the Zung-self-rating depression scale (SDS) test were used. Results In GERD group, the proportions of high education, city living, marriage instability ,heavy working pressure, high body mass index (BMI),smoking and drinking habit, peptic disease in family, using nonsteroidal anti-inflammatory drugs (NSAIDs) were higher than those in control group (Pall < 0.01). After treatment, the patients in GERD group had lower scores in the RDQ and SDS test than those before treatment (P< 0.01). Conclusions There are many risk factors in patients with GERD, especially the psychological factors. The therapy of GERD should combine with psy-chotherapy and avoid high BMI, smoking, drinking and overusing the NSAID.
3.Relationship of Adiponectin and Insulin Resistance in Patients with Chronic Kidney Disease

Objective To investigate the changes of serum and urinary adiponectin (ADPN) levels and insulin resis-tance (IR) states in patients with chronic kidney disease (CKD), and to explore their relationship thereof. Methods A total of 487 patients with CKD stages 2-5 were enrolled in this study, and 30 healthy subjects were served as control group. The se-rum ADPN levels in urine samples were examined by ELISA. The level of fasting insulin (FINS) was detected by radioimmu-noassay. Blood routine test, liver and kidney functions, blood glucose, serum lipids, 24 h urinary protein excretion and endoge-nous creatinine clearance rate (Ccr) and body mass index (BMI) were observed and calculated. The differences of ADPN lev-els in serum and urine samples and homeostasis model assessment for insulin resistance (Homa-IR) were compared between groups. Results The serum and urine ADPN levels and Homa-IR were higher in CKD patients than those of controls (P<0.05). With the decline in renal function, the ADPN and Homa-IR levels were increased gradually (P<0.05). The value of se-rum ADPN was significantly higher in patients with CKD stages 3-5 and high Homa-IR. The ADPN levels and Homa-IR were positively related to lipid parameters and 24 h urinary protein, and negatively correlated with hemoglobin and serum al-bumin in patients with CKD (P < 0.05). Conclusion CKD patients had higher ADPN level and more significant IR. The ADPN and IR were correlated with serum lipids, hemoglobin, albumin and urinary protein. Dynamic monitor of ADPN level may have clinical significance in judging metabolic disorders in CKD patients.

7.Relationship between expression of serum vascular endothelial growth factor and efficacy of local advanced esophageal carcinoma treated with intensity-modulated radiotherapy combined with chemotherapy

Objective To investigate the expression of serum vascular endothelial growth factor (VEGF) in local advanced esophageal carcinoma patients,and to explore the relationship between its change rule and the recent curative effect and significance.Methods The serum levels of VEGF in 46 patients with local advanced esophageal carcinoma were detected by ELISA before and after concurrent chemoradiotherapy,the serum levels of VEGF in 46 healthy people were detected at the same time.Results The serum VEGF levels of local advanced esophageal carcinoma patients before treatment and health control group were (264.66±62.11) ng/L and (101.96±39.53) ng/L,and the difference was statistically significant (t =23.367,P =0.000).After treatment,the serum VEGF level of patients with esophageal cancer was decreased to (127.50±49.07) ng/L (t =19.086,P =0.000).After the treatment,the serum VEGF levels of complete remission (CR) and partial remission (PR) patients were decreased [(94.03±38.86) ng/L vs (248.79±47.84) ng/L,(117.06±20.68) ng/L vs (295.26±67.20) ng/L,both P =0.000],the level of the stable disease (SD) patients was decreased,too [(235.63±26.05) ng/L vs (385.67±26.50) ng/L,P =0.017].Conclusions The recent curative effects of docetaxel and nedaplatin chemotherapy combined with intensity-modulated radiotherapy for the local advanced esophageal carcinoma patients are significant.The levels of serum VEGF before and after treatment can be used as an important index to judge the treatment effect.
8.Nursing research in peripherally inserted central catheter related infection in patients with tumor

Objective To analyze the causes of peripherally inserted central catheter (PICC) relat-ed infections, study the types of pathogens and their drug sensitivities in order to supply evidence for clini-cal medication. Methods 110 cancer patients were analyzed for PICC related infections and blood sam-pies underwent bacterial culture. Results Of 110 cases of cancer patients, catheter-related infections were found in 11 patients (10.00%), including 9 cases of local infection (8.18%), and 2 cases of hemato-ge-nous infection (1.82%). Catheter-related infections were not associated with gender, age, types of catheter, venous selection, and the inserted length of intravenous catheter. Catheter- related infections were likely to be associated with the indwelling time of catheters. Conclusions To avoid the infections related to long-term PICC, strict aseptic operation and standardized maintaining procedures are required.
9.Extracorporeal shock wave: An effective and safe therapy for the pain symptom of type IIIB prostatitis.

OBJECTIVETo investigate the effect and safety of extracorporeal shock wave (ESW) in the treatment of pain symptom of type III B prostatitis.
METHODSWe treated 50 cases of type III B prostatitis by ESW once a week for 4 weeks. Then we evaluated the clinical effect and safety of the therapy based on the NIH-CPSI scores, visual analogue scale (VAS) scores, IIEF-5 scores, prostate volume and morphous, state of urination, color of urine, results of routine semen analysis, and changes of cytokines (IL-6, TNF-α and IL-1β) in expressed prostatic secretion (EPS).
RESULTSAll the patients successfully accomplished the treatment. Compared with the baseline, decreases were observed after 4 weeks of cytokine treatment in the pain scores (14. 61 ± 1. 82 vs 9. 36 ± 1. 47, P <0. 01), urination symptom scores (4. 59 ± 1. 01 vs.4. 66 ± 0. 89, P >0. 05) , quality of life scores (6. 51 ± 1. 03 vs 4. 56 ± 1. 02, P <0. 01), NIH-CPSI (25. 43 ± 1. 72 vs 18. 28 ± 2. 32, P <0. 01 ), and VAS (6. 59 ± 1. 10 vs 3. 02 ± 1. 07, P < 0. 01). The concentration of IL-6 in the EPS was significantly increased ([55.82 ± 6. 28] vs [86.59 ± 4. 55] ng/ml, P <0. 01) , while the level of TNF-α ([3.89 ± 0. 12] vs [3. 19 ± 0.22] ng/ml, P<0.01) and that of IL-1β ([3.21 ± 1.01] vs [1.48 ± 0.95] ng/ml, P< 0. 01) remarkably reduced after treatment. However, there were no statistically significant differences in IIEF-5 scores (18. 58 ± 2. 03 vs 18. 51 1. 89, P >0. 05) or various sperm parameters before and after treatment (P >0. 05). And no significant changes were observed in the prostate volume, morphous or internal echoes.
CONCLUSIONThe ESW therapy is effective and safe for the pain symptom of type III B prostatitis.
