1.A Case of Allergic Contact Dermatitis to Cenasert® Vaginal Suppository.

Sensitization to Cenasert®, a broad spectrum antimicrobial for the treatment of leukorrhea, is rare. Perineal contact dermatitis due to vaginal suppositories has never been reported in the medical literature. We report a 31-year-old woman, who developed perineal skin eruptions following the treatment of leukorrhea with Cenasert® vaginal suppositories. Patch tests showed positive reactions to Cenasert® suppositories and to the three active ingredients of Cenasert®. Vaginal suppositories can sensitize not only the vagina but also the surrounding perineum due to leakage of the allergen from the vaginal vault.

2.Successful Desensitization of A Case of Anaphylaxis to Insulin.

A small proportion of diabetics treated with exogenous insulin, reportedly ranging from 0.1% to 2%, experience systemic allergic reactions to insulin. We saw a 55-year-old male patient with insulin-dependent diabetes mellitus who had developed an anaphylatic reaction immediately after subcutaneous injections of neutral protamine Hagedorn (NPH) human recombinant insulin. Skin tests with insulins and a passive transfer test demonstrated that his reaction was IgE mediated. Since there were no therapeutic alternatives, desensitization with insulin was done following a slow protocol. It was successful and he was able to resume insulin therapy. During two year follow-up period, he did not developed any adverse reaction to insulin.

3.Two Cases of Cement Burn.

Wet cement can cause caustic ulcers due to its alkalinity. Besides caustic ulcers frequently called cement burn, allergic and irritant contact dermatitides are common in cement workers. Alkalinity of calcium hydroxide in wet cement combined with the effect of pressure and occlusion against skin may result in cement burn. We report two cases of cement burn due to their occupational exposures.

4.Two Cases of Allergic Contact Dermatitis to Povidone-iodine.

Povidone-iodine(PVP-I) is a widely used antiseptic for the skin because of its low toxicity and high germieidal efficacy, Allergic contact dermatitis to PVP-I is known to be rare. Two patients who recently underwent surgical operations were referred to us due to pruritic well-demarcated eczematous skin lesions around the wounds. Their operation wounds had persisted in spite of the treatments using systemic antibiotics and topical antiseptics. Patch tests were performed with antiseptics including PVP-I, hydrogen peroxide, and benzalkonium chloride which had been used to dress their wounds. The patch tests resulted in positive reactions to PVP-I at the concentrations of 1% in one patient and 0.01% in another.

5.Two Cases of Lvmphomatoid Papulosis.

Lymphomatoid papulosis(LyP) is a chronic recurrent dermatosi characterized by involuting and recurring papules, plaques, and nodules showing histologic feaurs suggesting rnalignant lymphoma. On histologic ground, it is divided into two types. type A and type B. In type A, large atypical lymphocytes are the main cellular cornponent and most of these express CD 30(Ki-1) antigen, which has been p!reviously thought to be specific for Read Sternberg cells of Hodgkins disease. In type B, cerebriform mononuclear lymphocytes sirr la to those in mycosis fungoides predominates. We report two cases of lymphomatoid papulosis, type A and type B, respectively. The large atypical cells in type A LyP expressed Ki 1 antigen. Both case were treated with PUVA and the outcome of the treatment has been fair.

BACKGROUND: The main function of melanocyte is known to proiect the skin from hazardous sun-light. But, some investigators have claimed lately that melanocytes are also related to the immunologic role in the epidermis becauase these cells produce IL-1 activity and IL-lb convertase activity, in vitro. OBJECTIVE: Our purposee were to investigate the effects of rIFN-b on the proliferation of melanocytes, melanin content, and the expression of HLA-DR aritign on melanocytes after a rIFN-y exposure. MEHTODS: The number of melanocytes, the melanin content, and the expression of HLA-DR antigen were evaluated on culturect human melanocytes according to a time sequence and various concentrations of rIFN-y. RESULTS: Antiproliferative activity on melanocytes was dependent on the exposure time and the concentration of rIFN-r. According to the exposure time and the concentration of rIFN-r, melanogenic activity was inhibited or stimulated, Normal melanocytes didnt express HLA-DR antigen, but when normal melanocytes were exposed to rIFN-r, the expression of HLA-DR antigen increased in a timeand concentration-dependent fashion. After the removal of rIFN-r fiom the culture media the expression of HLA-DR antigen on melanocytes also disappeared. CONCLUSION: In our study, melanocytes seem to be related to the irnmunologic role in the epidermis because these cells expressed HLA-DR antigen after rIFN-r exposue and we think that study could help to investigate between melanocytes and immunalogic mechanisms in various inflammatory skin diseases.

8.A case of pigmented spindle cell nevus.

We report a case of pigmented spindle cell nevus occurred in a 4-year-old girl. The lesion was well-demarcated, 2.5 x 3mm sized black macule on the left thenar area. The histopathologic findings of excisional biopsy specimen revealed the prociferation of uniform spindle-shaped pigmented melanocytes at the dermoepidermal junction and sharply definded lateral margins.

10.A case of fibroma of tenden sheath.

We report a case of fibroma of tendon sheath developed in a 22-year-old woman. The tumor of about 5 years duration was protruding on the volar surface of her 4th finger tip. It was well-demarcated rubbery-hard, and immovable on palpation. The surface on the tumor was smooth and had some telangiectasias. It was asymptomatic and non-tender. The histopathologic findings of excised specimen revealed well-demarcated lobulated mass with widely-spacd: fibroblasts in a hyalinized collagenous matrix. The characteristic slit-like spaces were found in hypoellular and fasciitis-like region. After excision, local recurrence has not been observed for about one and a half years of follow-up period.
