1.Application of autologous platelet-rich plasma in spinal surgeries

BACKGROUND: Autologous platelet-rich plasma (PRP) is a concentrate of platelet-rich plasma protein derived from autologous whole blood. The activated platelets can release a large number of high concentrations of growth factors and bioactive substances, which provides a theoretical basis for its application in various disciplines and fields. At the same time, autologous PRP has been popularized because it will not cause immune rejection with extensive sources, easy to extract and low cost.OBJECTIVE: To summarize the preparation method and performance of autologous PRP, and to explore its latest research results and underlying mechanisms in spinal surgeries.METHODS: A computer-based online retrieval of CNKI and PubMed databases was performed for articles concerning autologous PRP used in spinal surgeries. Finally, 42 eligible literatures were included according to the inclusion and exclusion criteria.RESULTS AND CONCLUSION: (1) Autologous PRP is rich in a variety of growth factors and bioactive substances, so it has been used in the fields of disc repair, bone conduction and osteoinduction, and its effectiveness has been confirmed through abundant basic and clinical trials. (2) Meanwhile, autologous PRP also is used for tissue repair and skin regeneration, such as spontaneous cerebrospinal fluid leakage and pressure ulcer. (3) Notably, autologous PRP has shown a promising prospect in the field of spinal surgeries.
2.Therapeutic effects of recombinant human growth hormone on rat sepsis

AIM: To investigate therapeutic effects of recombinant human growth hormone(rhGH) on rat sepsis and its possible mechanisms. METHODS: Mean arterial pressure (MAP), levels of plasma TNF?,IL-1? and endotoxin,leukocyte count and survival rate within 1 week were determined after E. coli injection among control group,sepsis group and sepsis+rhGH group. RESULTS: (1)rhGH diminished the decrease of MAP, reduced plasma endotoxin and TNF? levels and increased neutrophil ratio in total leukocytes in sepsis rat. rhGH increased survival rate within 1 week on sepsis rat. (2)No changes were found in IL-1? level among the three groups. CONCLUSION: rhGH showed desirable beneficial effects on rat sepsis, which may attribute to: improving circulatory function; maintaining intestinal mucosa barrier, attenuating bacteria/endotoxin translocation and inhibiting the production and release of TNF?.
4.The diagnostic value of colonoscopy for chronic diarrhea

Objective To evaluate the diagnostic value of colonoscopy for patients with chronic diarrhea.Methods Data of 2449 patients with chronic diarrhea who underwent colonoscopy from January,1999 to December,2008 were reviewed.A total of 2110 patients who underwent colonoscopy screening for health checkup during the same period were used as controls.The rates of clinic-relevant abnormal endoscopic findings and negative finding were compared between two groups.Results Lesions with clinic significance were found in 44.1% of patients with chronic diarrhea (1080/2449) and in 41.7% of controls (870/2110,x2 =2.756,P =0.097).Compared with controls,incidence of non-IBD and noninfectious colitis (x2 =58.578,P < 0.001),IBD (x2 =59.609,P < 0.001),malignant tumor (x2 =21.649,P <0.001),terminal ileitis (x2 =6.275,P =0.012),infectious colitis (x2 =17.019,P <0.001),intestinal tuberculosis (x2 =7.021,P =0.008),melanosis coli (x2 =6.040,P =0.014) and parasitic infection (x2 =4.245,P =0.039) were all significantly higher in patients with chronic diarrhea.However,incidences of adenomatous polyps (x2 =14.124,P < 0.001),non-adenomatous polyps (x2 =33.427,P <0.001) and diverticular disease (x2 =9.921,P =0.002) were significantly higher in the control group.There was no significant difference in incidences of the benign tumor (x2 =1.627,P =0.202) and angiodysplasia (x2 =0.231,P =0.631) between the two groups.The overall screening rate of colonic polyps,diverticulosis,and vascular lesions was 37.3% in chronic diarrhea group.Conclusion Colonic polyps,diverticulitis,benign tumors and angiodysplasia may not be the causes of chronic diarrhea.Etiology of more than 1/3 patients with chronic diarrhea remains unknown after colonoscopy.
5.Analysis of curative effect of elder C type tibial plateau fractures patients with open reduction and internal fixation with bilateral plate

Objective To analyze the results of treatment of complicated tibial plateau fractures with open reduction and internal fixation with bilateral plates .Methods 29 elder patients with C type tibial platform fractures underwent internal fixation with bilat-eral plates and bone grafting .Fixation with bilateral plates was performed during operation with dynamic compression plate or 1/3 Tubular steel plate placed medially and the dissection plate or LISS system ones inserted laterally after the underlying separation . Meanwhile ,sufficient bone grafting during operation could effectively support reduction of articular surface .Results All patients were followed up of an average of 13 .8 ± 2 .43 months .According to Rasmussen score standard ,the excellent and good rate was 82 .7% .There was significant difference of TPA and PA between postoperation and postoperation one year later .Conclusion It is a simple and effective method to treat complicated elder tibial platform fractures with bilateral plates .It will improve the effect and re-duce complications significantly to perform sufficient bone grafting intraoperatively ,and make the drainage unobstructed and do the early functional exercises postoperatively .
6.PDCA circle in training of endoscopic nurse

Objective To evaluate PDCA circle in training of cooperation quality in endoscopic nurse.Methods PDCA circle was applied to train endoscopic nurses.Knowledge of endoscopic nursing and techniques as well as operators' satisfaction with endoscopic examination were compared before and after PDCA circle training.Results Nursing scores increased from 92.40 to 96.00 with statistical significance (P < 0.05).Performance scores increased from 93.03 to 98.13 (P < 0.05).Operators' satisfaction increased from 81.3% to 96.9%.Conclusion PDCA circle could improve not only endoscopic nursing but also the quality of endoscopic training.
7.CT findings and dynamic changes of COVID-19 in the patients younger than 18 years old infected with SARS-CoV-2 Omicron variant

navirus disease 2019 (COVID-19) in the patients younger than 18 years old infected with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) Omicron variant, and to provide a basis for determining the chest CT changes and efficacy of COVID-19 caused by Omicron virus variant in patients younger than 18 years old. Methods The clinical and imaging data of 30 cases of patients younger than 18 years old infected with COVID-19 Omicron variant, who admitted to the Third People's Hospital of Shenzhen from February 11 to March 26, 2022 were collected and retrospectively analyzed. The clinical manifestations, imaging features and dynamic changes of lesions were summarized. Results A total of 41 intrapulmonary lesions in 30 patients with COVID-19 caused by SARS-CoV-2 Omicron variant. The main manifestations were patchy or nodular ground-glass opacities and/or consolidation, with focal subpleural distribution, lesions mainly occur in the right lung (70.73%, 29/41). There were 42 lesion morphologies, with 22 (52.38%) striped shadows and 16 (38.10%) nodular shadows, with small lamellar and patchy shadows predominating. There were 36 lesion density variations, with ground glass shadows being the most common, with a total of 24 ground glass shadows (66.66%) in each lobe of the lung, and also 6 consolidation lesions (16.67%) and 6 mixed ground glass opacity and consolidation lesions (16.67%). With the progression of the disease, lesions gradually enlarged, appeared on the 2nd day (312.93 mm3), peaked on the 9th day (1 837.18 mm3). The average absorption time of the lesions was (16±3) days, and there was no significant difference between the absorption time of patchy and nodular lesions (ground glass and/or consolidation) (t=0.853, P>0.05). The lesions showed focal ground-glass opacity in the early stage, 77.78% lesions were absorbed after treatment in the late stage. Inflammatory nodules were absorbed slowly (9-19 days), without residual fibrotic changes. Conclusions The imaging manifestations of COVID-19 in patients younger than 18 years old infected with SARS-CoV-2 Omicron variant have certain characteristics, showed patchy or nodular ground glass opacities and/or consolidation, mainly distributed in the subpleural area, with small and few lesions and slow change, didn't remain fibrosis. Being familiar with its clinical and imaging manifestations can assist in early diagnosis, but confirming the diagnosis requires a combination of epidemiological history, clinical symptoms, SARS-CoV-2 nucleic acid and radiological manifestations.
8.Plasma endothelin-1 and nitric oxide correlate with ligustrazine alleviation of pulmonary artery hypertension in patients of chronic cor pulmonale from high altitude plateau during acute exacerbation.

OBJECTIVETo explore the mechanisms involved in the ligustrazine alleviation of the pulmonary artery hypertension (PAH) in patients of chronic obstructive pulmonary disease (COPD) associated with chronic cor pulmonale (CCP) during exacerbation.
METHODSSeventy patients of COPD and CCP with acute exacerbation were randomly and equally divided into control group and treatment group. The control group received standard treatment with antibiotics, antiasthmatic and expectorant medications, and oxygenation; and the ligustrazine treatment group received ligustrazine treatment (80 mg/d; i.v.; for 2 weeks) in addition to the standard treatment. Before and at the end of 2 week treatment, the clinic responses of the two regimens were evaluated, plasma levels of endothelin-1 (ET-1) and nitric oxide (NO) were determined; arterial oxygen partial pressure (PaO2, mean pulmonary arterial pressure (mPAP), outflow tract of right ventricle (RVOT), and internal diameter of right ventricle (RV) were measured.
RESULTSGood clinic benefits were achieved in both the standard and ligustrazine regimens, plasma level of ET-1, values of mPAP, RV and RVOT decreased significantly, plasma level of NO and PaO2 values decreased (all P < 0.01 vs pre-treatment to all parameters). Compared with the control group, ligustrazine greatly enhanced the clinic efficacy from 77.1% to 97.1% (P < 0.05), and also resulted in more significant changes of all these parameters (P < 0.01 vs control group for all parameters). For both groups, the levels of plasma ET-1 were positively correlated with values of mPAP, RVOT, and RV (r = 0.710, 0.853, and 0.766, respectively, all P = 0.000), and negatively correlated with plasma NO and PaO2 (r = - 0.823, and - 0.752, respectively, all P = 0.000).
CONCLUSIONLigustrazine is effective in treating pulmonary artery hypertension during acute exacerbation of COPD and CCP in patients from the plateau area. The observed changes in the plasma levels of NO and ET-1 in response to ligustrazine treatment suggest that ligustrazine may act through the selective effect on pulmonary blood vessels to enhance the synthesis and release of NO and suppress those of ET-1 from lung vascular endothelial cells, thus reducing pulmonary artery pressure and decreasing pulmonary arterial hypertension.

9.Analysis of epidemiological and clinical features of adenovirus infection in 80 children with acute respiratory tract infection.

By analyzing the epidemiological and clinical features of adenovirus in children with acute respiratory tract infection (ARTI), we provide a theoretical basis for early clinical diagnosis and treatment. Nasopharyngeal secretions were collected from 3480 children with ARTI, who were hospitalized at the No. 2 Hospital of Changzhou from January 2011 to December 2012. Adenovirus were detected using direct immunofluorescence assays. A total of 80 samples were positive for adenovirus (2.30%). The rate of adenovirus infection during 2011 was significantly higher than that in 2012, and the infection rate was higher in summer and autumn than in winter and spring. The infection rate was 1.14% among children aged < 1-year-old and the rates were higher among children in other age ranges. Adenovirus was found to be an important ARTI pathogen in children in Changzhou, mainly affecting children older than 1 year. ADV infections have various clinical presentations, but affected children tend to be severely ill with poor outcomes.

10.Expression and significance of erythropoietin and its receptors in rats with traumatic brain injury

Objective To study the expressions of erythmpoietin(EPO)and its receptors(EPOR)in the injured brain tissue ofthe rats with traumatic brain injury(TBI).Methods A total of78 SD rats were randomly divided into three groups including control group(six rats),sham group(36rats) and fluid percussion injury group(36 rats).The rats were sacrificed at 6,24 hours,3,5,7 and 14days after TBI in the sham group and the fluid percussion injury group(six rats at each time point).Then,the injured brain tissues were removed for observation of the mRNA and protein expressions of EPO and EPOR by meaDiB of real-time PCR and Western blot. Results The expression of EPO was increased at 24 hours and reached the peak at day 3 after TBI.The hish expression level of EPO could maintain for two days or so.began to decrease at day 7 and recovered to normal at day 14 after Till.While the expression of EPOR reached the peak at 24 hours after TBI and maintained hish level at day14. Conclusions The expressions of EPO and EPOR show increase within 24 hours after TBI.In fact,the expressions of both factors are not in consistency,with more transient expression of EPO.
