OBJECTIVE To lower the incidence of postoperative pulmonary infection in abdominal major operation.METHODS We performed a retrospective trial analysis on pulmonary infection after major abdominal operation in 402 cases from Oct 2004 to Oct 2005,and in the meanwhile effective preventive procedures were conducted.RESULTS Thirty three cases with pulmonary infection went weel with active treatment and nursing.CONCLUSIONS The predisposing factors for postoperative infection include anesthetic method,incision pain,and environmental pollution.The better methods for prevention of pulmonory infection are good preoperative instruction,planned respiration training,keeping airway confluent postoperatively,effectively reducing incision pain,infection control,and preventing environmental and air pollution.

Objective To determine the bacterial composition and resistance of wound infections in China. Methods Disc diffusion test,MIC test and E-test were used to detect the antimicrohial resistance of bacterial isolates from wound secretions.WHONET 5.4 was applied for analysis of the bacterial sensitive data from 86 domestic tertiary hospitals from June 1,2006 to May 31,2007. Results (1)2125 bacterial strains were coileeted in the survey period,which included 994 strains (46.8%) of Gram positive and 1131 strains (53.2%) of Gram negative bacteria,Staphylococcus(780 strains,36.7%),E.coli(338 strains,15.9%) and Pseudomonas aeruginosa (231 strains,10.9%)were the most common isolates.(2)38.2% and 84.1% of Staphylococcus aureus and Staphylococcus epidermidis were methicillin-resistant.respectively.No Staphylococcus strain was resistant to vancomycin or teicoplanin.(3)ESBLs positive rate of E.coli was about 60%,while the resistant rate to quinolones Was about 70%.(4)The resistant rates of Staphylococci and E.coli isolated from adults to aminoglyeoside and quinolones were higher than those from children.(5)The resistant rates of Staphylococei to cephalosporin,aminoglyeoside and quinolones from inpatients were higher than those from out-patients. Conclusion Staphylococci,E.coli and Pseudomonas aeriginoso were among the most common organisms isolated from wound infections.The ESBLs positive rate of E.coli from wound infections was higher than that from all specimen bacteria found in other surveys performed during the same period and the methicillin resistant Staphylococci were less than that.

Objective To study the efficacy and safety of selegiline in the treatment of patients with Parkinson's disease (PD) at early stage. Methods The randomized and blank controlled study was performed. Sixty PD patients at early stage who had been treated with trihexyphenidyl, amantadine and vitaminE were divided into two groups. One was added to selegiline hydrochloride (5 mg/d) for eight weeks, the other was blank controlled group. Efficacy of selegiline in each patient was evaluated by modified Webster scale. Phosphate-dissolving microorganisms are widely distributed in soil, rhizosphere and other ecological environment. Understanding the characteristics of these microorganisms in solubilizing phosphates is helpful to apply them in improving P use efficiency. The obtained results indicated that the fungi had much higher capacity to dissolve the rock than the bacteria. Existence of Fe, Al, Mg and Na in the culture media reduced the rock solubilization by the bacteria, but increased the solubilization of the fungi. The higher content of the rock in the media, the lower capacity of the rock phosphate solubilization was found. The capacity was also significantly reduced if the concentration of C material in the media was higher than 3%. It was also found that the microorganisms destroyed the rock structure. The P was more easily released from the rock at further incubation. In conclusion, there is some potential to utilize the microorganisms to activate the rock phosphate.

Objective To investigate the clinical characteristics and mutations of guanosine triphosphate eyclohydrolase (GCH) Ⅰ gene in patients with dopa-responsive dystonia (DRD). Methods Five families with 18 affected family members and 17 patients with sporadic DRD were examined. Patients were allocated into 3 groups according to onset time, either in childhood, or in adolescence or adult. Interview, physical examination, psychologic testings and CT/MR scan were performed. Mutation screening was performed on 26 patients and 1 normal family nember. Thirty-five healthy control subjects were matched for age and sex. Statistical analysis were conducted with the use of SPSS 13.0 computer software. Results(1)Most of patients started with dystonia. The main clinical manifestation was dystonia too. There was no difference among 3 groups.(2) There were significant differences in diurnal fluctuation among 3 groups(15/15,6/6,7/14, χ2=13.125,P=0.001). Diurnal fluctuation negatively correlated with age (r=-0.720, P<0.01).(3)The differences in postural tremor were also found among 3 groups (7/15,5/6,1/14, χ2=8.073, P=0.018). Postural tremor positively correlated with age (r=0.399, P=0.018).(4)There were differences in exaggeration of tendon among three groups(11/15,1/6,4/14, χ2=8.309, P=0.016). Exaggeration of tendon reflexes negatively correlated with age (r=-0.429, P=0.010).(5)The scores of Hamilton Depression Scale and Hamilton Anxiety Scale in patients were higher than those in controls.(6)DNA sequencing revealed a heterozygous A224G missense mutation(Tyr75Cys)located within exon 1 in one autosomal dominant inheritance family. Conclusions The manifestations of DRD varies. The clinical course is closely correlated with age. A missense mutation(A224G)in coding region of the GCH 1 gene probablyleads to the occurrence of DRD.

The nuclear factor erythroid 2 related factor 2 (Nrf2) is a key protein of endogenous antioxidant defense systems in the body. In response to oxidative stress, Nrf2 translocates to nucleus and binds to antioxidant response elements (ARE), regulating the expression of a large amounts of antioxidant genes and maintaining a proper redox balance. The pathological processes of neurodegenerative diseases are associated with generation of reactive oxygen species, which cause oxidative stress. Oxidative stress plays a cardinal role in the onset and progression of neurodegenerative diseases. Nrf2-inducer compounds can reduce oxidant stress and have shown therapeutic efficacy in many neurodegenerative disease models. How to activate the Nrf2 signaling pathway effectively has been received much attention. Here we provided an overview of specific mechanism of Nrf2-ARE pathway and the protective effects of Nrf2 in different neurodegenerative diseases, and summarized the Nrf2 activators recently in preclinical study.
