1.The application of case management based WeChat platform in the psychological intervention of patients with laryngeal cancer

Objective To explore the significance of case managers using WeChat platform in the treatment process and extended care for patients with laryngeal cancer.Methods From November 2014 to May 2015, 30 patients with partial resection or total resection of laryngeal cancer in the department of head and neck surgery were selected as a research subjects.Case managers provided fully supports,such as establishing WeChat group,replying online consultations,popularizing health education knowledge,providing psychological counselings.Psychological assessment of the patients was conducted by social support scale(SSRS)and anxiety scale(SAS)in the beginning of case management.Results comparison was performed before and after three months intervention.Results The score of subjective support of patients before intervention was (25.13 ±4.81)points,which after intervention was (28.33 ± 4.41)points.The score of support availability before intervention was (7.20 ±1.99)points,which after intervention was (35.27 ±5.90)points.The differences were statistically significant(t =-2.685,-5.280,2.917,all P <0.05).Conclusion The WeChat platform mediated by the case management is effective in the management of the psychological health of patients with laryngeal cancer,which can effectively promote the rehabilitation of patients.

3.Wall shear stress distribution in carotid sinus of type 2 diabetes mellitus patients based on contrast enhanced flow field

Objective To investigate the application of wall shear stress (WSS) in the carotid sinus of type 2 diabetes mellitus (T2DM) patients,and analyze the spatial distribution of WSS with contrast enhanced flow field (CEFF) quantita tive visualization analysis software.Methods Totally 47 T2DM patients and 25 healthy subjects were investigated.According to the intima-media thickness (IMT) of common carotid artery (CCA),the T2DM patients were divided into IMT normal group (n=21) and IMT thickening group (n=26).WSS in the carotid sinus was calculated by CEFF analysis soft ware,the corresponding spatial distribution maps of WSS were designed.WSS of the posterior wall of internal carotid artery (ICA) was recorded and statistical analysis.Results Both in the carotid sinus of healthy subjects and T2DM patients,there were two low WSS regions and one high WSS region.There were a large region of low WSS at the posterior wall of CCA and ICA,a small region of low WSS at the anterior wall of CCA,and a high WSS region at the anterior wall of ICA.The WSS at the posterior wall of ICA both in the IMT normal group [(3.39 ± 0.60)dynes/cm2] and IMT thickening group [(2.58±0.46)dynes/cm2] were significantly lower than that in control group ([3.74±0.53]dynes/cm2,both P<0.05).WSS in IMT thickening group was lower than that in IMT normal group (P<0.05).Conclusion CEFF quantitative visu alization analysis might constitute an approach to assess the presence of early-stage arteriosclerosis by measuring the WSS in the carotid sinus.
4.Rotablation in the treatment of high-risk patients with heavily calcified left-main coronary lesions

Objective Heavily calcified left-main coronary diseases (LMCA) remain a formidable challenge for percutaneous interventions (PCI). This study was to investigate the safety and efficacy of using rotational atherectomy (RA) in treating such lesions in actual practice. Methods From February 2004 to March 2012, all consecutive patients who received RA for heavily-calcified LMCA lesions in our cath lab were en-rolled. The relevant clinical and angiographic characteristics at the time of index PCI, as well as the clinical follow-up outcomes, were re-trieved and analyzed. Results A total of 34 consecutive patients were recruited with a mean age 77.2 ± 10.2 years. There were 82.4%pre-sented with acute coronary syndrome and 11.8%with cardiogenic shock. Chronic renal disease and diabetes were seen in 64.7%and 52.9%, respectively. Triple-vessel coronary disease was found in 76.5%of them. The mean SYNTAX score was 50 ± 15 and EuroSCORE II scale 5.6 ± 4.8. The angiographic success rate was 100%with a procedural success rate of 91.2%. The mean number of burrs per patient was 1.7 ± 0.5. Crossing-over stenting was used in 64.7%. Most stents were drug-eluting (67.6%). Intra-aortic ballon pump was used in 20.6%of the procedures. Three patients died during hospitalization, all due to presenting cardiogenic shock. No major complication occurred. Among 31 hospital survivors, the major adverse cardiac events (MACE) rate was 16.1%, all due to target lesion revascularization or target vessel revas-cularization. Conclusions In high-surgical-risk elderly patients, plaque modification with RA in PCI of heavily-calcified LMCA could be safely accomplished with a minimal complication rate and low out-of-hospital MACE.
5.Effect of advanced glycosylation end products on cell cycle of epidermal keratinocyte and the role of signal pathway.

OBJECTIVETo investigate the effect of advanced glycosylation end products (AGE) on cell cycle of epidermal keratinocyte and its possible signal pathway.
METHODS150 mg/L AGE-human serum albumin (AGE-HSA) was prepared in vitro. Primary cultured keratinocytes in logarithmic growth phase were harvested and divided randomly into: A group [with treatment of defined keratinocyte-SFM (DK-SFM) serum-free medium], B group (with treatment of DK-SFM medium including 150 mg/L AGE-HSA), C group (with DK-SFM medium after treatment of U0126) and group D (with D K-SFM medium including 150 mg/L AGE-HSA after treatment of U0126). Cell cycle distributions were analyzed by flow cytometer. The protein levels of cyclin D1, cyclin B1, CDK4 and p44/42 MAPK were measured by Western blot.
RESULTSCompared with those of A group, the percentage of S-phase and G2/M-phase keratinocytes were decreased obviously in B group, the percentages of G2/M -phase keratinocytes showed the same tendency in C and D groups [(9.7 +/- 1.1)% , (9.8 +/- 0.7)%, respectively, P <0.05]. Compared with that of A group, the expression of cyclin D1 were decreased significantly in other groups, among which a weak expression was showed in D group. There was no obvious difference between A and B groups in CDK4, or cyclin B1 and p44/42 MAPK protein levels ,which were significantly higher than those in C and D groups.
CONCLUSIONAGEs inhibit the progress of cell cycle of keratinocytes by downregulation of cyclin D1 expression.

6.Influence of solvent and drug preparation time on Shuanghuanglian injections induce pseudo-allergic reaction.

Choosing the right solvent and timely use is the basis of rational drug use and the most direct and efficient way to improve the safety of traditional Chinese medicine injections. This study aimed to evaluate the influence of solvent and drug preparation time on Shuanghuanglian injection inducing pseudo-allergic reactions with mouse mode. The two tests were carried out: (1) Comparative experiment between different solvent: Shuanghuanglian injection preparation to the appropriate concentration with 0.9% sodium chloride injection and 5% dextrose injection, mixed with Evans blue, at one time intravenous injected into mice, 30 minutes later, the mouse ears vascular permeability were observed and compared. (2) Comparative experiment among different preparation time: placed 10 min, 2.5 h, 6 h and 24 h after Shuanghuanglian injection were prepared and then to detect the pseudo-allergic reactions in mice using the same methods as in (1). The results showed that there was no significant difference in the pseudo-allergic reactions in mice which induced by the same dose of Shuanghuanglian injection, respectively with 0.9% sodium chloride injection and 5% dextrose injection preparation, and with the extension of preparation time, the degree of pseudo-allergic reactions of Shuanghuanglian injection was gradually severe.

7.Evaluation and study of pseudo-allergic reactions of three kinds of traditional Chinese medicine injections and its mechanisms.

To evaluated the pseudo-allergic reactions of cordate houttuynia, pulse-activating and Qingkailing injection in mice, the ICR mouse were divided into different test groups, then were intravenously injected with three traditional Chinese medicine injections, positive control compound 48/80 and physiological saline as normal control. All test substances were mixed with 0.4% Evans blue. The reaction and vascular permeability of the ears were observed and measured 30 min after injection. At the same time, the mechanisms of the traditional Chinese injections' pseudo-allergic reactions was studyed. ICR mice were injected with the test substances as above without EB, blood in a part of mice were collected after 5 min, and the level of histamine in the plasma were measured. Blood in the other part of mice were collected after 30 min, and the level of VEGF, TNF-α and IL-10 in the serum were measured. The reasults showed that except the cordate houttuynia injection, pulse-activating injection in 1. 5 times clinical concentration and Qingkailing injection in 3.3 times clinical concentration caused mild pseudo-allergic reactions mainly for vascular permeability, no pseudo-allergic reactions occurred when the concentration of the two injections was below the concentration metioned above; 5 minutes after intravenous injection of the three TCM injections into ICR mice with the highest dose, the levels of histamine in plasma of pulse-activating injection and Qingkailing injection groups were increased significantly, 30 minutes later, the levels of VEGF, TNF-α and IL-10 in the serum of all groups were no significant change compared to normal group. The mouse of pulse-activating and Qingkailing injection groups showed inflammatory changes in ear and lung tissues. It can be conluded that when the dose or concentration increased to a certain extent, pulse-activating and Qingkailing injection could induce pseudo-allergic reactions on ICR mice, the increased histamine realease maybe the main reason for pseudo-allergic reactions of the two traditional Chinese medicine injections. In addition the author preliminary thought that inflammatory mechanisms leading to hyperpermeabilities were the main reason of the traditional Chinese medicine injection's pseudo-allergic reaction.

8.Epidemical survey of glaucoma among Uigur peasants aged 40 years or above in Kuche rural

Background Glaucoma is one of the leading causes of blindness worldwide;while the different types of glaucoma is vary from different region. Objective Present study was to survey the prevalence and types of glaucoma among peasants of Uigur adults in Kuche county and offer the basis for the prevent and treatment of glaucoma in Uigur nationanlity. Methods 4191 Uigur peasants aged 40 years or above were collected in Kuche county for the survey of prevalence and types of glaucoma by randomized cluster sampling in March and April of 2009. The subjects were grouped into 40-,50-,60- and ≥70 years groups according to the distribution of age. The disease history of glaucoma, regular eye examination, funds examination and measurement of the anterior chamber depth, gonioscope were performed in the all subjects. Darkroom prone test and mydriasis test were carried out in suspicious glaucomous patients. The depth of periphery anterior chamber was assessed based on van Herick' s criteria, and the width of chamber angle was graded based on Scheie' s method. The standardized training was performed. This survey approved by Xinjiang Medical Ethics Committee, all subjects signed the informed consent before the examination. Results 4191 of 4873 subjects finished all the examinations with the response rate 86%. All the subjects showed a good compliance. The prevalence of glaucoma was 3. 79% , and the prevalence of primary angle-closed glaucoma(PACG) .primary angle-open glaucoma (PAOG) and secondary glaucoma was 2.22% ,0.26% and 1. 31% respectively, showing a significant difference among the different types of glaucoma( P<0. 05). The prevalence of glaucoma was elevated with aging (χ2 - 116. 69 ,P<0. 05) and presented a high rate in male subjects compared with female ones(χ2 = 7. 34, P<0. 05 ). Bilateral blindness was found in 19.75% glaucoma peasants, in which 25.3% glaucoma peasants received anti-glaucoma surgery. The distribution of visual acuity of patients was of significant difference among different age groups(χ2 = 37. 69 ,P<0. 05 ) . Conclusions The prevalence of the glaucoma among Uigur peasants in Kuche county was higher than most area no matter inland or overseas. PACG still is the common type in those people.
9.Effects of tangshen recipe on the homocysteine metabolism of patients with diabetic nephropathy.

OBJECTIVETo observe the effects of Tangshen Recipe (TR) on the homocysteine (Hcy) metabolism of patients with diabetic nephropathy.
METHODS64 patients with diabetic nephropathy were randomly assigned to two groups, 32 in each. Those in the Western medicine treatment group (Group A) received insulin and orally took anti-diabetic drugs, while those in the TR group (Group B) received insulin and orally took TR (consisting of astragalus, raw rehmannia root, sanchi root, euonymus branchlet, rhubarb, bitter orange, and dogwood fruit, etc. 4 g/package). Six months was taken as one therapeutic course. Another 48 healthy volunteers were recruited as the control group. Eight intermediate metabolites of the homocysteine metabolism in plasma were quantitated before treatment, three months and six months after treatment. The in vivo changes of each metabolite after treatment were analyzed.
RESULTSCompared with the healthy control group, contents of cysteine (Cys), Hcy, s-adenosylmethionine (SAM), and SAH significantly increased in Group A and B before treatment. Contents of methionine (Met), glutathione (GSH), and Cys-gly decreased significantly, showing statistical difference (P<0.05). Patients' in vivo contents of Cys, Hcy, SAM, and SAH significantly decreased, while contents of Met, GSH, and Cys-gly significantly increased after three and six months of treatment when compared with before treatment in the same group (all P<0.05). No statistical difference existed in contents of SAH, SAM, and GSH of Group A and B after six months of treatment when compared with the healthy control group (P>0.05). No adverse reaction occurred in Group A and B.
CONCLUSIONSTreatment of insulin and TR showed similar favorable effect to Western medicine in treatment of diabetic nephropathy. It could improve in vivo hypomethylation and oxidative stress.
