1.Combination of interferon α with mannan peptide in treatment of HBeAg-positive chronic hepatitis B

Objective To investigate the clinical effect of IFNα combined with mannan peptide in treatment of patients with HBeAg-positive chronic hepatitis B ( CHB ). Methods Eighty HBeAg-positive CHB patients with HBV DNA quantity ranging from 10 to 10 eopies/mL were enrolled and randomized into the treatment group and the control group ( n = 40 for each ). Patients in treatment group were given daily subcutaneous injection of IFNα-2b 5,000,000 U for 52 weeks, and received mannan peptide 10 mg per intravenous injection or 2. 5 mg per intramuscular injection for a total of 2 to 3 treatment courses (12 weeks for each). The control group received only IFNα-2b treatment. Liver function, serum markers of hepatitis B, HBV DNA quantity and blood tests were performed before the treatment and at 2, 4, 8, 16, 26 and 52-week during the treatment; and the adverse effects were recorded. Results The rates for ALT normalization, negative HBsAg, negative HBeAg, HBeAg seroconversion and negative HBV DNA were 91. 8% , 17. 5% , 52. 5% , 27. 5 % and 47. 5% at 52nd week in the treatment group, while those in the control group were 80. 0% , 12. 5% , 30. 0% , 10. 0 % and 25. 0% , respectively. There were significant differences in HBeAg-negative, HBeAg-seroeonversion and HBV DNA-negative rates between two groups (χ2 = 4. 178, 4.021 and 4.381, P < 0. 05 ) , and these indexes in the treatment group were increased to 57. 5% , 30. 0% and 50. 0 respectively at 52nd week after drug withdraw. White blood cells began to be elevated at 4th week and were restored to the normal levels at 8th week in the treatment group, while the count in the control was lower than the normal value even at 52nd week of the treatment with the average of (3.45±1. 18)×109/L. Conclusion Alpha-interferon combined with mannan peptide therapy is effective for patients with HBeAg-positive CHB, which may restore the declined peripheral WBC counts induced by interferon and improve the compliance.
2.Investigation of RHD 1227A allele in five pedigrees in Zhejiang Han population.

OBJECTIVETo analysis the genetic mode of Rh DEL phenotype and RHD 1227A allele in Zhejiang Han population through family investigations.
METHODSRh DEL phenotypes were identified by a serologic adsorption-elution method. Two polymerase chain reaction-sequence specific prime (PCR-SSP) methods which detectED RHD 1227A allele and Rhesus hybrid box, respectively, and a nucleotide sequencing method focused on the exon 9 of RHD 1227A allele were employed to determine the zygosity of RHD allele.
RESULTSAll five probands with Rh DEL phenotype harbored a RHD 1227A allele and had a RHD allele deletion, and they were RHD 1227A/RHd heterozygote. One of the parent members was found to contain a RHD 1227A allele and a normal RHD allele in pedigree 1, 2 and 3, respectively. Thus, they were RHD 1227A/RHD heterozygotes and presented normal D positive phenotype. The son of proband No 1. inherited the RHD 1227A allele and presented a normal D positive phenotype due to a RHD 1227A/RHD heterozygote; The offsprings of proband No. 2, No. 4, and No. 5 did not inherit RHD 1227A allele and presented a normal D positive phenotype.
CONCLUSIONRHD 1227A allele is an important genetic marker of Rh DEL phenotype; RHD 1227A is recessive to normal RHD allele and dominant to RHd allele; RHD 1227A allele is an ancestral, but not a spontaneously mutated allele.

3.Axis-line-distance technique:a new method in scoliosis measurement

Objective To investigate the clinical significance of a new method,axis-line-distance technique(ALDT),in scoliosis measurement.Methods Thirty cases with idiopathic scoliosis were measured on two separate occasions by six observers with the Cobb technique and the ALDT on PACS workstation.The interval time between two measurement occasions were three weeks.The data were analyzed statistically with the paired-sample t-test.Results(1)Concerning intraobserver variance in two measurement occasions,the minimum variance,the maximum variance and the average variance were 0, 24.00?,5.71??1.54?for Cobb technique and 0,12.00 mm,(1.95?0.58)mm for ALDT.There were significant measurement differences for four observers with Cobb method 39.00??10.69?versus 36.50?? 10.63?,31.73??10.96?versus 37.30??9.65?,32.03??7.49?versus 27.86??9.00?,29.77??8.87? versus 34.20??7.26?,all P0.05].(2)Concerning interobserver variances in six observers,the average measurement variance was 5.07??0.35?for Cobb method,and(2.32?0.26)mm for ALDT. 4.Treatment of fractures of extremities with expandable intramedullary nails.

OBJECTIVE:
To investigate the method and clinical effect of the expandable intramedullary nails on fractures of extremities.
METHODS:
Nineteen cases of extremities long tubular bone fractures were treated with Fixion expandable intramedullary nails.
RESULTS:
Nineteen cases were followed up for 4-18 months,all cases healed without any complications.
CONCLUSION
The application of expandable intramedullary nails in the treatment of extremity fractures has the advantages of little trauma, simple operation, rigid fixation and high healing rate. It is a good treatment for fractures of extremities.

5.In vitro properties of magnesium doped hydroxyapatite coating on the surface of carbon/carbon composites

BACKGROUND: In the early study, hydroxyapatite coatings containing magnesium (Mg-HA) were successfully prepared on the carbon/carbon composite surface by electromagnetic induction deposition. The effects of different induction deposition time on the morphology of Mg-HA coating were analyzed. OBJECTIVE:To investigate the in vitro properties of Mg-HA coating on the carbon/carbon composites. METHODS: HA (control group) and Mg-HA (experimental group) coating were prepared on the surface of carbon/carbon composites.(1)In vitro solubility:Carbon/carbon composites with different coatings were immersed in low-glucose DMEM medium, and surface morphology, ion concentration, and coating adhesion were tested before and after immersion. (2) Cytocompatibility: Carbon/carbon composites with different coatings were respectively cultured with mouse bone marrow mesenchymal stem cells for 6 days, and cell proliferation was measured by scanning electron microscope. RESULTS AND CONCLUSION: (1) One day after soaking, sediments formed on the coating surface in both groups, but there were more sediments in the experimental group than the control group. Six days after soaking, the sediments completely covered the coating surface in the experimental group, while there were still some gaps in the control group; Ca and P were both detected on the coating surface in the two groups, and the Mg content on the coating surface was obviously higher in the experimental group than the control group. (2) Six days after soaking, the concentrations of Ca, P and Mg in the control group continued to fall; in the experimental group, the concentrations of Ca and P were also reduced gradually, but the Mg concentration arose gradually. Moreover, the Mg concentration was significantly higher in the experimental group than the control group (P < 0.05). (3) The bonding strength of the coating in the experimental group was higher than that in the control group before and after 6 days soaking. In summary, the Mg-HA coating has better bonding strength and cell compatibility.
6.Evaluation of the clinical characteristics of patients with positional obstructive sleep apnea hypopnea syndrome

Objective To evaluate the clinical characteristics of patients with positional obstructive sleep apnea hypopnea syndrome (P-OSAHS).Methods The study retrospectively analyzed 171 men diagnosed as having OSAHS in Beijing Tongren Hospital,Capital Medical University,during July 2012 to June 2014.That the apnea hypopnea index (AHI) during supine position of ≥ 2 times by the AHI during lateral position was employed as the standard for P-OSAHS.Subjective symptoms,physical examinations,polysomnography (PSG),and three-dimensional computer tomography (3-D CT) were compared between POSAHS group and control group with non-positional OSAHS.Results Of 171 patients with OSAHS,47(27.5%) had P-OSAHS and 124 (72.5%) presented with non-positional OSAHS.There were significant differences between two groups in BMI,neck circumference,AHI,the lowest oxygen saturation,the percentage of time with oxygen saturation below 90% (CT90),and the minimal anterior-posterior airway dimension of the glossopharynx (P < 0.05).The AHI was the only predictive parameter for P-OSAHS (P < 0.01).There were significant differences in the incidence of P-OSAHS between patient with mild or moderate disease (AHI < 30),severe disease (AHI ≥30 but < 60),and extremely severe disease (AHI ≥ 60) (81.8% (18/22) vs45.3% (24/53) vs 5.2% (5/96),P<0.05).Conclusion The severity of OSAHS closely related to the incidence of P-OSAHS,with significant differentee in the incidence of POSAHS between the patients with different severity of OSAHS.
7.Observation on effects of aconitine via acupoint injection in rabbits.

OBJECTIVETo investigate the ability of the pericardium meridian (PM) to mitigate or enhance the cardiotoxic effects of aconitine injected at specific acupoint and non-acupoint sites in rabbits.
METHODSThis study consisted of 3 experiments that were designed to test the effects of injection of 30 μg/kg of aconitine at acupoints on the PM (Test 1), at non-acupoint sites on the PM (Test 2), and at acupoints on other meridians and non-meridian sites (Test 3). In Test 1, 24 rabbits were randomly assigned to receive injections at Quze (PC3), Tianquan (PC2), or intramuscularly. In Test 2, 24 rabbits were randomly assigned to receive injections of aconitine at non-acupoint I, non-acupoint II, or intramuscularly. In Test 3, 48 rabbits were randomly assigned to receive injections at Neiguan (PC6), Sanyinjiao (SP6), Yangjiao (GB35), a non-meridian and non-acupoint site (NMNA), intravenously, and intramuscularly. Electrocardiographs of the rabbits were performed before, during and after injection to determine the incidence of arrhythmia, latency of ventricular rhythm, and recovery rate after aconitine injection. The recovery time index and extent of arrhythmia scores were calculated.
RESULTSIn all groups the incidence of arrhythmia was 100%, and the latency of ventricular rhythm was less than 30 min. In Tests 1 and 2, the recovery rates of the Quze and non-acupoint II groups were significantly higher than those of the muscular group (P < 0.05). In Test 3, the recovery time index and extent of arrhythmia scores of the Neiguan group were low. There were no significant differences between the other acupoint groups, or the NMNA group, when compared with the group receiving aconitine intramuscularly.
CONCLUSIONSAcupoints or non-acupoints along the PM could reduce the severity of the arrhythmia induced by aconitine in healthy rabbits. Meridians play an important role in protecting body functions.

8.Standardized manipulations of sharp hooked needling for treating migraine and investigation on its mechanisms.

A comparison of recognition to migraine and treatment methods for it was made between traditional Chinese medicine and western medicine in this paper. Emphasis was made on standardized manipulation processes and techniques of sharp hooked needling for treating the disease, based on both the organic conception of the human body, the theory held by TCM, and on the principle of treatment according to pathogenesis obtained through differentiation of symptoms and signs. The necessary preparation before operations, methods for its application and announcements in the standardized manipulations of sharp hooked needling were all explained in detail. Based on its needling, pricking, dissecting and relaxing effects, the mechanisms of sharp hooked needling in treating the disease were investigated. It is concluded that sharp hooked needling is very effective in treating migraine.

9.Phenotypic and genetic analysis of four patients with 13q33-q34 microdeletion.

OBJECTIVETo explore the correlation between 13q33-q34 microdeletion and clinical phenotype.
METHODSRoutine chromosomal banding was performed to analyze the karyotype, while array-based comparative genomic hybridization (aCGH array) and single nucleotide polymorphism array(SNP array) were employed to investigate the genome copy number variations.
RESULTSThe karyotype of patient 1 was 46, XY, 9qh+,13qs. Patient 2 showed 46, XX, der (13). Patient 3 showed 46, XX, r(13) (p11.2q32) [43]/45, XX, 13[4]/46, XX, r(13;13) [2]/47, XX, 2r(13;13) [1]. Patient 4 did not undergo chromosome karyotyping analysis. Array analysis showed that four patients have different microdeletions in 13q33-34 region and had common features of 13q33-q34deletion including intellectual disability, facial dysmorphism, microcephaly, hypotonia, low birth weight and genital abnormality.
CONCLUSIONThe severity of phenotypes showed no correlation with the size of deletion in 13q33-q34. The lower percentage of patients with congenital heart disease suggested a complex pathogenesis of such disease. EFNB2, LIG4 and SOX1 in 13q33-34 region are promising candidates for mental retardation. LIG4 was also a likely candidate for microcephaly.

10.Fibrovascular polyp of the hypopharynx and esophagus.

Fibrovascular polyp of the hypopharynx and esophagus, a rare, benign, intraluminal and submucosal tumor, is most commonly originated from the proximal esophagus. We discussed four cases with regurgitation, respiratory symptom or the feeling of a mass in the throat. All the patients were examined with laryngoscope under general anesthesia. A transverse cervical incision was performed in one patient and the polyps were excised under laryngoscope with CO2 laser in the other three patients. All the lesions were removed successfully. Diagnostic and therapeutic principles involved in these cases are presented and discussed. The recognition of fibrovascular polyp of the hypopharynx and esophagus as a potential cause of regurgitation is paramount. Surgical excision is recommended because of the satisfactory outcome.
