1.Gd-DTPA Enhancement MR Angiography of Intracranial Occuping and Vascular Diseases

Purpose To investigate the diagnostic valuation of Gd-DTPA enhancement MRA in intracranial occuping and vescular diseases. Methods Gd-DTPA MRA(63 cases), MRA without Gd-DTPA(16 cases)as a control group was performed with Elscint Prestige 2.0T MR,MRA. 56 cases were proved by operation or pathology, 7 cases were proved by clinical observation. Results The shape of aneurysms, the feeding arteries, the draining veins of AVM and diploe angioma, the feeding arteries of occuping were clearly showed by Gd-DTPA enhancement MRA and were not showed by MRA without Gd-DTPA.There wasn't much difference between cavernous angioma with Gd-DTPA enhancement MRA and MRA without Gd-DTPA in the shape and blood supply of cavernous angioma. Conclusion Gd-DTPA enhancement MRA is useful and worthy for showing blood circulation of intracranial occuping and vascular diseases.
2.Three-dimensional conformal radiotherapy for 56 patients with liver neoplasms

Purpose:To evaluate the curative effects and side effects of 3-dimensional conformal radiotherapy (3DCRT) in the treatment of liver carcinoma.Methods:56 patients with liver carcinoma (Forty three patients were primary hepatic carcinoma and 13 liver metastatic tumors) with 62 lesions were treated with 3DCRT for 30~45 Gy/5~10 f/1~2 weeks with fractionated doses ranging from 4Gy to 8Gy.Results:In the 1~3 months after 3DCRT the results examined by CT or MRI showed that the 62 lesions(56 patients) had CR 24% (15/62),PR 50%(31/62),NC 16% (10/62) and PD 10% (6/62).The overall response rate (CR+PR) was 74%.Conclusions:3DCRT is an effective and safe treatment for hepatic malignant tumor and it can be tolerated by most of the patients.The immediate response of tumor is encouraging. The survival rate and late complications remain to be observed.

5.Analysis on reliability and validity of the Pittsburgh sleep quality index

Objective To assess the reliability and validity of the Pittsburgh sleep quality index (PSQI) .Methods The cross-sectional survey was conducted in the outpatient department of this hospital .544 insomnia and non-insomnia voluntary participants (18-65 years old) from July to October 2011 were assessed with PSQI and the insomnia severity index (ISI) ,which were randomly delivered to them .Results The test-retest reliability of PSQI was 0 .994 .The split-half reliability coefficient of PSQI was 0 .824 . The overall Cronbach′s alpha coefficient was 0 .845 .The confirmatory factor analysis showed that the fitting indicators for the χ2/df ,root mean square error of approximation(RMSEA) ,non-normed fit index(NNFI) ,comparative fit index(CFI) and goodness-of-fit index(GFI) were 4 .83 ,0 .09 ,0 .96 ,0 .98 and 0 .97 respectively .The correlation coefficient between the PSQI and ISI was 0 .842 . The correlations coefficients between PSQI with the questionnaire type ,interview date and interview department were -0 .046 , 0 .276 and -0 .331 .Grouping by individuals with or without insomnia ,the area under the receiver operator characteristic curve (ROC) for PSQI was 0 .944(95% CI:0 .925-0 .963) .Conclusion PSQI has good reliability and validity ,and could be used for the sleep quality comprehensive evaluation in the patients with insomnia .

7.Clinical Characteristics and Outcome of Cytomegalovirus Infection in Infants

Objective To study the clinical characteristics and outcome of cytomegalovirus(CMV) infection in infants.Methods All data including time of infection,clinical characteristics,laboratory tests and outcome of CMV infections in hospitalized infants were collec-(ted) and analyzed from January,1994 to July,2004.Results In 87 infected infants,congenitally infected newborns,perinatal infection in infants and postnatal infection in infants accounted for 27.6%,62.0%,16.6%,respectively.CMV hepatitis was the most frequent type of disease with the incidence of 41.3%,in which the incidence of splenomegaly was 10.3%.Most of CMV hepatitis infants had a good prognosis with the improved rate 80.5%.Central nervous system abnormality(including abnormal intension of muscle,convulsion,ocular and hearing abnormalities) occurred only in congenital and perinatal infection with the incidence of 20.4%.Generalized infection,the incidence of congenital infection and perinatal infection was 16.7%,1.8%,respectively.It did not occur in postnatal infection.The mortality rate of congenital infection and perinatal infection were 12.5% and 1.85%,respectively.Conclusions CMV infection is the main cause of infant hepatitis and it can also cause neurologic sequelae.The outcome of generalized infection in congenital infection is bad and the mortality rate is high.

Objective To investigate contents of serum free fatty acid(FFA)in old patients with metabolic syndrome(MS)and to analyze its correlation with the components of MS.Methods Among 1 372 over 60-year-old people undergoing physical examination,169 patients with MS and 89 healthy subjects were selected as MS group and control group.The blood pressure,height,weight,waist circumference,and hip circumference were measured.Levels of fasting blood glucose,insulin,blood lipid,and serum FFA were measured.Waist-hip ratio(WHR),body mass index(BMI),and insulin resistance index(HOMA-IR)were calculated.Results The levels of waist circumference,WHR,BMI,fasting blood glucose and insulin,HOMA-IR,blood lipid,and serum FFA in MS group were significantly higher than those in control group.The levels of HDL-C were lower in MS group(P
9.Value of clinical parameters in predicting the initiation of renal replacement therapy in acute kidney injury patients with cardiorenal syndrome

Objective To investigate the value of clinical parameters in predicting the initiation of renal replacement therapy(RRT) in acute kidney injury (AKI) patients with cardiorenal syndrome (CRS).Methods A total of 75 AKI patients hospitalized with CRS were enrolled.All patients received pharmacologic therapy on the beginning 3 days.The patients whose heart function improved were divided into control group (n=39),and the patients whose heart function worsened were divided into RRT group (n=36).Clinical and laboratory data on the first day and the fourth day were collected and analyzed.The factors on the first day were labeled asⅠ ,and those on the fourth day were labeled asⅡ. The ratio of some parameters calculated were labeled asⅡ/Ⅰ .Area under curve (AUC) of receiver operating characteristic curve (ROC) of these factors was used to evaluate the sensitivity and specificity in predicting the initiation of RRT.Results The patients in RRT group had significantly higher levels of BNP-Ⅱ,BNP Ⅱ / Ⅰ and creatinine Ⅱ / Ⅰ (P < 0.01),and lower levels of 24 hours urine volume-Ⅰ and 24 hours urine volume-Ⅱ (P < 0.01).From ROC curve analysis,the AUC of 24 hours urine volume-Ⅰ,24 hours urine volume-Ⅱ,creatinine Ⅱ / Ⅰ,BNP-Ⅱ levels and BNP Ⅱ/Ⅰ to predict RRT were 0.736,0.875,0.747,0.779 and 0.894 respectively.When the cutoff values of 24 hours urine volume-Ⅰ,24 hours urine volume-Ⅱ,BNP-Ⅱ levels,BNP Ⅱ / Ⅰ and creatinine Ⅱ / Ⅰ were 905 ml (sensitivity 75%,specificity 94.9%),1450 ml (sensitivity 75%,specificity 100%),3360 ng/L (sensitivity 72.2%,specificity 100%),1.37 (sensitivity 75%,specificity 100%) and 1.25 (sensitivity 72.2%,specificity 94.4%) respectively,the value of the parameters to predict RRT was high.Conclusions The 24 hours urine volume,BNP levels after treatment and the dynamic changes of BNP levels and creatinine levels can be used as predictors of the initiation of RRT in the AKI patients with CRS.
10.Analysis of microvessel density in pterygium tissue with corneal laser confocal microscopyc in vivo and immunohistochemistry in vitro

BackgroundPterygium is an ocular surface disease of abnormal cell proliferative kind and angiogenesis plays an important role in its development and recurrence.Several anti-angiogenic therapies have been used to treat pterygium,but there very few studtes for the in vivo observation of the microvessles in pterygium.ObjectiveThis study was to observe angiogenesis in pterygium with a high-resolution confocal microscope in vivo and to perform immunohistochemical study in vitro.MethodsA prospective case-controlled study was designed.Twenty eyes of 20 consecutive patients with primary pterygia and 20 age- and sex-matched patients with inner eye diseases and strabismus with normal conjunctiva were enrolled in this study.An in vivo confocal microscopy imaging system (Heidelberg Retina Tomograph Ⅱ Rostock Cornea Module) was used to collect microvascular pictures from the anterior part of pterygia and normal nasal conjunctiva of controls,and then immunochemistry was performed to examine the expression of CD31 in microvessel in vitro.The vascular density values were compared between these two groups.The correlation of vascular density values between in vivo Heidelberg Retina Tomograph and in vitro immunohistochemistry was calculated.Written informed consent was obtained from pationts before any examination and surgery.ResultsUnder the in vivo confocal microscope,the microvessel density was (8929±2993) μm/mm2 and (4202 ±692)μm/mm2,respectively,in pterygium and the normal conjunctiva group with a statistically significant difference between them (t =6.881,P<0.01 ).Immunochemistry revealed that the expression of CD31 to measure vascular density was ( 21.00 ± 4.06/400 × field ) and ( 6.07 ± 1.75/400 × field ) in pterygium and the normal conjunctiva group,showing significant difference (t =12.312,P<0.01 ).Positive correlations were found in the vascular density values between in vivo corneal laser confocal microscopy examination and in vitro immunochemistry examination in both the pterygium group and normal conjunctiva group (pterygium group:r=0.649,P<0.01 ;normal conjunctiva group: r=0.572,P<0.01 ) ConclusionsIn vivo confocal microscopy imaging is superior to in vitro immunochemistry in evaluating the microvessel of pterygium.The results of this study offer a new way index for further investigation of the biological behavior of pterygium and its mechanism.
