1.Perioperative anticoagulant therapy in the prevention of deep vein thrombosis after rectal carcinoma surgery

Objective To evaluate curative effect and safety of anticoagulant drugs used in perioperative period to prevent deep vein thrombosis (DVT) after rectal carcinoma surgery. Methods Clinical data of 96 postoperative rectal carcinoma cases from January 2006 to June 2009 were analyzed retrospectively.These cases were divided into two groups:48 cases in nonanticoagulant group and 48 cases in anticoagulant group.After radical operation of rectal carcinoma,low molecular heparin calcium was injected subcutaneously daily for 8 consecutive days beginning the day before surgery in anticoagulant group. Results Incidence of DVP in anticoagulant group (4%) was significantly lower than that in noncoagulant group (19%) (x2 = 5.031,P < 0.05).Postoperative hospitalization time in anticoagulant group [(12 ± 3) d] was significantly shorter than that of nonanticoagulant group [(22 ± 6) d] (t = 10.328,P< 0.01).There were no significant differences between two groups in operative time,blood loss and postoperative blood drainage,peripheral blood platelet counts before and after operation,prothombin time and active part thrombin time before and after operation,the time when postoperative DVT developes;secondary adverse reactions; postoperative major complication rate and mortality (all P > 0.05). Conclusion Anticoagulant drugs used in perioperative period is safe and effective in prevention of deep vein thrombosis in lower limbs after radical operation of rectal carcinoma.
2.Aspirin and low molecular weight heparin in the prevention of portal venous thrombosis after pericardial disconnection

Objective To evaluate the effect of early aspirin combined with low molecular weight heparin calcium in prevention of portal system thrombosis after portaazygous disconnection in patients of portal hypertension.Methods In this study.90 cases with cirrhotic portal hypertension were divided randomly into two groups(control group:44 cases,observation group:46 cases),after standard splenectomy and pericardial devascularization.On the day when abdominal and gastrointestinal drain was not bloody(at 2-5d,mean 3d after operation),low molecular weight heparin calcium 4100 IU per day was given to observation group by subcutaneous injection for 10 days.During treatment,blood platelet(PLT)count was monitored.When PLT in both groups>300 ×10~9/L.low molecular dextran 500 ml per day was given by iv drip for 7-14 days,oral enteric aspirin 100 ms/day was given,until PLT reached normal range.Portal venous thrombosis was evaluated by color Doppler ultrasonography weekly for 3 weeks.Results Postoperative incidence of portal vein thrombosis in observation group(7%)was lower significantly than that in control group(27%)(X~2=6.972,P<0.05).Incidence of postoperative complications in observation group(7%)were lower significantly than that in control group(32%)(X~2=9.393,P<0.01).Mortality rate in observation group(2%)were lower significantly than that of control group(18%)(X~2=4.748.P<0.05).There was no significant difference between the two groups in blood platelet count during observation(P>0.05).Prothrombin time,active pact thromboplastin time were also similar in the two groups(P>0.05).Side effect such as self-limited subcutaneous hemorrhage developed in no cases in control group compared to two cases in observation group(X~2=0.558,P>0.05).Conclusion It is a safe and effective method to use early aspirin combined with low molecular weight heparin in the prevention of portal system thrombosis after splenectomy and portaazygous devascularization in cirrhotic portal hypertensive patients.
3.Analysis the changes of SAA expression after liver transplantion by proteomics technique

Objective In this report the proteomics technique was used for analysis of the Molecu-lar marker related acute rejection after liver transplantation.Methods Highly abundant proteins in human serum were deleted using Aurum Serum Protein Mini Kit and separated by 2-DE.The maps were analyzed by ImageMaster 2D Platinum software.The differential proteins on the gel were did in gel digestion with trypsin and then identified by nanoUPLC-ESI-MS/MS.Results Reproducible 2-DE image was successfully ob-tained.2 up-regulated expressed protein spots were identified clearly are serum amyloid A protein.Conclu-sions In this study, it was confirmed that SAA expression upregulate after transplantation in patients with acute rejection by proteomics technique, This method may provide clues to the early diagnosis of liver al-lograft rejection.
4.Counselor verbal response modes of psychic hot-line counseling

Objective:To explore the counselor verbal response modes of psychic hot-line counseling and its characteristics.Methods:Through a qualitative study with category analysis method,8 pieces of audio recordings were collected,in each of which the entire duration was between 30 minutes to 50 minutes.And all of these record-ings were transcribed word by word.Meaningful 1202 pieces were deeply analyzed.Results:There were 5 core counselor verbal response types in hot-line counseling,greeting response,set-up response,listening response,follow-up response,and affecting response,all of which had 24 concept categories.All of the online counselors used the 5 types of verbal response.The listening response and follow-up response were the highest frequent types.Greeting response and set-up response were used mostly in the initial stage of counseling.Listening response,and follow-up response were used mostly in the stage of describing problem.Affecting response was sharply increased in the stage of analyzing problem.Greeting response and set-up response were used mostly in last stage of counseling.Conclu-sion:It suggests that there are 5 core counselor verbal response types and 24 concept categories in hot-line counse-ling.The listening response and follow-up response are the highest frequent types.Different types of verbal re-sponse modes used by counselor in different stages due to different intention.

Calpains are calcium-modulated proteases which respond to Ca2+ signals by removing limited portions of protein substrates,thereby irreversibly modifying their functions.Members of calpains are present in a variety of organisms in human.Calpains mediates crucial cellular functions,including rearrangement of cytoskeletal proteins and protein cleavage to activate various receptors and pro-enzymes.The abnormal activation of calpains is one of the important mechanisms in several pathogenesis,including neurodegenerative diseases,traumatic brain and spinal cord injuries,cataracts and ischemia-associated injuries.The recent findings on the role of calpains and its inhibitors in the liver injury will be reviewed in this paper.

7.Determination of Serum Concentration of Deoxyschisandrin by RP-HPLC

OBJECTIVE:To determine the serum concentration of Deoxyschisandrin by RP-HPLC.METHODS:The chr-omatographic separation was performed on Phenomenex C18(250mm?4.6mm,5?m) column with column temperature at 30℃.The mobile phase consisted of methanol - water (80∶20) at a flow rate of 1mL?min-1. The detection wavelength was set at 252nm. RESULTS:The calibration curve of Deoxyschisandrin was linear in the concentration range from 0.52 to 33.28?g?mL-1 (r=0.999 6). The average recovery of deoxyschisandrin was (97.27?4.11)%(RSD=4.23%,n=9).CONCLUSION:The method is simple, accurate and sensitive, and it can be used for the determination of serum concentration of Deoxyschisandrin.

There are one third of synthesized proteins must be secreted to the cell surface or to the surrounding environment to acquire their native functional state. Most of them are exported by Sec translocase (secretion pathway). Sec translocase consists of a membrane embedded protein-conducting channel, termed SecYEG and a peripherally associated motor domain, the ATPase SecA. The SecDFyajC heterotrimeric membrane protein complex can facilitates protein translocation. SecB is a molecular chaperone that functions in the protein translocation pathway. SecM (secretion monitor) encoded by the 5' region of the secM-secA mRNA, which elongation arrest is required for upregulated expression of SecA. The signal sequence in the N terminus of the nascent peptide is first recognized by the signal recognition particle (SRP). SecB, the Sec-system-specific chaperone, channels the preprotein to the Sec translocation pathway and, ad- ditionally, actively targets the bound precursor to the translocase by its ability to bind SecA. The preprotein-bearing SecA then binds to the membrane, at a high-affinity SecA-binding site, SecYEG, which constitutes a channel for polypeptide movement. Continued translocation requires cycles of ATP hydrolysis bySecA, which is thought to occur in a step-wise fashion with a step of 20~30 amino acid residues.

Influenza virus hemagglutinin (HA) is a key factor in the virus's invasion of host cells, involving the binding of the virus to target cells and the fusion of membranes. The proteolytic cleavage and activation of HA by host proteases are prerequisites for the virus to recognize host cells and initiate membrane fusion, and are also essential for viral infection of the host. This article summarizes the proteolytic activation of different subtypes of influenza virus HA by type II transmembrane serine proteases, human tissue kallikreins, and other host proteases, and discusses their potential as targets for antiviral therapy.
10.Comparison of axial length measurement by Lenstar and contact A-scan in patients with idiopathic macular hole

Objective To compare the axial length (AL) measured by Lenstar and contact A-Scan in the patients with idiopathic macular hole and study the correlation between the difference of the two measurements and the foveal thickness measured by optical coherence tomography (OCT).Methods Twenty-seven eyes of 26 idiopathic macular hole patients (IMH group) and 27 eyes of 25 patients with mild cataract (control group) were enrolled in this study.Foveal thickness was measured with 3D OCT.The AL was measured by Lenstar and contact A-Scan,and the consistency of the two measurements was determined by Bland Altman analysis.The correlation between the difference of the two measurements and foveal thickness was analyzed by Pearson correlation analysis.Results Mean foveal thickness of IMH and control eyes were (372.85±60.02) μm and (243.44±22.50) μm,respectively.The difference between the foveal thickness of the two groups was highly significant (t=-10.490,P<0.001).In the IMH group,the AL measured by Lenstar and contact A-Scan were (23.20± 1.12) mm and (23.18± 1.13) mm,respectively,the difference between the two measurements was not statistically significant (t =-0.549,P =0.588),whereas in the control group,the AL was (23.41 ± 0.72) mm by Lenstar and (23.33 ± 0.74) mm by contact A-Scan,the two measurements were significantly different (t=-4.832,P<0.001).However,no correlation was found by Pearson correlation analysis between the difference of the two measurements and the foveal thickness in either IMH or control group (r=0.181,-0.141; P>0.05).Conclusions Although there is no difference of axial length measurements using Lenstar and contact A-Scan in IMH eyes,in clinical measurements the results of two instruments should be taken into comprehensive consideration.
