ObjectiveTo explore the clinical effect of video-assisted thoracoscopic surgery(VATS) for chronic empyema. Methods74 patients with chronic empyema were divided into experimental group and control group, control group was given decorticationoflung, and experimental group was given VATS. ResultsThe duration of chest tube drainage, hospitalization and complications in experimental group was significantly better than that in control group,it showed better clinical effect. ConclusionFor part of patients without the serious disease change of chronic empyema, if the surgery indication could be controlled strictly, the VATS was considerable.

OBJECTIVETo summarize application of rafting K-wires technique for tibial plateau fractures.
METHODSFrom January 2013 to January 2015,45 patients with tibial plateau fractures were treated by locking plate with rafting K-wires, including 33 males and 12 females with an average of 44.2 years old ranging from 22 to 56 years old. According to Schatzker classification, 6 cases were type II, 8 were type Ill, 4 were type IV, 4 were type V, and 5 were type VI. Allogeneic bone graft were performed for bone defects. All patients were fixed with two to five K-wires. Part of weight loading were encouraged at 3 months after operation, and full weight-loading were done at 5 months after operation. Postoperative complications were observed, and Rasmussen clinical and radiological assessment were used to evaluate clinical results.
RESULTSAll Patients were followed up from 10 to 23 months with average of 14 months. According to Rasmussen clinical and radiological assessment, clinical scores 23.58 ± 6.33, radiological scores were 14.00 ± 6.33; and excellent and good rates were 82.2% and 77.8% respectively. Four patients occurred severe osteoporosis and collapse of articular surface; 5 patients occurred traumatic arthritis.
CONCLUSIONRafting K-wires technique with anatomized armor plate could effective fix and support platform collapse and joint bone fragments, increase support surface area and reduce postoperative reduction loss rate.

Objective To evaluate the clinical effect of absorbable self-enhancing cartilage nails in the treatment of radial head fractures. Methods Data of 22 patients with radial head fractures, who had undergone open reduction and internal fixation with absorbable self-enhancing cartilage nails from January 2008 to December 2011,were retrospectively analyzed.Among them, data of 18 patients were complete, including 10 males and 8 females.According to the Mason classification, there were 14 cases of Mason Ⅱ,3 cases of Mason Ⅲ and 1 case of Mason Ⅳ.Two patients were combined with radial neck fractures,9 with osteochondral fractures of capitellum, and 1 with posterior dislocation of the elbow and olecranon avulsion fracture. The radial head fractures were temporarily fixed with Kirschner wires after open reduction, then absorbable self-enhancing cartilage nails with length from 16 to 24 mm and diameters of 1.5 mm were applied to reach final fixation. The heads of nails were right underneath of cartilages without penetrating bone cortexes on the contralateral side. The radial neck fractures were reduced and fixed with T-plates or anatomical plates after the radial head fractures were fixed with cartilage nails.Four cases of osteochondral fracture of capitellum were fixed with absorbable sutures to external epicondyle of humerus. For other 5 cases of osteochondral fracture of capitellum, the bone fragments were removed due to too small to fix. Results All 18 patients were followed up for 6 to 54 months (average,31.3 months). All fractures achieved bone union, and the healing time ranged from 5 to 12 weeks (average,8.7 weeks). There were no complications, such as internal fixation failure, nonunion, hematoma, sinus formation, osteolytic changes of radial heads and so on.According to the Broberg-Morrey elbow performance score,13 cases were excellent,4 good, and 1 fair, with the total excellent and good rate of 94.44% (17/18).At final follow-up,the elbow range of motion was 90° to 150° (average,123.8°),and the range of forearm rotation was 130° to 180° (average,152.5°).Conclusion Absorbable self-enhancing cartilage nails can be used to treat various kinds of radial head fractures, and the results are satisfactory.

Objective To investigate the effect of curcumin an extract of a Chinese medical herb on the sensitivity of CD133 + rectal cancer cells to radiotherapy. Methods In vitro experiments:CD133 +cells were purified with immunomagnetic beads from HRT-18 cell line and divided into curcumin group, radiotherapy group and curcumin plus radiotherapy group.MTT assay and Annexin V/PI staining were used to measure the proliferation and apoptosis of the cells. In vivo experiments:Transplanted rectal tumor was established in 46 nude mice and randomly divided into curcumin group, radiotherapy group and curcumin plus radiotherapy group.Tumor size and apoptosis were detected by daily observation and TUNEL staining respectively. Results Curcumin inhibited proliferation and apoptosis of CD133 + rectal cancer cells when combined with radiotherapy. It also significantly increased the growth inhibition of rectal tumor and promoted the apoptosis of rectal cancer in vivo.MTT assay showed that after 24 hours, compared with that of radiotherapy group(14.6% ± 1.0%),curcumin plus radiotherapy group (18.7% ± 1.7%) inhibited the growth of the tumor(P < 0.01).Annexin V/PI showed that curcumin plus radiotherapy group (28.8% ±3.7%) was significantly different from the radiotherapy group(13.1% ± 1.4%) in cell apoptosis (P <0.01).In vivo, after 6 days, tumor volume (521 ± 79) mm3 in curcumin plus radiotherapy group was significantly lower than that of radiotherapy group(717 ± 134) mm3 (P < 0.01) ; TUNEL staining results indicated that the RCST in curcumin plus radiotherapy group (26.1% ± 3.3%) were higher than that in radiotherapy group (12.0% ± 2.1%) (P < 0.01).Conclusions Curcumin significantly enhances the radiosensitizing effect for CD133 + rectal cancer cells.
