3.Retrospective analysis of the clinical outcomes in 12 recurrent borderline ovarian tumor patients with second fertility-sparing surgery

Objective To analyse the clinical practicability and significance of fertility-sparing surgery for women with recurrent borderline ovarian tumors ( BOT).Methods The clinical and pathological recordings of 12 patients suffered from recurrent BOT in the Obstetrics and Gynecology Hospital of Fudan University from Jan.2002 to Dec.2012 were analyzed retrospectively.The menstruation situation , fertility function , recurrence rate and survival after second conservative surgery were collected and analyzed .Results The average onset age of these 12 patients was 26 years, and none of them had procreation before the second operation.The pathological diagnosis were still BOT and the recurrent sites of 10 patients were still restricted to the ovaries.Of the 12 cases, 8 cases of them had normal menstruation functions and 4 cases had less menstrual volumes or longer menstrual periods.Among 6 cases desired for pregnancy after the second operation,4 cases of them were successful.The spontaneous pregnancy rate was 4/6.The mean duration of follow-up was 53 months(range:23-97 months).All recurrent BOT, three cases developed recurrence once more,two cases of them were just be hinted by B ultrasounds and only one cases was approved to be BOT again by a once more surgical pathology.But no disease related deaths occurred.Conclusions The second fertility-sparing surgery may be feasible for women with recurrent BOT who are young and strongly desired for preservation of fertility.The spontaneous pregnancy rate is satisfactory.However, carefully follow-up is still needed.
4.Experimental Study on Anti-Gastric Ulcer Effect of Aqueous Extractive of Clarke Boea Herb

Objective To observe effects of aqueous extractive of Clarke boea herb on mice with gastric ulcer, and provide basis for clinical application.Methods Gastric ulcer model was induced by reserpine. The mice were divided into normal group, model group, ranitidine group, low and high dose of Clarke boea herb groups. Mice in treatment groups were given corresponding drugs by gavage, and the normal group and control group were given the same amount of saline for 4 weeks. The area of gastric ulcer, serum proinflammatory cytokines gastric juice volume, gastric juice acidity, iNOS and COX-2 gene expressions were detected.Results Compared with control group, the gastric ulcer area in low and high dose of Clarke boea herb groups decreased significantly. Clarke boea herb increased the SS and VIP, decreased the MOT and SP serum levels (P<0.05,P<0.01). Clarke boea herb reduced the levels of serum proinflammatory cytokines of IL-6, IL-12, TNF-α and IFN-γ(P<0.05,P<0.01). Clarke boea herb also decreased the genetic expressions of iNOS and COX-2 (P<0.05,P<0.01).ConclusionClarke boea herb has good gastric ulcer preventive effects, and the high dose of Clarke boea herb group showed the best efficiency.
5.Clinical characteristics and ultrasonographic features of local chest wall tumor recurrence after mastectomy for breast cancer

Objective To observe the ultrasonographic features of local chest wall tumor recurrence after mastectomy for breast cancer and its clinical and histopathological characteristics. Methods The ultrasonographic features, clinical and histopathological characteristics of 27 patients with local chest wall tumor recurrence after mastectomy confirmed histopathologically were retrospectively reviewed. Results The disease-free intervals of twenty-seven patients ranged from 3 to 129 months [mean (31.9±31.4) months]. Most of the recurrence(18/27, 66.7%) occurred within 3 years after mastectomy. The clinical manifestations were:7 cases (7/27, 25.9%) with regional skin redness and swelling, red rash or ulceration on chest wall associated with or without palpable mass, 20 cases(20/27, 74.1%) with chest wall palpable masses without obvious skin change. On ultrasonography, 2 cases showed diffuse inifltrative type with ill-deifned inhomogeneous hypoechoic lesion and skin thickening. And twenty-ifve cases(43 lesions) showed mass type with a lesion size range of 5.4-114.7 mm [mean (24.4±21.6) mm]. Among them, 32 lesions were located near to the operation incision scar, 36 involved muscle layer, 38 were hypoechoic, 31 had irregular shape, 24 had indistinct margin, and 31 had blood lfow signal. In addition, calciifcation, halo, and taller-than-wide shape were absent in all 43 lesions. Conclusions The tumor recurrence often occurred within 3 years after mastectomy in high-risk patients. Ultrasonographic feature of chest wall recurrent lesion is of great value in the diagnosis.
6.Patient safety culture training for nursing staff

The Hospital Survey on Patient Safety Culture (HSOPSC) questionnaire was used for appraisal in a newly-built general hospital for all the nursing staff, and they were provided with a six-month safety culture training according to appraisal results.The training has elevated the positive response rate of the nursing staff towards all dimensions of patient safety culture, proving that scientific and reasonable safety culture training is conducive to nurses' perception of patient safety culture.
7.Application of MRDWI combined with CEA and SCC in evaluation of the efficacy of concurrent chemoradiotherapy for cervical squamous cell carcinoma

Objective To determine the value of the apparent diffusion coefficient (ADC) of magnetic resonance diffusion-weighted imaging (MRDWI) combined with squamous cell carcinoma antigen (SCC) and carcinoembryonic antigen (CEA) in the evaluation of the efficacy and prognosis of concurrent chemoradiotherapy for cervical carcinoma.Methods A total of 80 patients with cervical squamous cell carcinoma confirmed by histology or cytology in our hospital from 2013 to 2016 were included in this study.Of the 80 patients, 39 were FIGO stage ⅡB, 7 were stage ⅢA, 26 were stage ⅢB, and 8 were stage ⅠVA.MRDWI examination and SCC and CEA measurements were first performed for the patients following group assignment, and the patients were then given extrapelvic radiotherapy (45-50 Gy)+platinum-based chemotherapy plus brachytherapy (20-25 Gy) based on their conditions.MRDWI, SCC, and CEA examinations were performed again after treatment to determine the changes in ADC, SCC, and CEA.In addition, ADC, SCC, and CEA were examined in the middle stage of treatment for 40 patients.Data were analyzed using the paired t-test or ANOVA.Results The overall response rate of the 80 patients after concurrent chemoradiotherapy was 100%.No disease progression was identified in any of the patients until the end of treatment, and the overall survival time of the patients was all above 6 months.Serum SCC and CEA were reduced after treatment (P=0.000,0.000), whereas the ADC value was increased after treatment (P=0.000).The increase in ACD following the decreases in SCC and CEA after treatment (P=0.000, 0.000) was indicative of increased efficacy of the concurrent chemotherapy and radiotherapy.Conclusions MRDWI combined with SCC and CEA is highly reliable for the evaluation of efficacy and prognosis of concurrent chemoradiotherapy for cervical cancer.

Objective To illustrate the evaluation effect of bivariate analysis of sensitivity and specificity meta-analysis model in diagnosis test to provide basis for selecting better evaluation method of diagnostic test.Methods Bivariate model was presented by reanalyzing the data from a published meta-analysis of two diagnostic techniques in diagnosis of schistosomiasis japonica.Results The bivariate model could directly provide summary estimates of(logit)sensitivity,specificity and DOR with corresponding 95% CI for two diagnostic tests(IHA and ELISA).Also,it could elicit any significant difference that existed among sensitivity,specificity and DOR between the two diagnostic methods,and incorporate any correlation that existed between sensitivity;specificity.Conclusion The bivariate model preserves the two dimensional nature of the original data,and separates effects of sensitivity and specificity,which is more rational than a net effect on diagnostic odds ratio scale as in SROC approach.The bivariate model is appropriate and agile,and can be used as an extension and improvement of the traditional SROC method.
10.The effects of silencing Smo gene on proliferation and apoptosis of rat primary chondrocyte

Objective To investigate the effects of silencing Smo gene on proliferation and apoptosis of rat prima-ry chondrocyte in vitro.Methods The primary chondrocyte was obtained by mechanical-enzyme digestion and identified by Immunohistochemical cells ( ColⅡ) .The animals were divided into control group , control siRNA group and Smo siRNA 1 ~3 group.The siRNA was transfected into chondrocytes by lentivirus vector .After 72 h, the cell viability was detected by MTT, Smo expression was detected by RT-PCR and Western blot, and the apoptosis of chondrocyte was assessed by flow cytometry .Results All types of siRNA were transfected into primary chondrocyte by vectors, the Smo siRNA 1 ~3 may inhibit the expression of Smo mRNA and protein in chondrocytes, and Smo siRNA2 had the highest silencing rate ( the expressions of Smo mRNA and protein were 0.19 ±0.03 and 0.39 ±0.07 ) .The cell viability in Smo siRNA2 group was lowest ( 77.38% ±7.19%) , while the apoptosis rate of Smo siRNA2 was highest ( 21.43%±2.97%) .Conclusions Silencing Smo gene in primary chondrocytes may inhibit proliferation and promote apoptosis , Smo may have a protecting role from apop-tosis of the chondrocyte.
