1.Sectional anatomy characteristics versus image anatomy of bone nasolacrimal duct

BACKGROUND: Researches are recently focus on topography and image dissection of nasolacrimal duct, but it is lack of systemically contrast researches between sectional anatomy and image anatomy on bone nasolacrimal duct by using dry cranium samples.OBJ ECTIVE: To investigate the characteristics of sectional anatomy and image anatomy on bone nasolacrimal duct and provide evidences for related operations of nasolacrimal duct.DESrGN: Self controlled study.SETTING: Office of Teaching Supplies, Chengdu Medical College.MATERIALS: The experiment was carried out in the Topography Laboratory, Department of Human Anatomy, Chengdu Medical College from September 2005 to September 2006. Non-injured dry cranium was randomly selected from 34 adults (68 sides), including 34 sides on males and 34 sides on females.METHODS: ① Based on OM line, a routine scanning base line, which was regarded as the axial scanning baseline,samples were scanned at flat level with SHIMADZU CT device. Bone nasolacrimal duct was factitiously divided into three parts, including 1/3 superior segment, 1/3 middle segment and 1/3 inferior segment. The means at each related layer were determined as the final results. ② Cranium samples were signed based on image scanning baseline, and then they were cut into sections at cross section fault along scanning baseline at flat level with section razor. In addition, related indexes of bone nasolacrimal duct were measured and compared with image results.MAIN OUTCOME MEASURES: ① Anterior, posterior, left and right diameters of superior aperture, 1/3 superior segment,1/3 middle segment, 1/3 inferior segment and inferior aperture of bone nasolacrimal duct; ② depth of internal bone wall and posterior bone wall in superior aperture, 1/3 superior segment, 1/3 middle segment, 1/3 inferior segment and inferior aperture if bone nasolacrimal duct; ③ position and form of inferior aperture of bone nasolacrimal duct; ④ comparisons of bone nasolacrimal duct between image anatomy and sectional anatomy.RESULTS: ① Anterior, posterior, left and right diameters of cross section of bone nasolacrimal duct: Superior aperture of bone nasolacrimal duct was narrow, but inferior aperture was changed obviously. Diameter was increased from top to bottom. In addition, anterior and posterior diameters were more than left and right ones. There was significant difference in internal diameter of superior aperture and 1/3 inferior segment of bone nasolacrimal duct between male and females (t =2.458, 2.227, P < 0.05). However, there was no significant difference in internal diameter of 1/3 superior segment, 1/3 middle segment and inferior aperture of bone nasolacrimal duct between male and female (P > 0.05). ② Depth of internal bone wall and posterior bone wall of bone nasolacrimal duct at cross section: Depths of internal bone wall and posterior bone wall of bone nasolacrimal duct were (0.87±0.23) mm and (0.21±0.19) mm, respectively. In addition, there was significant difference between them (t =2.547, P < 0.05). However, there was no significant difference in depth of internal bone wall and related posterior bone wall of superior aperture, 1/3 superior segment, 1/3 middle segment, 1/3 inferior segment and inferior aperture of bone nasolacrimal duct between male and female (P > 0.05). ③ Position and form of inferior aperture of bone nasolacrimal duct changed remarkably. Results of image anatomy of bone nasolacrimal duct were as the same as those of sectional anatomy.CONCLUSION: Perfection of normal sectional anatomy and image anatomy of bone nasolacrimal duct is beneficial for successfully performing related operations of nasolacrimal duct and reducing complications.
2.Neurons in the hippocampus of chemobrain versus non-chemotherapy brain

BACKGROUND:Chemotherapy drugs have been shown to penetrate the blood-brain barrier and cause neurotoxicity,further damaging nerve cells.OBEJCTIVE:To investigate the changes in the expression and morphology of hippocampal neurons in chemobrain,and to investigate the underlying mechanism of nerve regeneration in chemobrain.METHODS:The brain specimens were obtained from the six patients undergoing cancer chemotherapy and six patients without chemotherapy,and were then subdivided into two groups:< 60 years and > 60 years groups (n=3 per group).Immunofluorescence staining with specific antibodies was used to detect the changes in the morphology and number of neurons in the hippocampus CA1,CA2 and CA3.RESULTS AND CONCLUSION:The number of neurons positive for NeuN,DCX and PV in the hippocampal CA1,CA2 and CA3 in the chemotherapy group was significantly lower than that in the non-chemotherapy group.Moreover,the neurons positive for PV were smaller,with less and shorter nervous processes.Immunofiuorescence staining findings showed that the < 60 years old patients in the chemotherapy group had less neurons positive for NeuN and DCX than that in the non-chemotherapy;for > 60 years old patients,there was no significant difference between two groups.These findings suggest that chemotherapy can downregulate the expression of immature neurons in the hippocampus and reduce the number of neurons in the hippocampus.Moreover,chemotherapy can change the morphology and reduce the number of PV neurons.
3.Inducing effect of chidamide on apoptosis of multiple myeloma cells and its relerance to DNA damage response.

OBJECTIVEThis study was aimed to explore the effect of a novel histone deacetylase inhibitor Chidamide on apoptosis of human multiple myeloma(MM) cells and its relevance to DNA damage response(DDR).
METHODSThe cell proliferation was detected by MTT method, apoptosis and cell cycle distribution were analyzed by flow cytometry, the expression levels of targeted proteins were detected by Western blot, the DNA damage response was blocked by ATM kinase inhibitor KU-55933.
RESULTSChidamide inhibited RPMI 8226 cell proliferation in dose- and time-dependent manner and its IC50 values of 24,48,72 h were 9.6, 6 and 2.8 µmol/L respectively. Chidamide induced cell cycle arrest of RPMI 8226 cells in G0/G1 phase by upregulating the expression of P21. Chidamide triggered caspase-3 dependent apoptosis and upregulated expression of DDR-related proteins including γH2AX, pATM in RPMI 8226 cells. Pretreatment with ATM kinase inhibitor KU-55933 down-regulated expression of DDR related proteins induced by chidamide, thereby inhibiting DNA damage response and finally resulting in suppression of apoptotic cell death.
CONCLUSIONProliferative inhibtion, cell cycle arrest and apoptosis of multiple myeloma cells induced by chidamide involve DDR.

4.Diagnostic concordance and influencing factors of quantitative flow fraction and fractional flow reserve

Objective This study aimed to explore the diagnostic concordance of fractional flow reserve(FFR)and quantitative flow ratio(QFR)and the characteristics affecting this concordance.Methods Patients with non-acute myocardial infarction admitted to the Department of Cardiology,Peking University Third Hospital between January 2019 and December 2021 were enrolled.The patients were divided into four groups:FFR+/QFR+and FFR-/QFR-,FFR+/QFR-and FFR-/QFR+with FFR or QFR≤0.80 as positive and>0.80 as negative.Using FFR as the gold standard,the diagnostic value of QFR was analyzed,and differences in clinical features and pathological characteristics among the groups were compared.Results A total of 236 patients were included.The mean age was(64.48±9.63)years,and 67.8%were male.All patients had 30%-70%coronary stenosis.The consistency rate of QFR and FFR was 78.0%(n=184),and the Person correlation coefficient was 0.557(P<0.001).Among FFR+patients,the minimum lumen diameter was larger[(1.56±0.34)mm vs.(1.39±0.31)mm,P=0.019],lesion length was shorter[(21.37±11.73)mm vs.(36.86±18.09)mm,P<0.001],and coronary angiography-based index of microcirculartory resistance(AMR)was higher[(277.50±28.87)mmHg·s/m vs.(178.02±49.13)mmHg·s/m,P<0.001]in the disconcordance group.Multivariate regression analysis suggested that AMR[OR 0.93,95%CI 0.88-0.99,P=0.030]and lesion length[OR 1.27,95%CI 1.01-1.60,P=0.045]were independent predictors of disconcordance.In the FFR-group,the lesion length was longer[(33.08±16.05)mm vs.(21.40±13.36)mm,P=0.020],and AMR[(169.66±24.01)mmHg·s/m vs.(265.95±44.78)mmHg·s/m,P<0.001]and low-density lipoprotein-C[1.57(1.10,1.97)mmol/L vs.2.15(1.79,2.74)mmol/L,P=0.031]were lower in the disconcordance group.No statistically significant variables were identified by multivariate regression.Conclusions QFR had high diagnostic value compared with FFR.In the FFR+group,AMR and lesion length may have affected the diagnostic consistency of QFR and FFR.The study provided more evidence for the clinical application of QFR.

Objective To observe the efficacy and safety of Morinda officinalis oligosaccharides in the continuation treatment of mild and moderate depression.Methods An open,single-arm,multi-center design was adopted in our study.Adult patients with mild and moderate depression who had received acute treatment of Morinda officinalis oligosaccharides were enrolled and continue to receive Morinda officinalis oligosaccharides capsules for 24 weeks,the dose remained unchanged during continuation treatment.The remission rate,recurrence rate,recurrence time,and the change from baseline to endpoint of Hamilton Depression Scale(HAMD),Hamilton Anxiety Scale(HAMA),Clinical Global Impression-Severity(CGI-S)and Arizona Sexual Experience Scale(ASEX)were evaluated.The incidence of treatment-related adverse events was reported.Results The scores of HAMD-17 at baseline and after treatment were 6.60±1.87 and 5.85±4.18,scores of HAMA were 6.36±3.02 and 4.93±3.09,scores of CGI-S were 1.49±0.56 and 1.29±0.81,scores of ASEX were 15.92±4.72 and 15.57±5.26,with significant difference(P<0.05).After continuation treatment,the remission rate was 54.59%(202 cases/370 cases),and the recurrence rate was 6.49%(24 cases/370 cases),the recurrence time was(64.67±42.47)days.The incidence of treatment-related adverse events was 15.35%(64 cases/417 cases).Conclusion Morinda officinalis oligosaccharides capsules can be effectively used for the continuation treatment of mild and moderate depression,and are well tolerated and safe.
8.Three dimensional nephrometry system for partial nephrectomy: Our initial exploration.

OBJECTIVE:
To construct a preoperative evaluation system for partial nephrectomy using CT three-dimensional visualization technology and to explore its practical value.
METHODS:
The clinical data of the patients who underwent partial nephrectomy for renal tumors in Department of Urology, Peking University First Hospital were collected retrospectively. At the same time, the homogenized standard data of patients who underwent partial nephrectomy for renal tumors were collected in 16 clinical centers in China. The CT three-dimensional visualization system was applied (IPS system, Yorktal) to evaluate tumor anatomy, blood supply, perirenal fat and other information. The parameters were summarized to build a three-dimensional nephrometry system, on the basis of which virtual surgery design and intraoperative navigation were completed.
RESULTS:
A three-dimensional visualization image was established based on the enhanced CT urography. The nephrometry system included the longest diameter and volume of the tumor, proportion volume of tumor invading the parenchyma, maximum depth of the tumor invading the parenchyma, contact surface area, flatness of the tumor surface, renal segment where the tumor was located, vascular variation, and perirenal fat. The average two-dimensional diameter of the tumor was (2.78±1.43) cm, the average three-dimensional maximum diameter was (3.09±1.35) cm, and the average postoperative pathological size was (3.01±1.38) cm. The maximum tumor diameter in the three-dimensional image was significantly related to the prolonged renal artery clamping time and intra-operative blood loss (r=0.502, P=0.020; r=0.403, P=0.046). The three-dimensional and pathological tumor volume were (25.7±48.4) cm3 and (33.0±36.4) cm3, respectively (P=0.229). The tumor volume was significantly related to the intraoperative blood loss (r=0.660, P < 0.001). The proportion volume of the tumor invading into renal parenchyma was significantly related to the prolongation of renal artery clamping and the occurrence of postoperative complications (r=0.410, P=0.041; r=0.587, P=0.005). The tumor contact surface area and the presence of vascular variation did not show correlation with the perioperative data and postoperative complications. While the preoperative evaluation was completed, the reconstructed three-dimensional image could be zoomed, rotated, combined display, color adjustment, transparency, and simulated cutting on the Touch Viewer system. The process generally consisted of showing or hiding the tissue, adjusting the transparency of the interested area, rotating and zooming the image to match the position of the surgical patient. Together, these functions met the requirements of preoperative virtual surgery plan and intraoperative auxiliary navigation.
CONCLUSION
Three-dimensional images can provide a more intuitive anatomical structure. The CT three-dimensional visua-lization system clearly displays tumor anatomical parameters, blood supply and perirenal fat. The three-dimensional nephrometry system for renal tumors can help predict the difficulty of partial nephrectomy and perioperative complications. Importing the reconstructed three-dimensional visualization image into the specified program or robot operating system can complete virtual surgery and intraoperative navigation, helping the surgeon to better grasp the surgical process. The indexes included in the nephrometry system and the score weights of each index need to be confirmed and perfected by multi-center study with large samples.
