Cancer stem cell hypothesis has become a new theory. According to this theory, cancer stem cells play a decisive role in car-cinogenesis, progression, and metastasis. This theory offered new ideas and strategies for cancer treatment. However, only a small fraction of cells in a tumor were cancer stem cells. Therefore, the identification and enrichment of these cells are significant challenges for numerous researchers conducting further study. Micro-sphere culture has been an effective tumor stem cell enrichment technolo-gy in recent years. The application of this technique makes the cancer stem cell research process easy. In addition, it makes the evalua-tion of tumor progression and prognosis of patients in clinical possible. This article will focus on the applications of this technology, and microsystem integration will be described in detail.

Drug transporters are functional membrane proteins located in various tissues, which play vital roles in absorption, distribution and excretion of drugs, especially those located in intestine, liver and kidney. The expression and function of transporters will alter in diseases state, which affects the therapeutic effects of drugs by altering their pharmacokinetics. In this review, we focus on the alterations in related transporters and the effect on the drug therapy in common intestinal diseases, liver diseases, kidney diseases and diabetes mellitus.

Multidrug regimens and corresponding drug interactions cause many adverse reactions and treatment failures. Drug efflux transporters: P-glycoprotein (P-gp), multidrug resistance associated protein (MRP) and breast cancer resistance protein (BCRP) in conjunction with metabolizing enzymes (cytochrome P450, CYP450) are major factors in such interaction. In recent years, a large number of studies have shown that P-gp plays a role in the oxidative metabolism of its substrates that are also substrates of CYP3A4. Combined actions of P-gp and CYP3A could account in some part for the low oral bioavailability determined for many of these dual substrates. P-gp along with efflux transporters (MRP and BCRP) having overlapping substrate specificity plays critical role in drug disposition. The relationship between MRP or BCRP and CYP3A is similar to that between P-gp and CYP3A. In this paper, we summarize the classification of efflux transporters, the main metabolizing enzymes CYP3A, clinical significance interactions mediated by efflux transporters and CYP450 enzymes and in vitro studies.

Objective To investigate the preoperative management and the clinical effeciency of Lichtenstein tension-free hernioplasty in adult patients with inguinal incarcerated hernia.Methods Clinical data of 86 patients with inguinal incarcerated hernia were analyzed retrospectively.Hernia was repaired with Lichtenstein tension free after reposition.Results There were 59 male patients and 27 female patients with median age of (63 ± 18) years.There were 8 patients with liver cirrhosis.The operation was performed successfully in all patients.Segmental bowel resection with end-to-end anastomosis was performed in 38 emergency cases.Operative time was 20-120 min,with an average time of 54 min.The postoperative hospitalization was 5-17 d,with an average of 8 d.There were 7 cases of skin ecchymosis at the scrotum,there were no intestinal perforation,hepatic encephalopathy and upper gastrointestinal hemorrhage after operation.In early series of 24 cases without drainage tube left in place,there were 10 cases of fat liquefaction,10 cases of hydrops of hernial sac,6 cases of seroma and 3 cases of wound infection after operation.After 12 to 48 months of follow-up,there was no mortality after 2 years,no hernia recurrence.Conclusions Tension free repair in the treatment of incarcerated inguinal hernia is safe and feasible.

The application of four kinds of hemostatic tools in tactical combat casualty care (TCCC) of foreign armies was introduced,including extremity tourniquet,junctional tourniquet,hemostatic dressing.and elastic bandage as well as tranexamic acid.The above hemostatic tools were described from the aspects of structure,operating process,application range and technical points,and the histories of some tourniquet were also expounded.The problems of Chinese PLA were analyzed in designing and developing hemostatic tool.Some suggestions were put forward such as enhancing the practicability and portability of hemostatic tool,rational allocation of TCCC drugs and instruments and etc.

Objective To evaluate the sensitivity of patient-specific volumetric-modulated arc therapy (VMAT) quality assurance (QA) to minor multileaf collimator (M LC) positioning errors.Methods Systematic multileaf collimator (MLC) positioning errors (+0.5 mm,+ 1 mm and +2 mm) were introduced into the clinical VMAT patient plans with 2 types of MLC positioning errors:systematic MLC gap width errors and systematic MLC shift errors for 6 cases,including 3 cases with prostatic cancer and 3 cases with nasopharyngeal cancer.The planar dose distributions of the original and modified plans were measured using ArcCheck array.The coincidence between the measured results and the calculated results was evaluated using both absolute distance-to-agreement (AD-DTA) analysis with 3％/3 mm and 2％/2 mm criteria.Results The average passing rate of the 6 original VMAT plans was 96.0％ with the ADDTA criteria of 3％/3 mm which was commonly adopted in clinical practice.For the MLC gap width errors of + 1 mm,+2 mm,and-2 mm and the MLC shift errors of 2 mm,the drop levels in average passing rate with the AD-DTA criteria of 3％/3 mm were 8.8％,15.5％,6.1％ and 7.9％,respectively.The + 2 mm MLC positioning errors and + 1 mm MLC gap width errors could be detected by the patient-specific VMAT QA procedure.The AD-DTA criteria of 2％/2 mm was more sensitive compared with the criteria of 3％/3 mm.Conclusions Patient-specific VMAT QA is not sensitive enough to detect the systematic MLC positioning errors within 1 mm.Additional MLC QA is needed to guarantee the accuracy of VMAT delivery.

ObjectiveTo observe the effect of sport walking on heart and lungs functions in old women.MethodsForty-two old women aged from 60 to 69 had sport walking for 4 months; the speed and intensity were controlled according to their heart rate respectively. The indexes of respiration, circulation and rheoencephalography were measured before and after exercise.ResultsAfter exercise, blood-pumping function of heart improved obviously, stroke volume increased from (65.22±11.41)ml to (72.10± 10.78)ml, ejection fraction increased from (60.10±5.03)% to (68.78±6.25)%, while heart rate declined from (77.45±8.69) times/min to (7.89±8.21)times/min, capacity increased from (2.86±0.36)L to (3.34±0.53)L, and maximal voluntary ventilation for every minute increased from (96.14± 15.21)L to (114.02±16.01)L, significantly different compared with that before treatment ( P<0.01). The fluid time of rheoencephalography reduced from (0.171±0.058)s to (0.128±0.049)s ( P<0.01).ConclusionSport walking under proper intensity can improve the function of respiration and circulation system in old women, so it is a good way to keep health for the elders.
