1.Present situation and development strategies of Chinese medicine preparation in medical institutions.

As the actual clinical reflecting of transform Chinese medicine special curative effect, Chinese medicine preparation not only satisfies the need of hospital clinic, scientific research and teaching, but also plays an important role in deepening medical and health system reform, improving people's health level and contributing to the economic growth. However, some problems about administration and approval (tending to western medicine), contraction of the scale, lack of synchronization for clinic and scientific research, and the imbalance of regional development make Chinese medicine preparation move forwards slowly in contradiction. It has not only reduced the effectiveness of the Chinese medicine preparation in hospital clinic, but also brought bad effect on modernized development of Chinese medicine preparation. Research shows that main influencing factors of status quo of Chinese medicine preparation in medical institution include imperfect laws and regulations, high cost than income, and shortage of talents in preparation research. The analysis indicated that the necessary measures to break the contradiction, improve clinical effect of Chinese medicine, and promote the modernization development of Chinese drugs preparation were as follows: government and related departments should strengthen the supporting force in policy by adjusting the examination and approval policy, speeding up dispensing use, reforming pricing system, including into medicare reimbursement, integrating advantage resources and so on; medical institution should actively carry out research and development of traditional Chinese Medicine through drawing the traditional and modern essence, reserving professional talents, and developing characteristic preparation; companies cooperate with hospitals for complementary advantages, which can rapidly transform Chinese medicine preparation into clinical practice.

3.Neurobiological mechanisms and interventions in drug craving after withdrawal

4.Therapies of simvastatin on AECOPD combined with PAH through affecting VEGF and the pulmonary function

Objective To assess the effect of simvastatin on secretion of VEGF in blood serum and sputum of elderly chronic ob-structive pulmonary disease(COPD)combined with PAH patients and the pulmonary function.Methods Seventy cases of elderly COPD patients combined with PAH patients were randomized in two groups,namely the normal treatment group and combined sim-vastatin group (35 cases in each group),then compare the changes of the VEGF in blood serum,sputum and lung function.Results VEGF in blood serum and sputum showed significant changes after therapies of simvastatin (P <0.05).After 4 weeks′therapy, the FEV1% pre and FEV1/FVC increased significantly (P <0.05).Conclusion Normal treatment combine with simvastatin could decrease VEGF in blood serum and sputum of elderly COPD combined with PAH patients,meanwhile improve pulmonary function.
5.The standardization of a Uighur Aphasia battery

Objective To study the reliability and validity of an aphasia battery of Uighur (ABU).Methods The Chinese Aphasia Battery (ABC) was translated into Uighur.Appropriate adaptation was made according to the features of Uyghur language,but the overall structure was unchanged.Three rounds of comments and revisions were conducted by experts and the ABU was back-translated into Chinese after language adjustment and pretesting.The ABU was then applied to evaluating 104 cases of stroke aphasia among Uighur speakers,among which 30 cases were retested one week later.The construct validity,internal reliability,retest reliability and splithalf reliability of the scale were tested using factor analysis and Spearman rank correlation analysis.Results The discrepancy between the original and back-translated ABC was less than 10%.The Kaiser-Meyer-Olkin value was 0.917 and the p value of the Bartlett's test for sphericity was ≤0.005.Three common factors were extracted using principal component analysis with a varimax orthogonal rotation:a writing factor;and a listening comprehension factor.Together they explained a total of 81% of the variance.The Cronbach's α of the components were 0.969 for oral expression,0.922 for listening comprehension,0.966 for reading and 0.924 for writing,giving a total α coefficient of 0.978.The test-retest correlation coefficients of 22 items were greater than 0.8,with only another two below 0.7.The split-half reliability of the scale was 0.906.Conclusions The aphasia battery in Uighur has good validity and reliability,and can quantitatively reflect the functional status and language changes among Uighur's with stroke aphasia.It may also be suitable for application in the clinical rehabilitation of other sorts of Uighurspeaking aphasics.
6.The random and comparative study on therapeutic effect and safety between Levetiracetam and Phenobarbitaladministration for neonatal seizures

Objective To randomly compare the therapeutic effect and safety between Levetiracetam (LEV) and Phenobarbital (PB) in the treatment of neonatal seizures.Methods A total of 61 infants with acute convulsion were randomly divided into 2 groups:LEV group (n =30) and PB group (n =31) during January 2013 to December 2014 in Urumqi Children's Hospital.All neonates received routine management including etiology treatment and adverse drug reaction monitoring.In the LEV group,subjects received oral formulation of LEV with initial loading dose 30 mg/kg,followed by 15 mg/kg twice a day.If the seizures were not controlled completely,PB treatment was added until seizures were completed controlled.If seizures were controlled quickly,the dose of PB was gradually reduced and LEV was used as monotherapy.The subjects in PB group received intramuscular or intravenous injection of PB with 10 mg/kg as the first dose,then 5 mg/(kg · d) oral PB was administered,if seizures were not controlled,LEV treatment was added,then dose of PB was gradually reduced until seizures were controlled completely,and then patients were switched to LEV monotherapy gradually.The drug adverse reactions were observed.Results (1) After LEV or PB monotherapy,66.7% (20/30 cases) and 54.8% (17/31 cases) of the subjects obtained sustainable seizure free respectively.Although,there was a higher control ratio in LEV group,but no significant difference was observed between the 2 groups (P >0.05).(2) LEV group (16/30 cases,53.33%) had higher rapid seizure control ratio with seizure controlled within 24 h after first dosage administration than that of PB group (8/31 cases,25.80%),and there was significant difference (x2 =4.841,P =0.028).Further more,if adding the cases who had to change to use another comparative one (LEV or PB) due to their seizures failure control with the first one treated,LEV group (21/44 cases,47.72%) still had higher rapid seizure control ratio in total patients than that of PB group(10/41 cases,24.39%),and there was significant difference (x2 =4.988,P =0.026).(3) Eight cases who changed to LEV after PB as the first treatment drug failed obtained sustainable seizure free.(4) One case in PB group with transient urinary retention was observed but the symptom disappeared 36 h after PB withdrawal,and no significant drug adverse reaction was observed in LEV group.Conclusion LEV is more rapid and safe for seizure control of neonates than PB.
7.Development of Chinese herbal pieces and analysis of problems of total quality management.

Chinese herbal pieces are a key factor to protecting the quality of the clinical efficacy of traditional Chinese medicine (TCM), and it is one of the basic elements of ensuring the quality of TCM and people's usage safety. However, Chinese herbal pieces has massive problem such as adulteration and counterfeit, dyeing and weighting, pesticide residues, heavy metals in excess of the standards, and all the issues are repeated excessive in the clinic treatment. These issues impacted sound development of production, management and use of TCM, but also brings common people hidden trouble for the clinical safety of medication. Protect and improve the quality of the Chinese herbal pieces demand that continue improve quality system, in-depth scientific research, and strengthen self-discipline and other factors. So it is fundamentally to ensure good quality of Chinese herbal pieces with the color, taste and shape by systematic supervision to it from the source, production, management and research, with strengthened implementation and en- forcement of the "3G".

8.Effect of dexmedetomidine on intraoperative wake-up test in patients undergoing scoliosis surgery

Objective To evaluate the effect of dexmedetomidine on intraoperative wake-up test in patients undergoing scoliosis surgery.Methods Sixty ASA Ⅰ patients,aged 13-18 yr,scheduled for elective scoliosis surgery,were randomly divided into 2 groups (n =30 each) ∶ sufentanil group (group S) and dexmedetomidine combined with sufentanil group (group DS).Anesthesia was induced with sufentanil,etomidate and cisatracurium,and maintained with target-controlled inhalation of sevoflurane combined with target-controlled infusion of sufentanil in both groups.In group DS,dexmedetomidine 0.2 μg·kg-1 · h-1 was infused during operation,while the equal volume of normal saline was given instead in group S.Before the wake-up test,cisatracurium infusion was stopped,the end-tidal concentration of sevoflurane was adjusted to 0,and the target effect-site concentration of sufentanil was adjusted to 0.1 and 0.08 ng/ml in groups S and DS,respectively.The wake-up test was performed 5 min later.The wake-up time and bucking/restlessness and cardiovascular events during the wake-up test were recorded.The occurrence of intraoperative awareness was recorded in the patients who were followed up after operation.Results No cardiovascular events occurred during the wake-up test in group DS.The wake-up time was significantly shorter and the incidences of cardiovascular events and bucking/restlessness were significantly lower in group DS than in group S (P < 0.05).No intraoperative awareness occurred in the patients.Conclusion Dexme-detomidine can be used for intraoperative wake-up test in patients undergoing scoliosis surgery,with shorter wakeup time,stable hemodymamics and fewer adverse reactions.
9.Effect of dexmedetomidine combined anesthesia on postoperative delirium in elderly patients undergoing hip joint replacement

Objective To investigate the effect of dexmedetomidine combined anesthesia on postoperative delirium in the elderly patients undergoing hip joint replacement.Methods One hundred and sixty ASA Ⅱ or Ⅲ patients,aged 75-85 yr,with a body mass index of 20-25 kg/m2,scheduled for elective total hip joint replacement under general anesthesia,were randomly divided into 2 groups (n =80 each):sevoflurane-fentanyl control group (group C) and dexmedetomidine-sevoflurane-fentanyl group (group D).Dexmedetomidine was intravenously infused at 0.2 μg· kg-1 · h-1 until the end of operation after a loading dose of 0.6μg/kg over 15 min before induction of anesthesia in group D,while the equal volume of normal saline was infused in group C.Anesthesia was induced with iv injection of fentanyl 2-4 μg/kg and etomidate 0.1-0.3 mg/kg at 5 min after the end of administration of the loading dose.Tracheal intubation was facilitated with cisatracurium 0.15 mg/kg and the patients were mechanically ventilated.Anesthesia was maintained with target-controlled inhalation of sevoflurane (the end-tidal concentration 1%-3%) and intermittent iv boluses of fentanyl 0.05 mg.BIS was maintained at 40-60 during operation.The consumption of etomidate,fentanyl,and sevoflurane,agitation during emergence from anesthesia,and postoperative delirium within 24 h after operation were recorded.Results Compared with group C,the consumption of etomidate,fentanly,and sevoflurane was significantly reduced and the incidences of agitation during emergence from anesthesia and postoperative delirium within 24 h after operation were significantly decreased in group D (P <0.05).Conclusion Dexmedetomidine combined anesthesia can reduce the occurrence of postoperative delirium in the elderly patients undergoing hip joint replacement.
