Experimental incised wounds 8 mm in diameter were made on the right side of the back of 48 rats under anesthesia using aseptic technique. All animals were randomly divided into 12 groups according to the time of wound biopsies which were taken immediately after skin cutting, lh,12h,24h,2,3,4,5,6,7,8,9 and 10 days after wounding.One rat of each group was given colchicine before animal sacrifice in order to observe cell mitosis. Skin specimens were fixed in 10% formalin,embeded in paraffin and sectioned for HE stain,Gorden and Sweet stain for reticular fiber,Weigert stain for elastic fiber,PTAH stain for fibrin and Van Gieson stain.Macroscopically, wounds were getting smaller daily,and reduced in size markedly on the 6～7th day. The histologic changes were as follows:Acute inflammation on the wall of wounds occurred 1h after skin cutting;profuse fibrin exudation at the top of the base and on the wall of wounds at 24h;new capillary formation and regeneration of the epidermis on the 2nd day;granulation tissue formation and collagen fiber deposition on the 3rd day;small artery formation in the granulation tissue on the 4～5th day;granulation tissue maturation and the regenerating tissue covering the whole wound area on thd 6～7th day;the scar formation and contraction on the 8～9th day;further sear contration on the 10th day.The epidermis regenerated from the proliferation of the epidermis at the edge of the wound with the speed of 1.23mm/day. The regeneration of the epithelium proceeded under the necrotic tissue on both the wall and at the base of wounds. The mechanism of the wound contraction was discussed.

Objective To explore the influence of intravesical chemotherapy combined with hyperthmia on levels of insulin growth factor( IGF) and tumor necrosis factor( TNF) in bladder cancer after operation. Methods 64 cases of transitional cell carcinoma of bladder accepted TURBt were randomly divided into two groups: intravesical chemotherapy combined with hyperthmia group and intravesical chemotherapy group. Levels of ICF and TNF in two weeks before therapy 、two weeks after therapy, four weeks after therapy were tested. Results The IGF levels were (65.97±5.25)μg/L,(49.93±1.98)μg/L,(36.82±1.89)μg/L in intravesical chemotherapy combined with hyperthmia group and(61.87 ±5.41)μg/L, (57.86 ±2. 18) μg/L, (48. 68 ±2. 26) μg/L in intravesical chemotherapy group. The TNF levels were(2.32 ±0. 36)μg/L, (3. 84 ±0. 31) μg/L, (8. 79 ±0. 46)μg/L in intravesical chemotherapy combined with hyperthmia group and(2. 21 ±0. 19)μg/L,(2. 89 ±0.47) μg/L, (3. 87 ±0.61)μg/L in intravesical chemotherapy group. The influence of intravesical chemotherapy with hyperthmia on levels of IGF and TNF were significant than intravesical chemotherap, and the influence degree showed in time-dependent manner (all P ＜ 0. 05 ). Conclusion Intravesical chemotherapy combined with hyperthmia had curative effect in inhibiting the recurrence of tumor after operation.

AIM: To investigate the differences between synoptophore and triple prism strabismometry and its possible cause.?METHODS: There were 347 patients with horizontal concomitant strabismus involved, in which 76 patients were esotropia, 37 patients were male while 39 were female, with average age of 13. 27 ± 7. 77 years old. There were 271 patients with exotropia, 131 cases were male while 140 were female, with average age of 15. 43 ± 8. 42 years old. All the patients were examined by synoptophore and prism plus shaded strabismometry in a long distance of 6m. Datas were analyzed by SPSS 17. 0.?RESULTS:In the exotropia patients, the conversions of circular degree(°) and prism degree(△) were:1°=0. 29△ ~1. 78△, which was statistically significant with intermittent strabismus (P=0. 001). While in the esotropia patients, the conversions were:1°=2. 01△ ~2. 15△.?CONCLUSION: The diversity between the two methods is enlarged with the increase of squint angle for exotropia patients. While in esotropia patients, the diversity decreased with the increase of squint angle. Synoptophore equipped with +7. 00D, defects of the triple prism itself and proximal convergence during exam may be the reasons for the diversity.

Objective To study the scan parameter of spiral CT angiography in aortic dissection.Methods 34 cases with aortic dissection successively underwent SCTA were studied retrospectively.The CT scan parameters,methods of posteriorimage treating were reviewed and analysed one by one.Results In 34 cases,32 cases were diagnosed as aortic dissection and classified correctly,in accordance with the result of operation and DSA,the quality of image was satisfied and the rate of success at least above 88.9%.The use of main scan parameters:(1)the ascend aortic dissection:slice thickness 4 mm,reconstruction interval 2 mm,pitch 1.25;tube current 175 mA;(2)involed in ascend,arch,descend and throacic aorta dissection:slice thickness 5 mm,reconstruction interval 2～3 mm,pitch 1.5;tube current 150 mA;(3)involved in abdominal aortic dissection:slice thickness 6～8 mm,reconstruction interval 2～3 mm,pitch 1.5 or 1.75;tube current 125 mA.The tube voltage all were 120 kV,the dosage of contrast media was 90～100 ml;the delayed scan time was choiced 20 second in throacic aortic and 25 second in abdominal aortic.The posterior image methods main used MPR,SSD,MIP and VR. Conclusion To set a sensible scaning plan,choose and match scan parameters properly according to the scaning length,can avoid the shortage of restrain SCTA scaning length and get satisfied image.

At the same time, Through introducing various experience of the design experiment teaching, resolvent and way of thinking against problem meeted in design experiment teaching were put forward.

Objective To summarize the results of valve re-operative cases after cardiac valve replacement, to find the better re-operative time, and to estimate the re-operative methods and influencing factors of the operation. Methods Thirteen valve re-operative cases after cardiac valve replacement from October 2008 to February 2010 were retrospectively studied. According to NYHA classification, 9 cases belonged to class Ⅳ, and only 4 cases belonged to class Ⅲ preoperatively. Mitral valve replacement (MVR)was performed in 7 cases, aortic valve replacement (AVR) in 3 cases, MVR + AVR in 3 cases. Results The early-stage postoperative mortality was 7.7%( 1/13),and the reason was low cardiac output syndrome. Two cases who underwent re-operation and re-intubation respectively after operation for hemorrhage were improved after treatment. Twelve cases were discharged in 3-6 weeks after heart valve surgery and all were followed up for 6-15 months. The cardiac function of all the discharged patients recovered well and no death occurred during follow-up. Conclusion The key factors to reduce the death of re-operation are improving preoperative heart function,setting up extracorporeal circulation as soon as possible,consummating myocardial preservation,perfecting operating skills,correcting low cardiac output syndrome in time and preventing complications.

ObjectiveTo explore the surgical treatment of congenital heart disease combined with severe pulmonary hypertension.MethodsThirty-two patients of congenital heart disease combined with severe pulmonary hypertension were included in the study.The saturation of arterial oxygen ranged from 0.84 to 0.94 and echocardiography showed left to right slow velocity shunt in 20 eases,double direction shunt in 10 cases and no shunt in 2 cases.The pulmonary arterial pressure was 65-120 (82 ± 14) mm Hg ( 1 mm Hg =0.133 kPa).All patients received surgical repairs under cardiopulmonary bypass and were treated preoperatively with oxygen inhalation therapy,oral intake of beraprost sodium or silaenafil respectively according to the degree of pulmonary hypertension.During cardiopulmonary bypass,pulmonary artery perfusion was performed with protective solution containing ulinastatin for lung protection.Vasoactive drugs were routinely administrated postoperatively.Results Thirty-two cases underwent the surgical treatment successfully with their postoperative pulmonary arterial pressure decreased 20 -40 mm Hg and 2 cases died of severe low cardiac output syndrome and fatal arrhythmia.The mortality was 6.25％(2/32).ConclusionComprehensive perioperative management followed by radical operation can achieve a good clinical result on congenital heart disease combined with pulmonary hypertension.

ObjectiveTo investigate the myocardial protection on cardiac valve replacement surgery with creatine phosphate of myocardial GIK (GIK) in order to reduce the surgical risk and improving the efficacy.Methods126 cases were unergone surgical treatment of heart valve disease,whose cardiac function on admission wereⅢor Ⅳ. 126 patients were randomly divided into two groups. Cardiopulmonary bypass time, aortic cross clamp time, postoperative myocardial injury markers ( CK-MB, cTNI) changes, arrhythmias, heart function recovery, length of stay and mortality rate and other indicators were compared between the two groups. ResultsThe age, gender, body mass,heart disease and surgery combined data were not statistically significant between the two groups( all P ＞0. 05). The cardiopulmonary bypass time, aortic cross clamp time and mortality had no significant differences between the two groups( all P ＞ 0.05 ). The CK-MB( 21.36± 9.21 ) U/L and cTNI(0.83 ± 0. 35 ) ng/ml of creatine phosphate group were significantly lower than those of the control group. The incidence of arrhythmia in phosphocreatine group (37. 1％ ) was significantly lower than ordinary group (57.8 ％ ) ( X2 ＝ 5. 418, P ＜ 0. 05 ). ConclusionThe application of creatine phosphate GIK before valve replacement surgery could effectively reduce reperfusion injury after myocardial ischemia,myocardial protection,and significantly reduce the incidence of arrhythmia and improve heart function in patients.
