2.Analysis of the research status of private hospital in China based on bibliometric method

Objective To analyze the research status of private hospitals in China for policy recommendations of their development.Methods Using CNKI as the object to retrieve,and key words ofprivate hospital ,social capital to run hospitals or the titles of'private hospitals ,social capital to run hospitals to retrieve 853 articles that are valid and published since end of 2013,for a biliometric analysis.Results The number of articles published were increasing year by year;the most of the articles published in the developed southern areas;the number of articles in hospital internal management was the largest (384 articles,45.02%),followed those on external management and development situation and countermeasures (accounting for 27.43% and 22.39% respectively).Conclusion The increase of articles on private hospitals is connected with the support of national policy,the research of private hospitals is not in balance and its development environment is not favorable.
3.Clinical study of carotid elasticity in subjects with different plasma glucose level by wave intensity

Objective To assess the carotid elasticity using wave intensity(WI) in subjects with different plasma glucose level.Methods 107 subjects were enrolled in this study,according to their plasma glucose levels,subjects were categorized as normal plasma glucose group (group A),normal plasma glucose in higher level group (group B) and pre-diabetes group (group C).Carotid WI examination was performed and analyzed.The parameters included magnitude of the peak during late systole (W2),negative area during the mid-ejection (NA),and stiffness parameter (β),pressure strain elasticity modulus (Ep),arterial compliance(AC),augmentation index(AI),one point pulse wave velocity(PWVβ).Results Compared to group A,W2,β,Ep,PWVβ increased significantly while AC decreased in group C(P <0.05),but there was no obvious difference of NA between two groups.Furthermore,this statistically difference was not found in group B(P >0.05).Conclusions Carotid elasticity have been altered in pre-diabetes group which can be evaluated by WI,but no marked change is observed in normal plasma glucose of higher level group.
4.Comparison of characteristics between indocyanine green and fundus fluorescence angiography in patients with age-related macular degeneration

Objective To compare the characteristics between indocyanine green angiography (ICGA) and fundus fluorescence angiography ( FFA ) in patients with age-related macular degeneration ( ARMD ). Methods Twenty-five cases ( 35 eyes ) were diagnosed as exudative ARMD by ocular examination. Their age ranged from 60 to 81 years. The visual acuities were 1.0 at FC/30 cm before eye. We analyzed and compared the characteristics of FFA and ICGA in these patients. Results 5 eyes ( 14.3% ) with classic choroidal neovascularization ( CNV ) were found by FFA examination. 15 eyes ( 42.9% ) with classic CNV were found by ICGA examination. Of 20 eyes with occult CNV considered by FFA, 7 eyes had classic CNV by ICGA. Of 3 eyes considered as serous retinal pigment epithelial detachment ( PED ) without classic CNV by FFA, 1 eye had serous PED with classic CNV and 2 eyes had classic CNV by ICGA.1 eye was diagnosed as serous PED with CNV by FFA, and as occult CNV by ICGA. Conclusions ICGA raise detectability and accuracy of CNV in ARMD compared with FFA. It is contributive to early treatment in ARMD.

6.Optimization of extraction technology of compatibility of couplet medicines Epimedium brevicornu and Ligusticum chuanxiong by orthogonal design.

The study was using the orthogonal test and making the extraction rates of icariin, ferulic acid, epimedin A, epimedin B, epimedin C, baohuoside I and ligustilide determinated by HPLC multiwavelength switch, gradient elution and multi-index comprehensive weighted scoring method (weight coefficient was 0.47: 0.16: 0.07: 0.07: 0.08: 0.06: 0.09) as evaluation index, combine with SPSS 16.0 software to optimizing the best extraction. It was Yinpian soak 1 h, 12 times more than the volumn of 50% ethanol solution, by heating reflux extraction for 60 min. The compliance test indicates that the optimized compatibility extraction technology is stable and practical, and it has provided an experimental basis for compound preparation technology research of Epimedium brevicornu and Ligusticum chuanxiong.

7.Efficacy assessment of sodium valproate in treating 607 patients with epilepsy in rural China

Objective To observe the efficacy and the side-effects of sodium valproate (VPA) in patients with epilepsy in rural China. Methods Epilepsy patients were selected from rural areas of Tianmen in Hubei province and Tiandong county in Guangxi province according to the inclusion and exclusion criteria.Efficacy evaluation standard depending on the change of seizure frequency compared with the situation prior to entering the treatment group. Among the treated patients, no seizures, seizure reduced > 75%, seizure reduced 50%-75% sums for the total effective rate. Results All 607 patients with epilepsy were treated and followed up, the male were 395 (65. 1% ) female 212 (34. 9% ), and 579 patients were treated for 12 months. Patients with generalized tonic-clonic seizures were 517 (85.2%), absence seizures 20 (3. 3% )and the other types of seizures 70 (11.5% ) including simple partial seizures, tonic, clonic, myoclonic or atonic seizures etc. The completed control of seizures ( without any seizures) during the period after taking 3 months, 6 months, 12 months were 270 (45.5%), 249 (42. 3% ) and 238 (41.1%) respectively. The total effective rates in the three periods were 65.2% , 75.4% and 85.5% respectively. The 58 patients in the total group showed side-effects during the follow-up period, mainly including drowsiness, lethargy,fatigue, dizziness, headache and tremor. Nineteen patients (3. 1% ) quitted the treatment group.Conclusions The VPA is an effective antiepileptic drug and no more severe side-effects. It is the same as Phenobarbital and suitable to be used in rural areas of China.
8.A short-term safety study of tumor necrosis factor antagonists in rheumatic diseases

Objective To evaluate the safety and tolerance of tumor necrosis factor-or (TNF-α)antagonists in the treatment of rheumatic diseases. Methods The incidence of adverse events and their ultimate outcomes based on the clinical symptoms, signs and laboratory parameters of patients treated with etanereept or infliximab during January 2007 to October 2008 were analyzed. Results Severty eight patients were included. Most were rheumatoid arthritis (35%) and ankylosing spondylitis (41%) patients. Few of them were psoriasis arthritis (17%) patients and undifferentiated spondyloarthropathy (6%) patients. Among those patients, 59 patients were treated with etanercept, 7 patients (12%) had adverse events in which the majority were injection reactions, upper respiratory tract infection and tuberculosis. Nineteen patients were treated with infliximab, in which 3 patients (16%) had adverse events. One patient (AS) had upper respiratory tract infection. One case (AS) had red papules all over the body and palpitations in the first 24 hours after two infusions. One patient (RA) had fever without identifiable causes after the 4th infusion. Some of the adverse reactions might subside without intervention, while others were controlled after proper treatment. Conclusion Both etanercept and infliximab have good safety and tolerance in treating rheumatic diseases, the adverse reactions are generally mild and can be controlled by appropriate treatment.
