1.Points selection pattern in acupuncture-moxibustion treatment of gouty arthritis: an analysis based on bibliometrics

Objective: To discuss the selection pattern of points and meridians in acupuncture-moxibustion treatment of gouty arthritis (GA) by analyzing clinical literatures related to GA, and to provide reference for acupuncture-moxibustion treatment of GA. Methods: Clinical literatures related to GA treated with acupuncture-moxibustion published between January 1981 and July 2017 were collected from Chinese databases. The points, meridians, frequency and treatment methods were analyzed. Results: A total of 78 studies were included. For meridians, the top 5 meridians used were the Spleen Meridian (67 times), Liver Meridian (59 times), Stomach Meridian (53 times), Kidney Meridian (49 times) and Large Intestine Meridian (44 times). For points selection, the leading 5 points were Taichong (LR 3) (79 times), Taibai (SP 3) (68 times), Sanyinjiao (SP 6) (61 times), Taixi (KI 3) (57 times) and Yinlingquan (SP 9) (55 times). For major and adjunct points, the 5 most significant major points were Taichong (LR 3) (79 times), Taibai (SP 3) (68 times), Sanyinjiao (SP 6) (61 times), Yinlingquan (SP 9) (37 times) and Taixi (KI 3) (32 times). The 5 most significant adjunct points were Yanglingquan (GB 34) (31 times), Fenglong (ST 40) (30 times), Xuehai (SP 10) (29 times), Taixi (KI 3) (25 times) and Quchi (LI 11) (21 times). For treatment methods, the top 5 methods used were acupuncture-moxibustion alone for 15 studies, acupuncture combined with medicinal herbs for 11 studies, acupuncture-moxibustion combined with medicinal herbs for 10 studies, acupuncture alone for 7 studies, and electroacupuncture combined with bloodletting for 5 studies. Conclusion: The main meridians chosen in acupuncture-moxibustion treatment of GA were the Spleen, Liver, Stomach, Kidney and Large Intestine Meridians, conforming to the syndrome differentiation principle of tonifying spleen and kidney, clearing heat and draining dampness. Points mainly locate at lower limbs and feet around the affected area, which was a reflection of peripheral treatment function of points. The data mining results of meridian and point selection in acupuncture-moxibustion treatment of GA conform to the disease and syndrome differentiation theory and provide references for acupuncture-moxibustion treatment of GA.
2.Observation of the effect of perioperative nutritional support for patients with esophageal cancer

Objective To investigate the clinical effect of preoperative nutritional treatment and perioperative nutrition support for the patients with esophageal cancer.Methods According to the digital table,90 patients with esophageal cancer were divided into the control group(15 cases),preoperative nutrition group(25 cases),postoperation nutrition group(25 cases)and preoperative nutrition + postoperation nutrition group(25 cases).The postoperative quality of life and the complications were compared.Results Postoperative quality of life in the preoperative nutrition group,postoperation nutrition group and preoperative nutrition + postoperation nutrition group were all better than that in the control group(t =14.76,15.13,17.87,all P < 0.05).The incidence rate of postoperative complications in patients received nutritional intervention significantly decreased than in the control group(x2 =9.17,12.13,19.18,all P < 0.05).Conclusion The preoperative nutrition and postoperation nutrition can improve the postoperative quality of life and reduce the incidence rate of the complications.

5.Effect of glutaredoxin on oxidative stress of umbilical vein endothelial cell exposed to Porphyromonas gingivalis lipo- polysaccharide.

OBJECTIVEThis study measures the glutaredoxin (Grx) gene and protein expression in umbilical vein endothelial cells upon exposure to Porphyromonas gingivalis (P. gingivalis) lipopolysaccharide (LPS). The involvement of the Akt-signaling pathway is also determined.
METHODSEA-hy926 cells were pretreated with 1,000 ng · mL⁻¹ P. gingivalis LPS for 4, 12, 18, and 24 h, and then real-time reverse transcription polymerase chain reaction was employed to detect Grx1 expression. The effect of Grx on Akt activity was investigated using Western blot for the control, LPS (1,000 ng · mL⁻¹ LPS), and carmus- tine (BCNU) groups (1,000 ng · mL⁻¹ LPS, and the EA-hy926 cells were pretreated with 25 μmol · ml⁻¹ BCNU for 30 min).
RESULTSGene expression of Grx1 significantly increased in LPS group compared with that in the control group. The Grx1 expression reached the peak level in 12 h, and the variation between the expression in 4 and 12 h was significant (P < 0.05). After 12 h, the protein levels of Grx and phosphorylated-Akt (p-Akt) significantly increased in the LPS group (P < 0.05), whereas the BCNU group showed a considerable decrease in both Grx and p-Akt expression levels (P < 0.05). Moreover, a slight difference was observed in the total Akt protein levels in the three groups (P > 0.05).
CONCLUSIONGrx expression increased upon exposure of EA-hy926 cells to the LPS. Akt activity could be inhibited by BCNU (a Grx inhibitor), which indicated that Akt might act as a downstream regulator of Grx.

6.The effects of sulforaphane on the proliferation and apoptosis of salivary adenoid cystic carcinoma ACC-M cells

Objective:To study the effects of sulforaphane(SFN)on the proliferation and apoptosis of salivary adenoid cystic carci-noma ACC-Mcells in vitro.Methods:ACC-Mcells were treated with SFN at the doses(μmol/L)of 5,10,20,30,40,60,80 and 100 for 24,48 and 72 hours,respectively.The growth inhibition was examined with MTT assay and typan blue exclusion assay.Morpholo-gy of ACC-M cells was observed with phase contrast microscope,giemsa staining and transmission electron microscope.Flow cytome-try with Annexin-V-FITC/PI double staining was used to detect the apoptosis of ACC-M cells.Results:SFN inhibited the prolifera-tion of ACC-Mcells,the IC50 values(μmol/L)after 24,48 and 72 h treatment were 75.6,21.3 and 16.5 respectively.The highest inhibition rate was 89.2%.The growth inhibition rate of SFN on ACC-Mcells was positively correlated with concentrations of SFN and treatment time.SFN induced the apoptosis of ACC-Mcells in a dose and time dependent manner(P<0.01).Conclusion:SFN can inhibit proliferation and induce apoptosis of salivary adenoid cystic carcinoma ACC-M cells time and dose-dependently.

In this study, Professor Yan Zhenghua's recipes for treating heart diseases were collected to determine the frequency and association rules among drugs by such data mining methods as apriori algorithm and complex system entropy cluster and summarize Pro- fessor Yan Zhenghua's medication experience in treating heart diseases. The results indicated that frequently used drugs included Salviae Miltiorrhizae Radix et Rhizoma, Parched Ziziphi Spinosae Semen, Polygoni Multiflori Caulis, Ostreae Concha, Poria; frequently used drug combinations included "Ostreae Concha, Draconis Os", "Polygoni Multiflori Caulis, Parched Ziziphi Spinosae Semen" , and "Salviae Miltiorrhizae Radix et Rhizoma, Parched Ziziphi Spinosae Semen". The drug combinations with the confidence of 1 included "Dalbergiae Odoriferae Lignum-->Salviae Miltiorrhizae Radix et Rhizoma", "Allii Macrostemonis Bulbus-->Parched Ziziphi Spinosae Semen", "Draconis Os-->Ostreae Concha", and "Salviae Miltiorrhizac Radix et Rhizoma, Draconis Os-->Ostreae Concha". The core drug combinations included" Chrysanthemi Flos-Gastrodiae Rhizoma-Tribuli Fructus", "Dipsaci Radix-Taxillus sutchuenensis-Achyranthis Bidentatae Radix", and "Margaritifera Concha-Polygoni Multiflori Caulis-Platycladi Semen-Draconis Os".

8.Comparison of the effect of medical calcium sulfate local bone graft and simple focus debridement for the treatment of joint tuberculosis

Objective To evaluate the clinical outcome of Osteoset artificial bone mixed Rifampicin for injection after radical debridement in the treatment of sacroiliac joint tuberculosis,and compare with debridement alone.Methods From May 2005 to August 2010,48 patients with sacroiliac joint tuberculosis patients underwent two different surgical methods were selected as analysis subjects in the Orthopedics Department of Chest Hospital of Hebei Province.Among them,27 cases underwent radical debridement and fusion with Osteoset artificial bone combined with Rifampicin for injection (Grafting group),and 21 case were treated with radical debridement only(Control group).The local wound healing,adverse reactions and regular imaging examination (pelvic X-ray and CT scan) were observed,and the surgical time,the intraoperative blood loss,postoperative 6 months and 12 months,18 months bone graft fusion rate,and Majeed scoring results of the patients were recorded and statistically analyzed.Results All patients were followed up from 20 to 30 months with an average of 24 months.There was no significant difference in terms of surgical time and operative bleeding between the two groups(P>0.05).The rate of bone fusion in grafting group was 33.33% (9/27) of 6 months after operation,88.9%(24/27) of 12 months after operation,and 96.3% (26/27) of 18 months after operation.While that in control group was 4.76% (1/21) of 6 months after operation,42.85 % (9/21) of 12 months after operation,and 42.85% (9/21) of 18 months after operation.The differences of these parameters between the two groups were significant(x2 =5.85,20.92,15.90;P<0.05).According to Majeed scoring system,the excellent and good rates were 100% (27/27) in grafting group,76.19% (16/21) in control group,the difference was statistically significant(x2=7.18 P<0.05).Conclusion With Rifampicin loaded Osteoset artificial bone can recruit bone graft mass during the bone fusion for sacroiliac joint tuberculosis,and reduce the adverse reactions,achieve bone fusion earlier than the control group.
9.Quantitative Analysis on Staging of Chronic Viral Hepatitis B and Hepatic Fibrosis by Real-time Shear Wave Elastography

Objective To explore the application value of real-time shear wave elastography (SWE) technique in diagnosing and staging of chronic viral hepatitis B and hepatic fibrosis and to establish Young's modulus reference range for diagnosing and staging of hepatic fibrosis.Methods Forty-eight patients with chronic hepatitis B and fifty-eight healthy adults were enrolled and their Young's modulus values of S5 and S6 segments of liver were measured.Histopathologic examination was performed on 48 patients with chronic hepatitis B.Comparative analysis was conducted between the pathological findings and Young's modulus values,by means of which Young's modulus reference range for diagnosis and staging of hepatic fibrosis was obtained.Results There was significant difference in Young's modulus values of S5 and S6 segments of liver between chronic hepatitis B group and the normal control group (P<0.05).Young's modulus values of S5 and S6 segments of liver in chronic hepatitis B group were (11.7 ± 2.9) kPa and (12.1 ± 3.2) kPa respectively,which were significantly higher than those in the normal control group,(5.7 ± 1.1) kPa and (5.8 ± 1.3) kPa respectively.Significant differences of Young's modulus values were detected in every staging of hepatic fibrosis (P<0.05).S5 segment of liver Young's modulus values in S0-S4 stages were (5.8 ± 2.2) kPa,(7.3 ± 1.9) kPa,(10.3 ± 2.8) kPa,(10.3 ± 2.8) kPa,and (25.3 ± 3.6) kPa,respectively.S6 segment of liver Young's modulus values in S0-S4 stages were (5.7 ± 2.3) kPa,(9.2±2.1) kPa,(10.5±2.1) kPa,(14.7±4.5) kPa,and (26.1 ±2.1) kPa,respectively.Young's modulus value of the liver rose with the increase of S stage.Conclusion SWE technique can establish the Young's modulus reference range for hepatic fibrosis stage.Besides,it features high sensitivity,specificity and accuracy.
10.Case-control study on hidden blood loss in perioperative period of femoral intertrochanteric fractures in elderly patients treated with intramedullary nailing.

OBJECTIVETo analyze the neglected hidden blood loss before and after operations in the elderly patients with intertrochanteric fractures and strengthen the treatment and clinical nursing in the perioperative period in order to ensure clinical effects.
METHODSThe clinical data of 99 patients with intertrochanteric fracture treated with intramedullary nailing from January 2010 to January 2014 were retrospectively analyzed (47 males and 52 females). Forty-seven cases got blood transfusion supporting and 52 cases were not (blood transfusion group and no-blood transfusion group respectively). According to the Gross equation, the average hemoglobin (Hb) and hematocrit (HCT) were used to analyze blood loss in perioperative period and assess the patients' hidden blood loss situation.
RESULTSThere were 22 males and 30 females in blood transfusion group. The average operative time was (62.13±4.01) min; intraoperative visible blood loss and postoperative drainage were 215 ml, totally; preoperative Hb was (103.22±9.01) g/L and postoperative was (81.13±6.20) g/L; preoperative HCT was (96.93±3.38) I/L and postoperative was (308.00±11.81) I/L. There were 25 males and 22 females in no-blood transfusion group. The average operative time was (60.12±3.27) min; intraoperative visible blood loss and postoperative drainage were 196 ml, totally; the average blood transfusion were 621 ml; preoperative Hb was (92.15±5.46) g/L and preoperative was (95.20±8.93) g/L; preoperative HCT was (96.52±3.63) I/L and preoperative was (392.70±14.03) I/L. According to the Gross equation, the blood loss of no-blood transfusion group and blood transfusion group in peroperative period were (937.29±63.04) ml and (706.43±35.02) ml, respectively. The hidden blood loss was dominant. At 1, 3 months after operation, Harris score of blood transfusion group was better than that of no-blood transfusion group, and 12 months after operation, there was no significant difference between two groups.
CONCLUSIONThe hidden blood loss of intramedullary nailing for intertrochanteric fracture should be emphasized in perioperative period, it can avoid the perioperative complications caused by anemia and affect the prognosis of patients.
