5.Modified posteromedial and posterolateral approaches for the treatment of posterior tibial plateau fractures
Chinese Journal of Orthopaedic Trauma 2012;14(11):945-949
Objective To observe the clinical outcomes of modified posteromedial and posterolateral approaches for the treatment of posterior tibial plateau fractures. Methods Between January 2006 and October 2011,25 patients with posterior tibial plateau fracture were identified. They were 17 men and 8 women, aged from 22 to 76 years (average,46.4 years).According to the AO/OTA classification, there were 4 cases of type 41-B1,6 cases of type 41-B2 and 15 cases of type 41-B3.According to Luo's three-column classification, they were all posterior column fractures.Ten patients with posterolateral tibial plateau fracture were treated by the modified posterolateral approach and the other 15 patients with posteromedial tibial plateau fracture by the modified posteromedial approach. The posterior fractures were fixated with a buttress plate. The knee function was evaluated at the last follow-up by the The Hospital for Special Surgery (HSS) Score. Results All the 25 cases were followed up for a mean time of 13.2 months (from 10 to 24 months).By the radiological Rasmussen criteria for fracture reduction, the mean score immediately after surgery was 16.5 points (from 13 to 18 points),with 20 excellent cases,4 good cases and one fair case. All patients were pain-free with full range of motion.Radiologically, bone union was achieved in all cases after a mean time of 13.9 weeks (from 12 to 18 weeks). No surgery-related complications were found. The mean HSS score was 91 points(range,74 to 97 points) and the good to excellent rate was 100% (17 excellent and 8 good cases) at the last follow-up.The average range of knee motion ranged from 0° to 125°.Conclusion In the treatment of posterior tibial plateau fractures, because the modified posteromedial and posterolateral approaches can result in excellent fracture reduction under direct visualization and allow for posterior buttress plating, soft tissues can be well preserved and good functional results can be expected.
6.Therapeutic effect of rehabilitation intervention on patients with diabetes mellitus complicated tubercu-losis
Chinese Journal of cardiovascular Rehabilitation Medicine 2015;24(4):359-362
Objective:To explore therapeutic effect of rehabilitation intervention on patients with diabetes mellitus (DM)complicated tuberculosis (TB).Methods:A total of 108 DM + TB patients treated in our hospital from Jan 2008 to Aug 2013 were enrolled.According to random number method, they were randomly and equally divided into rehabilitation intervention group and routine treatment group. The intervention course was eight weeks, and then rehabilitation effect was compared between two groups. Results:Compared with before treatment, after treatment, there were significant reductions in fasting blood glucose (FBG)and 2h postprandial blood glucose (2hPBG)in both groups,P <0.01 all;compared with routine treatment group, after treatment, there were significant reductions in levels of FBG [(6.77±1.82)mmol/L vs.(5.54±1.46)mmol/L]and 2hPBG [(7.94±1.83)mmol/L vs.(6.87 ±1.42)mmol/L],P <0.01 both, and significant rise in percentages of significant reduced or disappeared pleural effusion (51.85% vs.83.33%)and significant reduced or closed TB focus (44.44% vs.72.22%)in rehabilitation intervention group,P <0.01 both. Conclusion:Rehabilitation intervention can make patients get better control of blood glucose, promote absorption of pleural effusion and pulmonary tuberculosis focus.
7.Improving effect of testicular tissue graft on impaired learning and memory in endocrine recession mice
Chinese Journal of Organ Transplantation 2015;36(4):235-240
Objective To study the improving effect of testicular tissue graft on impaired learning and memory in endocrine recession mice, and to explore the possible mechanisms.Method Forty Kunming male mice were randomly divided into four groups by random number table:sham operation group, castration group, androgen (30 mg/kg) control group and testicular tissue graft group. The bilateral testes of mice were cut off to establish castrated mouse model. After modeling, the back subcutaneous muscle layers of castrated mice were implanted on the allogeneic testicular tissue blocks in graft group.Androgen group mice were intragastrically administrated with androgen for three months, and other two groups of mice were given an equal volume of normal saline At the end of treatment, the levels of serum testosterone in each group of mice were measured by ELISA.Learning and memory abilities of the mice were assessed by Morris water maze behavioral test.Morphological changes in the hippocampal CA1 region were observed through hematoxylin-eosin staining (HE).The expression levels of AR and apoptosis-related proteins (Bcl-2,Bax,caspase-3 and caspase-9) in the brain were detected by Westem blotting.Result As compared with castration group, the levels of serum testosterone in graft group were increased significantly (P<0.01),learning and memory abilities were greatly strengthened (P < 0.01),the morphological structures in hippocampal CA1 neurons were improved, and the expression levels of Bax,caspase-3 and caspase-9 were significantly reduced (P<0.01),while the expression levels of AR and Bcl-2 had obvious enhancements (P <0.01).Conelusion Testicular tissue graft can improve learning and memory ability probably by secreting endogenous testosterone which will bind with androgen receptor in the brain, then regulating the expression of neuronal apoptosis related genes, increasing Bcl-2 protein level and reducing the expression of Bax,caspase-3 and caspase-9 protein, and finally inhibiting neuronal apoptosis.
8.Pay attention to the research hotspots and evidence quality for the diagnosis and treatment of non-arteritic anterior ischemic optic neuropathy
Chinese Journal of Ocular Fundus Diseases 2015;31(6):521-523
Non-arteritic anterior ischemic optic neuropathy (NAION) is one of the most common acute optic neuropathy in adult characterized with impaired visual acuity and visual fields. The pathogenesis of NAION mostly result from the interactions between the systemic risk factors (such as diabetes mellitus, night hypotension, hereditary) and the local ocular risk factors (such as small optic disc and vitreo-papillary traction).A fully promoted diagnosis and treatment of NAION are based on the higher levels of clinical cvidence, as well as the comprehensive assessment of relationship between the systemic and ocular risk factors in the pathogenesis of NAION.Secondary optic neuropathy of NAION and the early diagnosis with effective treatment of the fellow eye would be highly emphasized.
