OBJECTIVE:To evaluate the effectiveness of TCM and antibiotics in the treatment of urinary tract infection(UTI) systematically.METHODS:According to the requirements of Cochrane systematic review,literatures were retrieved from CNKI and Wanfang database.A meta-analysis was performed on 18 literatures conforming to inclusion criteria.RESULTS:Compared with control group,the OR value of treatment group and subgroups including TCM group,TCM combined with antibiotics group,chronic UTI group and senile UTI group were 3.99,3.95,3.95,4.23,4.35,respectively.The 95%CI of those groups were(2.97,5.37),(2.80,5.80),(2.80,5.80),(2.61,6.83),(2.64,7.18).The OR value was of treatment group 0.25(95%CI:0.17,0.37;P

Objective To explore the diagnostic and treatment value of serum B-type natriuretic peptide (BNP) levels for the chronic heart failure and its correlation with cardiac function classification.Methods 155 patients with heart failure were selected as treatment group.According to cardiac function,grade Ⅱ in 48 cases,grade Ⅲ in 56 cases,grade Ⅳ in 51 cases.122 healthy people during the same period were selected as the control group.Retrospectively detected serum BNP levels of the subjects,and the BNP levels of the treatment group were detected before and after treatment,and its correlation with heart function was analyzed.Results The serum BNP level of the treatmentgroup was (738.67 ± 61.32) pg/mL,which was higher than (22.36 ± 21.89) pg/mL of the control group,the difference was statistically significant (t =19.38,P < 0.05).The serum BNP level of the treatment group after treatment was (329.58 ± 90.74) pg/mL,which was lower than (738.67 ± 161.32) pg/mL of the control group,the difference was statistically significant(t =19.38,P < 0.05).The serum BNP levels of grade Ⅱ,grade Ⅲ and grade Ⅳ patients after treatment compared with pre-treatment,the differences were statistically significant (t =13.65,15.18,17.39,all P < 0.05),from pre-treatment to pro-treatment,the serum BNP levels were gradually increased with increased grade,the differences were statistically significant (t =10.61,21.33,11.06,16.40,all P < 0.05).Conclusion The serun BNP level of chronic heart failure patients significantly increases,and it increases with the increase of cardiac function classification,and shows correlation.As the objective indicators of chronic heart failure,it has better clinical diagnosis value and treatment and prognostic significance.

5.Effect of intracoronary stenting for bifurcational lesions on the side branches

Objective To study the effect of intracoronary stenting for bifurcational lesions on the related side branches. Methods One hundren and twenty patients with 132 coronary bifurcational lesions were enrolled in the study. Intracornary stents were deployed in the lesions. The incidence of side branch oppression was analysed. Results Twenty-four side branches were oppressed during PCI (14.28%), especially in type Ⅰ and type Ⅳ bifurcation lesions. Conclusion The details of PCI and morphotogical characteristics of bifurcation lesions were the important factors leading the side branch oppression during PCI in bifurcation lesions.
6.Analysis on Measles Surveillance in Two Sorts of Areas of Jiangsu Province in 1999

Measles surveillance in Jiangsu Province started in 1999 by dividing the province into two sorts of areas. The first sort of area is economic well-developed area where EPI is carried out well and measles incidence is lower. The cities of this area investigated included Suzhou, Wuxi, Changzhou and Zhenjiang. The second sort of area included 9 cities (Nanjing, Xuzhou, Liangyungang, Nantong, Huaiyin, Suqian, Yangcheng, Yangzhou and Taizhou) where all the conditions are not as good as that mentioned in the first sort of area. The surveillance system of the province reported in 1999 a total of 938 suspected measles cases in 13 cities, of which 684 were confirmed as measles. The notifiable reporting system reported 846 suspected measles cases and 684 cases were confirmed. This indicated that the surveillance system operated well. The measles epidemiological features in two sorts of areas were compared. In first sort of area, including Nanjing, the measles cases of ≥15 years old accounted for 66.3% of the total cases. In second sort of area, not including Nanjing, the measles cases of <15 years old accounted for 90.8% of the total cases, and 73.2% of them were patients <10 years old. In 1999, there were 10 measles outbreaks occurred in the province and 218 measles cases were reported which accounted for 31.7% of the confirmed measles cases reported by the provincial measles surveillance system. 9% the 10 outbreaks occurred in the second sort of area, the cases were preschool and primary school children. The results showed that progression of EPI work was uneven in two sorts of areas of Jiangsu Province.
7.Effects of low-frequency ultrasound combined with Tongluo Zhitong herbs on urokinase-type plasminogen activator system and inflammatory cytokines in knee osteoarthritis

BACKGROUND: Low-frequency ultrasound combined with Tongluo Zhitong herbs can effectively relieve pain and improve joint function of patients with osteoarthritis, but the underlying mechanisms are still unclear.OBJECTIVE: To investigate the effects of low-frequency ultrasound combined with Tongluo Zhitong herbs on urokinase-type plasminogen activator (uPA) and inflammatory cytokines of osteoarthritis rabbits and the related mechanisms.METHODS: Forty rabbits were randomly divided into four groups: normal, model, control, and experimental groups. The normal group was treated with normal feeding, not for any processing. For the other three groups, papain was injected into the rabbit knee articular cavity to establish osteoarthritis models. One week later, the experimental group was treated with low frequency ultrasound combined with Tongluo Zhitong herbs for 3 weeks, the control group treated with the same volume of normal saline, and the model group received no intervention. The levels of inflammatory cytokines in the serum and synovial fluid were measured, the histological changes of synovial tissues and articular cartilage were evaluated using Mankin's score, and the expression levels of uPA and its receptor were determined by western blot assay.RESULTS AND CONCLUSION: The levels of inflammatory cytokines in the model group were significantly higher than those in the normal and experimental groups (P < 0.05). The Mankin's scores of synovial tissue in the experimental group were significantly lower than those in the model and control groups (P < 0.05), while Mankin's scores of the cartilage tissue had no significant difference between the experimental and model groups (P > 0.05). Compared with the model group, in the experimental group, the expression levels of uPA and its receptor were significantly downregulated,and the expression of uPA inhibitor was notably upregulated. These data show that low-frequency ultrasound combined with Tongluo Zhitong herbs attenuates inflammatory reaction in the synovial tissues, by inhibiting the formation of joint fluid and production of inflammatory cytokines, which is an effective treatment for knee osteoarthritis.

Objective To examine the changes in respiratory mechanics and efficacy of mechanical ventilation during perioperative period in patients undergoing valve replacement and determine the possible causes Methods Twenty ASA Ⅱ Ⅵ patients (8 male, 12 female), aged 24 65 years , undergoing valve replacement were studied The patients were premedicated with intramuscular phenobarbital 0.1g, pethidine 50mg and scopolamine 0 3mg Anesthesia was induced with midazolam, fentanyl and vecuronium and maintained with inhalation of low concentration of desflurane and intermittent intravenous boluses of fentanyl, midazolam and vecuronium Valve replacement was performed with moderate hypothermic cardiopulmonary bypass (CPB), The patients were mechanically ventilated with pure oxygen Tidal volume was maintained at 8 10ml/kg, frequency 12 15 bpm and I∶E ratio 1∶1 5 2 During CPB airway pressure was maintained below 5 cmH 2O After operation the patients were transferred to ICU and mechanically ventilated using Siemens 900C ECG, SpO 2, BP, CVP, left auricle pressure and urine output were routinely monitored Respiratory mechanics was measured with Novametrix 8100 multifunction respiratory monitor Mean airway pressure, peak airway pressure, airway resistance, chest lung compliance and respiratory work were measured 20 min after induction of anesthesia, 10 min before CPB, 10 min after termination of CPB, at the end of operation and 2, 6 and 18h after operation Results Work of breathing and airway resistance increased gradually after induction and reached the maximum at 6h after operation Dynamic compliance decreased significantly before CPB but increased slightly after CPB, then decreased again and to the minimum at 6h after operation Mean airway pressure and peak inspiratory pressure went up significantly after operation All parameters mentioned above returned to the preoperative levels before the patients were weaned from respiratory support in ICU Couclusions Thoracotomy and CPB are the main causes of changes in respiratory mechanics in patients undergoing valve replacement The effects reached the peak at 6 h after operation Therefore mechanical respiratory support is essential for the patients until respiratory function recovers and should be maintained for more than 6h after operation

10.Existence of heme oxygenase-carbon monoxide-cyclic guanosine monophosphate pathway in human trabecular meshwork cells in vitro.

To confirm the existence of heme oxygenase (HO)- carbon monoxide (CO)- cyclic guanosine monophosphate (cGMP) pathway in the cultured human trabecular meshwork cells (HT-MCs) in vitro, and to evaluate the inductive role of hemin on this pathway, HTMCs of the third to fourth generation were cultured in vitro. Reverse transcripase-polymerase chain reaction (RT-PCR) was employed for detection of HO-1 and HO-2 mRNA. Immunohistochemical staining was used to detect HO-1 and HO-2 proteins. Hemin was added into the culture solution. The HO-1 mRNA levels were quantified by RT-PCR. The relative amount of carbon monoxide released into the media was measured with the quantifying carbon monoxide hemoglobin (HbCO) by spectrophotometry. Radioimmunoassay was used to determine changes of cGMP in HTMCs. The results showed that cultured cells had the specific characteristics of HTMCs. Both HO-1 and HO-2 genes were expressed in HTMCs, as well as HO-1 and HO-2 proteins in HTMCs. Hemin induced HO-1 mRNA, HbCO and cGMP in a dose-dependent manner. In conclusion, HO-CO-cGMP pathway exists in the cultured HTMCs and can be induced by hemin. Pharmacological stimulation of HO-CO-cGMP pathway may constitute a novel therapeutic approach to rescuing glaucoma.
