1.Application of chain management in education among hypertensive patients of the community

Objective To investigate the effect of chain management on education among hypertensive patients of the community. Methods A total of 144 cases of hypertension patients in community were selected as the research objects, they were divided into the control group(72 cases, from February to July 2015) and the countryside group (72 cases, from September 2015 to February 2016).The control group was given routine education, and the countryside group received education with chain management. The treatment compliance and blood pressure values were compared between the two groups before and after intervention. Results The score of patients behavior of drug compliance, adverse drug, self life management and cigarettes-alcohol addiction management after intervention in the countryside group were (20.32±4.41), (34.62±5.08), (41.27±3.54), (10.02±1.79) points, while they were (13.41±3.07), (26.23 ± 5.12), (39.78 ± 2.47), (7.94 ± 1.83) points in the control group. And the scores of the countryside group were significantly higher than those of the control group (t=2.930-9.864, P<0.05). There were no significant differences in the systolic blood pressure, diastolic blood pressure and mean arterial pressure before the intervention between the two groups (P > 0.05). The systolic blood pressure, diastolic blood pressure and mean arterial pressure after intervention in the countryside group were (129.63 ± 8.07), (80.12 ± 7.62), (87.43 ± 9.35) mmHg(1 mmHg=0.133 kPa), while they were (135.42 ± 8.13), (87.56 ± 7.79), (99.31 ± 8.72) mmHg in the control group, and there were significant differences(t=4.295, 3.800, 9.201, P<0.01). Conclusions The chain management mode of community hypertension patients with education, can effectively improve the control of blood pressure in patients, improve the treatment compliance of patients.

3.Application of C-type fixation in the application of the catheter tunnel for peritoneal dialysis

Objective To explore the C-type fixation in the application of the catheter tunnel peritoneal dialysis. Methods One hundred and twenty patients undergoing peritoneal dialysis were selected from January to December 2014 in Xijin Hospital and were randomly divided into the control group and experiment group. The peritoneal dialysis catheters of the control group were fixed vertically at the tube orifice with surgical tape, while the peritoneal dialysis catheters of the experiment group were fixed horizontally first and then placed with a shape ofC and fixed at the tube orifice with surgical tape. The two groups were compared in terms of tunnel score, infection, tube shedding and pulling-induced stimuli. Result The tunnel score and the cases of infection rate, tube shedding and pulling-induced stimuli in the experiment group were lower and fewer than those of the control group with statistical significance (all P <0.05). Conclusion The type C fixation of tube for peritoneal dialysis can effectively reduce the incidence of infections in patients with peritoneal dialysis tunnel, reduce pulling-induced stimuli, and improve comfort of the patients.

5.Clinical analysis on breast conservative therapy for 304 cases of breast cancer

Objective To analyze the clinical results and risk factors according to cause-specific survival and local recurrence rates for the patients with Ⅰ-Ⅱstage breast cancer after breast conservative therapy(BCT).Methods During Jun.1988 to Jun.2005,304 patients with Ⅰ-Ⅱstage breast cancer in the General Hospital of PLA underwent lumpectomy,axillary lymph node dissection and definitive irradiation.All patients received adjuvant chemotherapy.The patients with positive estrogen hormone receptors received orally tamoxifen administration.The median follow up was 8.1 years.Results For all the patients,the total 5-year survival rate,cause-specific survival rate and local recurrence-free survival rate were 92.58%,93.46% and 86.26%,respectively.And the total 10-year overall survival rate,cause-specific survival rate and local recurrence-free survival rate were 85.29%,88.27% and 77.75%,respectively.The 5-year and 10-year local recurrence rates were 5.5% and 11.7%,respectively.The univariate and mutivariate analyses revealed that the age(4)were the risk factors according to cause-specific survival rates.The age(
6.Effects of Rehabilitation Surgery Assisted to FOLFOX Chemotherapy on Postoperative Recovery,Complica-tions and Quality of Life in Patients with Colorectal Cancer

OBJECTIVE:To investigate the effect of rapid rehabilitation surgery assisted to fluorouracil,oxaliplatin,leucovo-rin(FOLFOX)on postoperative recovery,complications and quality of life in patients with colorectal cancer. METHODS:60 pa-tients with colorectal cancer were randomly divided into control group(30 cases)and observation group(30 cases). After resection of colorectal cancer,control group received 85 mg/m2 Oxaliplatin for injection,2 h intravenous infusion,d1+200 mg/m2 Calcium le-vofolinate for injection,intravenous infusion,once a day,d1-2+400 mg/m2 Fluorouracil injection,intravenous infusion,once a day, d1-5 then 600 mg/m2 for 22 h intravenous infusion,d1-2. Observation was additionally given rapid rehabilitation surgery. 2-week was regarded as 1 course,it lasted 12 courses. Urinary retention time,anal exhaust time,first defecation time,postoperative hospital-ization time,scores of life quality (overall health,emotional function,mental health,physiological functions,physical function,so-matic pain,vitality,social function)in 2 groups were observed,and the incidence of postoperative complications was recorded. RE-SULTS:The urinary retention time,anal exhaust time,first defecation time,postoperative hospitalization time in observation group were significantly shorter than control group,the incidence of postoperative complications in observation group was signifi-cantly lower than control group,overall health,emotional function and mental health score were significantly higher than control group,the differences were statistically significant(P<0.05),but there were no significant differences in physiological functions, physical function,somatic pain,vitality,social function score in 2 groups (P>0.05). CONCLUSIONS:Rapid rehabilitation sur-gery assisted to FOLFOX chemotherapy can reduce the incidence of postoperative complications in patients with colorectal cancer, improve quality of life,shorten hospitalization time and promote postoperative recovery.

8.Study on clinical utility of continuous glucose monitoring system

Objective To determine the accuracy and reliability of continuous glucose monitoring system(CGMS)in intensive monitoring blood glucose level.Methods The analysis was conducted on 48 type 2 diabetes patients to compare the glucose levels of glucose meter and CGMS.Each patients received CGMS for 3 days during his/her hospitalization.The data were analyzed according to correlation coefficient analysis,mean absolute error,sensitivity and specificity.Results The mean duration of CGMS recording was(71.4?0.6)h.From the data that paired glucose readings and CGMS,the coefficients of correlation ranged from 0.79 to 0.90 and the mean absolute error ranged from 6.7% to 10.0%.The sensitivity and specificity to detect hyperglycemia were 93.15% and 95.95%,respectively.Conclusion There is a good correlation between sensor readings of interstitial fluid glucose level and capillary blood glucose readings.The results indicate that CGMS is of clinical utility as a holter-type glucose tracking device.
9.Protective Mechanism of Naoxing Nasal Spray on Ischemic Cerebral Injury

[Objective] To explore the protective mechanism of Naoxing Nasal Spray (NNS) on ischemic cerebral injury. [Methods] Seventy Wistar rats were randomized into 7 groups: high-dose NNS group (group A), moderate-dose NNS group (group B), low-dose NNS group (group C), nimodipine group (group D), model group (group E), solvent control group (group F) and pseudo-operation group (group G). Rat models of ischemia/reperfusion were established by blocking left middle cerebral artery. Plasma levels of tumor necrosis factor a (TNF-?) and interleukin 1? (IL-1?), malondialdehyde (MDA) content, activities of superoxide dismustase (SOD), glutathione peroxidase (GSH-Px) and nitric oxide synthase (NOS) in brain homogenate were detected. [Results] NNS decreased the contents of MDA, TNF-? and IL-1?, and increased the activities of SOD, GSH-Px and NOS. [Conclusion] NNS has protective effect on ischemic cerebral injury by promoting the clearance of free radicals, inhibiting the release of inflammatory mediator and increasing the synthesis of NO and thus to reduce injury of free radicals, inhibit the local inflammatory reaction and improve cerebral blood perfusion.
10.Role of atorvastatin in improving the inflammation-induced adipokine imbalance in mice with acute myocardial infarction

Objective:To investigate the effect of acute myocardial infarction (AMI)-activated inflammation on adipokine imbalance and the therapeutic effects of statin.Methods:A total of 32 C57BL/6 mice were divided into 4 groups:a sham group,an AMI group,a low-dose atorvastatin [2 mg/(kg.d)] group and a high-dose atorvastatin [20 mg/(kg.d)] group.AMI models were established by surgical coronary artery ligation.Plasma levels of high sensitive C reaction protein (hs-CRP),adiponectin and resistin were measured.Adiponectin and resistin expressions were determined.In addition,mouse 3T3-L1 preadipocytes in vitro were differentiated and they were stimulated by oxidized low density lipoprotein (ox-LDL).The protein expressions of adiponectin and resistin in adipocytes were detected.The effects of atorvastatin on ox-LDL-induced adipokine imbalance in adipocytes were identified.Results:The plasma levels of hs-CRP and resistin in AMI mice were significantly increased,whereas the plasma levels of adiponectin were remarkably decreased.However,atorvastatin treatment blocked the changes in the plasma levels of hs-CRP,resistin and adiponectin in AMI mice in a dose-dependent manner.Consistent findings regarding the adipose expressions of the two adipokines were obtained.The plasma levels of hs-CRP were positively correlated with resistin but negatively with adiponectin.In vitro study,ox-LDL increased resistin protein and adiponectin expressions in adipocytes,which were dose-dependently reversed by atorvastatin.Conclusion:Inflammation activation in AMI mice leads to adipokine imbalance.Atorvastatin ameliorates the AMI-induced adipokine imbalance via anti-inflammation.
