1.Surgical treatment of bone metastasis from thyroid cancer and analysis of prognostic factors

Object To investigate clinical features,surgical strategy and prognostic factors of bone metastasis from thyroid cancer.Methods A total of 46 patients with bone metastasis from thyroid cancer,who had undergone surgical treatment from January 1999 to July 2011 and had complete follow-up data,were enrolled in this retrospective study.There were 20 males and 26 females,aged from 40 to 87 years (average,56.87 years).Forty two (91.3%) cases of metastatic lesion located in axial skeleton,while 4 (8.7%) in limbs.The tumor histology type was well-differentiated in 35 patients and poorly differentiated in 11 cases.Nineteen patients (41.3%) had an isolated metastatic bone lesion,while other 27 patients (58.7%) had multiple lesions.Kaplan-Meier survival curve and Cox regression model were performed to identify prognostic factors among 19 factors including gender,age,tumor subtype,number of bone metastasis,site of bone metastasis,visceral metastasis,skeletal-related events,timing of metastasis,whether metastatic lesion was detected when thyroid cancer was diagnosed,whether thyroid surgery was performed,whether 131Ⅰ therapy was performed,whether chemotherapy or radiotherapy were performed,preoperative general condition (good or poor),surgical type (excisional surgery or palliative surgery),characteristics of metastatic lesion (osteolytic or mixed),intraoperative blood loss,whether measures were applied to prevent intraoperative bleeding,perioperative complications and long-term complications.Results Excisional surgery was performed in 36 patients (78.3%),while palliative surgery in other 10 patients (21.7%).The average amount of blood loss was 2603.26ml.The postoperative 5-year survival rate and 10-year survival rate were 37.3% and 12.4%,respectively.Univariate analysis showed the significant prognostic factors include tumor subtype,number of bone metastasis,whether 131Ⅰ therapy was performed,and preoperative general condition (good or poor).Multivariate analysis showed the independent prognostic factors were number of bone metastasis (solitary or multiple) and whether 131Ⅰ therapy was performed.Conclusion For patients with bone metastasis from thyroid cancer,the prognostic factors are useful for determining indications for operation and improving treatment outcome.

3.Foreskin-deglove and shaft-fix procedure combined with Z-plasty of penis and scrotum skin in treatment of concealed penis

Objective To explore the efficacy of foreskin-deglove and shaft-fix procedure combined with Z-plasty of penis and scrotum skin in the treatment of concealed penis. Methods Fifty-eight patients diagnosed as concealed penis were included in the study.All of them underwent the foreskin-deglove and shaft-fix procedure.Foreskin was degloved from 5 mm to 10 mm proximal to corona deep to the root,and bilateral tunica albuginea at this level was sutured to suprapubic scarpas' fascia.Z plasty of penis and scrotum skin was applied to cover penile shaft. Results After follow-up for 6 to 12 months,there was no recurrent retraction of the penile shaft,and all the patients were satisfied with the appearance of penis after plastic surgery.Edema of prepuce occurred in 2 patients,and completely regressed in 2 to 3 months.There was no incision scar hyperplasy,and the function of urination and erection were normal. Conclusion Foreskin-deglove and shaft-fix procedure combined with Z-plasty of penis and scrotum skin can resolve pathologic problems of concealed penis and has favourable aesthetic results with less complications.It is easy to perform and is an ideal procedure in the treatment of concealed penis.
4.Clinical effect of Yisui decoction plus western medicine in treating multiple system atrophy.

To observe the clinical effect of Yisui decoction plus western medicine in treating multiple system atrophy patients, totally 65 patients from China-Japan Friendship hospital during 2008-2012 with complete clinical data and received consecutive traditional Chinese medicine and western medicine treatment for more than 3 months were observed changes of traditional Chinese medicine symptom score, part 1 of unified multiple system atrophy rating scale, orthostatic hypotension before treatment and after 3 months treatment. After 3 months treatment, total effective rate of traditional Chinese medicine symptom was 70.8%. Compared with before treatment, score of part 1 of unified multiple system atrophy rating scale was obviously reduced after 3 month treatment (P < 0.001). Ex- cept swallow function without significant improvement, the remaining projects of unified multiple system atrophy rating scale were im- proved obviously (P < 0.05), of which the most obvious differences were orthostatic symptoms, falls and intestinal function (P < 0.001). Orthostatic hypotension after 1 month treatment and 3 month treatment was obviously better than before treatment (P < 0.001). There was no significant difference in orthostatic hypotension between 1 month treatment and 3 month treatment. The research results show that Yisui decoction plus western medicine has a certain effect on improving clinical symptoms of multiple system atrophy patients, especially has a significant effect on orthostatic hypotension, and can maintain a stable clinical effect in a certain period of time.

5.The synergistic effect of lidamycin and rituximab on human B cell lymphoma.

This study aimed to investigate the synergistic effect of lidamycin (LDM) and rituximab on human B cell lymphoma Ramos cells. Cell proliferation was measured using MTS assay, cell apoptosis was analyzed by Annexin V-FITC/PI assay, the expression of apoptosis related proteins was analyzed by Western blotting, and the in vivo lymphoma inhibition was verified using BALB/c mice inoculated via tail vein using Ramos cells which stably expressed pEGFP-N1 plasmid. The results showed that, after the pretreatment with rituximab for 48 h, rituximab and LDM showed significantly synergistic effects on cell proliferation. Cells in combined treatment group had a higher apoptosis rate than that in LDM treatment group. Compared with the LDM treatment group, the expression of apoptosis-related proteins such as Cleaved caspase-3, Cleaved caspase-7, Cleaved caspase-9 and Cleaved PARP in combined treatment groups increased, and expression of cIAP-2 and Bcl-2 decreased. The result of in vivo experiment showed that, in the combined treatment group, the survival time of BALB/c mice was significantly longer than the mice in control group and LDM treatment group, and the degree of tumor accumulation and metastasis to lymph nodes and spleen was lower.

6.A voxel-based morphometry study of gray matter abnormalities in patients with hypothyroidism

Objective To investigate the gray matter volume differences between adult hypothyroid subjects and healthy controls using voxel-based morphometry (VBM).Methods A total of 28 untreated hypothyroid patients and 28 age and gender-matched uormal controlswere recruited.The coucentrations of their freetriiodothyronine (FT3),free theroxine (FT4),and thyroid stimulating hormone (TSH)were testedfrom their venous blood on the day they accepted MR scanning.High resolution 3D-datawereobtained using 3.0 T MRI scanner.VBM analysis was performed to detect the volume differences between the groups by using voxel-wise two sample t test.Multiple regression analysis was employed to analyze the influence exerted upon gray matter volume by serum FT3,FT4,and TSH concentrationof hypothyroid patients.Results Compared to controls,gray matter volumes in the bilateral superior frontal gyri,left precentralgyrus and bilateral cerebellum were decreased in hypothyroid patients (voxels 211-2 142,t=3.36-3.69,P<0.01,corrected).No significant correlation between the gray matter volume in the above-mentioned brain regions and the clinical variables (serum FT3,FT4,and TSH concentration) (P>0.05) was found in the multiple regression analysis.Conclusions VBM analysis revealed that hypothyroid patients exhibited reduced gray matter volume in multiple brain regions.It suggests that the lack of thyroid hormones can lead to the structural abnormalities in adult brain and selective gray matter atrophy in multiple brain regions.
7.Progress in genetics of autoimmune hepatitis

Autoimmune hepatitis (AIH) is an autoimmune liver disorder characterized by chronic inflammation of liver. Although the etiology of AIH remains obscure, genetic and environmental factors may contribute to the development of AIH. In the past years, investigators have attempted to uncover the genetic architecture of AIH. Multiple genetic loci have been reported to be associated with AIH, in which human leukocyte antigen (HLA) alleles were strongly associated with disease onset and clinical manifestations for decades. A recent genome wide association study identified that two loci (SH2B3 and CARD10) increased susceptibility of AIH, in addition to the HLA loci. The genetic study is aiming to help to elucidate the genetic pathogenesis for AIH.
8.Clinical value of the serum long noncoding RNA BC200 in the diagnosis and treatment of gastric cancer

Objective To detect serum level of long noncoding RNA ( lncRNA) BC200 in gastric cancer(GC) patients, and investigate its relationship with clinical features , and evaluate its diagnostic value for GC.Methods A case-control study was performed.From November 2014 to July 2015, serum levels of lncRNA BC200 were detected by real-time quantitative polymerase chain reaction in 124 patients with GC , 41 patients with atrophic gastritis and 59 normal controls who were hospitalized in Qilu Hospital of Shandong University.Meanwhile , serum carcinoembryonic antigen ( CEA ) and carbohydrate antigen 72-4 ( CA72-4 ) were detected by electrochemical luminescence immunoassay .Serum levels of lncRNA BC200, before and 3, 7, 10, 30, 100 days after radical operation in another 31 patients with GC were determined.The sensitivity and specificity of serum lncRNA BC200, CEA and CA72-4 were analyzed by using of the receiver operating characteristic ( ROC) curve.The comparison between two groups was performed with Mann-Whitney U test and the comparison among many groups was conducted with Kruskal-Wallis H test.Results Serum levels of lncRNA BC200 in GC patients with stage Ⅰ and Ⅱ[1.041(0.794,1.462)] and stage Ⅲ and Ⅳ[1.290 (0.978,1.794)]were significantly higher than those in patients with precancerous lesion [0.969(0.699, 1.219)]and normal controls[0.801(0.556,1.599)](H =54.68,P<0.000 1).Compared with pre-operation[1.120 (0.859,1.663)], the serum BC200 levels decreased significantly in 10 days [0.903 (0.724,1.182)](U=55.0,P<0.000 1), 30 days[0.759(0.671,1.037)](U=299.0,P=0.026 1), and 100 days[0.478(0.378,0.635)](U=41.0,P<0.000 1) after surgery.The area under the receiver operating characteristics curve ( AUC) of serum lncRNA BC200 was 0.865 for GC diagnosis, which was significantly higher than that of serum CA 72-4 ( AUC =0.699 ) or CEA ( AUC =0.807 ) .The AUC of combined detection of three tests was 0.934.Conclusion Serum lncRNA BC200 levels are significantly increased in GC patients , which may be used as a potential biomarker in GC diagnosis and monitoring .

Objective To establish a population pharmacokinetics model of azithromycin in Chinese population.Methods Based on the dataset from a bioequivalence study held in the Chinese healthy volunteers,the nonlinear mixed effect model of azithromycin oral formulations was constructed by the NONMEM program.The pharmacokinetic parameters and the relationship between the inter-individual effects and fixed effects were estimated.Resuits A two compartment model with firstorder absorption and elimination was established.The body weight affects the CL_1 and CL_2,and age affects the V1 evidently.Conclusion A nonlinear mixed effect model for oral azithromycin formulations Was developed for the Chinese healthy population.
