3.MRI with multiple contrast weightings and dynamic contrast enhancement in evaluation of vulnerable atherosclerotic plaques

Objective To analyze the utility of MRI with multiple contrast and dynamic contrast weightings enhancement(DCE) in evaluation of vulnerable atherosclerotic plaques. Methods Forty male New Zealand white rabbits were fed with hypercholesterolemic diet,and right iliac arteries including the common and external iliac arteries were examined by multiple contrast and DCE MRI at intervals 6 to 20 weeks after balloon denudation.For multiple contrast weightings scanning,T1-,T1-/T2WI with fat suppression,proton density weighted and double invention recovery were used.Meanwhile,post DCE T1-with fat suppression images were obtained in 1,5,15 and 25 min after a bolus injection of Gd-DTPA contrast agent.Then a comparative analysis of plaque morphology and components to images was performed. Results There were 34(42.5%) vulnerable plaques and 46(57.5%) stable plaques amomg the 80 atherosclerotic lesions located at the right common or external iliac arteries.The accuracy,sensitivity and specificity of MRI with multiple contrast weightings and DCE for the detection of vulnerable plaques were 87.5%,94.1% and 82.6%,respectively,significantly higher than those only with multiple contrast weightings,which were 73.8%,82.3% and 67.4%,respectively(P

Objective To investigate the prevalence of integrons in A.baumannii isolates,analyze the correlation between inte- grons and resistance of A.baumannii,and study the resistance genes in integrons.Methods A total of 106 strains of A.bau- mannii were collected to test the antibiotic susceptibility by disk diffusion method.The classification of integrons was per- formed by analyzing the positive PCR products using restriction fragment length polymorphism (RFLP).The variable region of integrons was amplified by integron PCR.RFLP and DNA sequencing were used to analyze the resistance genes in integrons. Results About 52.8% (56/106) of the isolates showed integron positive.PCR-RFLP analysis revealed that they were all class I integrons.About 94.6%(53/56) of the positive strains with integrons owned the variable region,which was confirmed by integron PCR.The sizes of the amplicons ranged from 0.15 kb to 2.8 kb.All together 7 different cassette arrays were detec- ted,including genes coding resistance to aminoglycosides (aadA1,aadA2,aadA5,aadB,aacA4),sulphonamides (dfrⅫ, dfr17),?-lactam compounds (bla_(ara-10)),chloramphenicol (catB-like,catB8),and two open reading frames (orfF,orfI) with unknown function.A novel cassette array orfI-aadA1 was reported,and its GenBank accession number was DQ092497.Conclu- sions Class I integrons are widespread in A.baumannii isolates in Nanjing.The integrons are closely associated with the resist- ance and multidrug resistance in A.baumannii isolates.
5.CXCR4 expression of bone marrow CD34+cells in myelodysplastic syndromes and its correlation with cell migration

Objective:To evaluate the expression of CXCR4 and the migration rate of bone marrow stromal CD34+cells in differ-ent risk groups with myelodysplastic syndromes (MDS) using correlation analysis. Methods: Forty MDS patients were divided into low-and high-risk groups based on the International Prognosis Scoring System (IPSS). The former was composed of 20 patients with IPSS<1.5, whereas the latter was composed of 20 patients with IPSS≥1.5. Bone marrow (BM) samples of these patients and 10 nor-mal controls were collected. CD34+cells were separated and purified. The expression of CXCR4 was determined by flow cytometry. The migration rate of CD34+cells on the chemotactic effect of SDF-1αand on the effect of bone marrow stromal cells were measured. Results:The expression rate of CXCR4 was higher in the high-risk MDS group than in the low-risk and control groups (P<0.000 1). No significant differences existed between the low-risk and the control groups (P>0.05). The migration rate of CD34+cells on the ef-fects of SDF-1αand marrow stromal cells were significantly increased in the high-risk MDS group compared with those in the low-risk and control groups (P<0.000 1). Migration rate of CD34+cells on the effect of marrow stromal cells was positively correlated with CX-CR4 expression (P=0.000 1). Conclusion:The CXCR4 expression and migration rates of CD34+cells on the effect of marrow stromal cells are significantly higher in the high-risk MDS group than in the low-risk group. Migration rate has a positive correlation with the CXCR4 expression, which further indicates that MDS is a heterogeneous group of hematopoietic stem cell malignancies. The expres-sion and function of SDF-1 and its receptor CXCR4 differ within each group with various risks. SDF-1 and CXCR4 may be involved in MDS pathogenesis.
6.Non-invasive method for assessment of liver fibrosis in clinically diagnosed chronic hepatitis B viruscarriers

Objective To verify the value of routine laboratory markers for assessment of liver fibrosis status in chronic hepatitis B virus (HBV) carriers.Methods A total of 196 patients who were clinically diagnosed with chronic HBV carriers with liver biopsy and routine laboratory test were included in this study. The data of complete blood count, aspartate aminofransferase/alanine aminotransferase (AST/ALT),aspartate aminotransferase to platelet ratio index (APRI) and ageplatelet index (API) were collected and calculated.Patients were divided into group S0 (n=112) and group S1- S3 (n =84) based on liver fibrosis stages.Measurement data were analyzed by Wilcoxon rank sum test and enumeration data were analyzed by chi square test.Results All 196 enrolled HBV carriers were HBV DNA positive,with 156 (79.6%) HBeAg-positive.Age,ALT,AST,AST/ALT,APRI and API were all significantly higher in group S1- S3 than those in group S0 (statistic value=7.705,6.33,7.095,4.977,11.059,8.936,respectively; all P<0.05).However,PLT level was lower in the former group compared to that in the latter group (statistic value=10.196,P<0.05 ).The area under receiver operating characteristic curve (AUROC) of APRI and API were 0.827and 0.829,respectively.The highest sensitivity and negative prediction value (NPV) were 70.46 %and 71.43 % respectively when using API.The best specificity and positive prediction value (PPV)was 92.94% and 92.86%,respectively when using APRI.When APRI≥0.30 was used as the cut-off of liver fibrosis,97.62 % of 119 patients were diagnosed with liver fibrosis; when API≥4.0 was used as the cut-off of liver fibrosis, 96.43% of 112 patients were diagnosed with liver fibrosis.Conclusion APRI and API are two simple and feasible non-invasive biochemical markers that can be used to determine liver fibrosis status in chronic HBV carriers.
7.Worldwide trends in epidemiology and control of tuberculosis

Tuberculosis (TB) is a global burden disease and is being resurrected as a major worldwide public health problem after two decades of neglect.In 1993,the World Health Organization (WHO) declared that TB had been a global emergency because of the scale of the epidemic and the urgent need to improve global tuberculosis control.China is one of the countries with the largest population,and also the top of the 22 TB high-burden countries in the world.In the United States,the longstanding downward trend in TB incidence was interrupted in the mid-to-late 1980s,where the national TB incidence peaked in 1992.Sub-Saharan Africa is one of the three regions to dominate the worldwide distribution of notified TB cases.Of the 15 countries with the highest estimated tuberculosis incidence rates in the world,13 are in sub-Saharan Africa,where HIV is the most important single predictor of tuberculosis incidence.The largest share of the global burden of HIV-related tuberculosis falls on this region.The reasons for the persisting global tuberculosis burden include increased poverty in some regions,immigration from countries with high tuberculosis prevalence,the impact of HIV,and most importantly,the failure to maintain the necessary public health infrastructure under the mistaken belief that tuberculosis was a problem of the past.Relying on currently available methods of diagnosis and treatment,the DOT strategy promoted by the WHO for global tuberculosis control is effective,affordable,and adaptable in different settings.
8.Effect of PTP4A3 gene on tumor growth and metastasis

PTP4A3 is an oncogene,which encodes phosphatase of regenerating liver(PRL)-3 protein that is a metastasis-associated phosphorylase. At present,a number of studies have found that it promotes cell proliferation, cell motility,cell invasion,tumor metastasis and epithelial-mesenchymal transition. Therefore,it is inseparable with the occur-rence and development of cancer. Here,we summarize the relationship between PTP4A3 gene and the occurrence and de-velopment of cancer,the alteration of PTP4A3 gene expression and the functional role of PTP4A3 gene in a variety of canc-ers. This review will help us to understand the correlation between PTP4A3 gene and cancer as well as the mechanism of signaling pathway,providing new insights of PTP4A3 gene targeting strategy for treating cancer.
9.18F-FLT and 18F-FDG PET/CT Scan in Diagnosing Lung Cancer:A Meta-analysis

Purpose To assess the diagnostic value of 18F-FLT and 18F-FDG PET/CT scan in patients with lung cancer. Materials and Methods Computer-based retrieval was performed on Medline, PubMed, EMbase, Wanfang data, CNKI and the Cochrane Library to search reports on diagnostic value of lung cancer with 18F-FLT and 18F-FDG PET/CT scan. The quality of included studies was evaluated according to quality assessment of diagnostic accuracy studies (QUADAS), and MetaDisc software was adopted to conduct meta-analysis. The pooled specificity, sensitivity, and diagnostic odds ratio (DOR) were calculated. The heterogeneity was tested. The summary receiving operating characteristic (SROC) curve was drawn, and the areas under the curve (AUC) as well as Q* were measured. Results Ten studies were included. Meta-analysis showed pooled sensitivity was 0.88, pooled specificity was 0.56, DOR was 9.10, AUC was 0.8102, Q*was 0.7448 for FDG group;pooled sensitivity was 0.79, pooled specificity was 0.78, DOR was 12.50, AUC was 0.8440, Q*was 0.7756 for FLT group. Conclusion Both 18F-FDG and 18F-FLT have well diagnostic value for lung cancer, but the specificity of 18F-FLT is higher than that of 18F-FDG in the diagnosis of lung cancer.
10.Forensic Analysis for 54 Cases of Suxamethonium Chloride Poisoning

Objective To observe and analyze the performance of forensic science in the cases of suxamethonium chloride poisoning,and to improve the identification of suxamethonium chloride poisoning.Methods Fifty-four cases of suxamethonium chloride poisoning were collected.The rules of determination of suxamethonium chloride poisoning were observed by the retrospective analysis of pathological and toxicological changes as well as case features.Results The pathological features of suxamethonium chloride poisoning were similar to the general changes of sudden death,which mainly included acute pulmonary congestion and edema,and partly showed myocardial disarray and fracture.Suxamethonium chloride could be detected in the heart blood of all cases and in skin tissue of part cases.Conclusion Suxamethonium chloride poisoning has the characteristics with fast death and covert means,which are difficult to rescue and easily miss inspection.For the cases of sudden death or suspicious death,determination of suxamethonium chloride should be taken as a routine detection index to prevent missing inspection.
