1.The clinical effect of hidden hemorrhage after hip and knee arthroplasty

Objective To study the correlated mechanisms and clinical effect of hidden hemorrhage after total hip arthroplasty (THA) and total knee arthroplasty(TKA). Methods From March 2001 to May 2005, 61 patients were treated with THA and 73 patients were treated with TKA. The patients treated with THA involved 22 males and 39 females with an average age of 68 years(range 61-79 years). The patients treated with TKA involved 23 males and 50 females with an average age of 73 years(range 65-77 years). All arthroplasties were primary and unilateral, and the 24 h fluid resuscitation was not more than 2000 ml. Using Gross formula, the true total blood loss was calculated depending on height, weight and pre- and post-operation Hct, and the hidden hemorrhage was got by subtracting the visible blood loss from total loss. Results Following THA, the mean total loss was 1520 ml and the hidden hemorrhage 482 ml(32%). Following TKA, the mean total loss was 1508 ml and the hidden hemorrhage was 776 ml(52%). 2.Expression and clinical significance of urokinase type plasminogen activator in giant cell tumor of bone

Objective To investigate the relationship of uPA and uPA R protein expression in bone giant cell tumors(GCT)with their pathological grades and relapse. Methods By using Avidin biotin complex(ABC) immunohistochemistry method, monoclonal antibodies against uPA and uPA R proteins were used to measure their expression in 23 cases of GCT (including 8 cases of pathological grade Ⅰ, 10 cases of pathological grade Ⅱ and 5 cases of pathological grade Ⅲ accord to Jaffe classification). Results According to the level of these immunoreaction, over expression of uPA and uPA R were detected in 10 and 8 patients (positive rate 43.5 % and 34.8 %). In cases with relapse or not, the positive expression rate of uPA and uPA R was 66.7 %, 28.6 %, 66.7 %, 14.3 % respectively, abnormally high expression of both uPA and uPA R proteins was found in 6 cases(66.7 %). 3.Effect of Zibai Gelatin for Cervical Cancer SiHa Cells

Objective To investigate the immunological reaction mechanism of medicated serum in Zibai Gelatin for SiHa cells of the cervical cancer infected by high risk human papilloma virus (HPV). Methods Through immunohistochemical comparison and the cell culture, and after medicated serum was administrated to SiHa cells of cervical cancer for 24 h, 48 h, and 72 h, optical densities of IL-6, IL-10, CD83 and TNF-αin the same time but different concentrations and different times but the same concentration were observed. Relationships of dose-effect and time-effect between expressions of IL-6, IL-10, CD83 and TNF-α and medicine action were analyzed by calculating average optical density. Results With the increase of medicine concentration and administration time of Zibai Gelatin, the expressions of IL-6 and TNF-αgradually decreased, while the expressions of CD83 and IL-10 gradually increased (P<0.05). Conclusion Zibai Gelatin with medicated serum can inhibit local inflammatory reaction by improving local immunologic function of cervix, which is beneficial to reduce cervix high-risk HPV.
4.Cytotoxic alkaloids from stems of Nelumbo nucifera.

Chemical investigation was carried out to study the alkaloids from stems of Nelumbo nucifera and their cytotoxic activities. The constituents were separated by column chromatography, and their structures were elucidated by spectroscopic data analyses. The isolated compounds were evaluated for their cytotoxic activities by MTr method. Fifteen compounds were isolated from the total alkaloids extract and identified as asimilobine (1), isococlaurine (2), N-acetylnorarmepavine (3), crykonisine (4), velucryptine (5), pycnarrhine (6), liriodenine (7), nuciferine (8), nornuciferine (9), armepavine (10), N-methylasimilobine (11), coclaurine (12), N-norarmepavine (13), N-methylcoclaurine (14) and lysicamine (15). Compounds 1-7 and 12-15 were isolated from stems of this plant for the first time, and compounds 2-6 were firstly isolated from the family Nelumbonaceae. Compounds 7-10, 13 and 14 showed significant cytotoxic activities against HL-60 carcinoma cell line with inhibitory ratios of 51.36%, 59.09%, 52.51%, 53.93%, 51.43%, and 64.31% at concentration of 1 x 10(-5) mol x L(-1), respectively.

5.Fast 3D Medical Image Segmentation Based on CUDA

Objective: 3D segmentation is an important part of medical image analysis and visualization. It also continues to be large challenge in the medical image segmentation. While level sets have demonstrated a great potential for 3D medical image segmentation, these algorithms have a large computational burden thus are not suitable for real time processing requirement. To solve this problem, we propose a parallel accerelated method based on CUDA. Methods: We implement C-V level set algorithm in the CUDA environment which is the NVIDIA's GPGPU model.The segmentation speed can greatly improved by using independence of image pixel and concurrence of partial differential equation .The paper shows the flow chart of the parallel computing and gives the detailed introduction of the C-V level set algorithm which is implemented in the CUDA environment. Results: Realizing the C-V level set parallel accerelated algorithm. This method has faster segmentation speed while preserving the qualitative results, Conclusions: This method is viable and makes the fast 3D medical image segmentation come hue.
6.Efficacy and strategies of routine cleaning and disinfection for neonatal in-cubators in use

Objective To evaluate efficacy of routine cleaning and disinfection methods for incubators,and put forward a feasible improvement solution.Methods 30 incubators used in a neonatal intensive care unit of a hospital between Decem-ber 2013 and June 2014 were chosen and randomly divided into baseline,control,and trial groups(10 incubators in each group).Baseline group and control group were disinfected by routing disinfection method (wiping internal and external sur-faces of incubators with water and chlorine-containing disinfectant),trial group adopted intensified disinfection method (wi-ping internal surfaces of incubators with alcohol)on the basis of routine disinfection,disinfectant efficacy of three groups were compared.Results In baseline group,unqualified incubators were initially detected on the fourth day of monitoring, all incubators were contaminated in varying degrees on the seventh day of monitoring,the detection rate of unqualified spec-imens was 31.43% (88/280).The median time for the initial detection of unqualified incubators in control group and trial group were on the fifth day and seventh day respectively,there was significant difference between two groups(χ2 =12.38, P <0.05);The unqualified rate of trial group was significantly lower than control group (15.36%[43/280]vs 32.86%[92/280],χ2 =23.43,P <0.05 ).Conclusion Intensified disinfection with alcohol on the basis of routine disinfection method can effectively improve the disinfectant efficacy of the surface of incubators,it is convenient,inexpensive and safe, and worth to be popularized in primary hospitals.

Objective: Real time medical image registration technique is one of the key techniques in image based surgery navi-gation system. While in medical image analysis, image registration is usually a very time-cousuming operation, and this is not conducive to clinical real-time requirements. This paper studies and realizes the acceleration of the process of image registra-tion. Methods: In order to improve the regisWation rate, in this paper, we propose a new technology which is based on CUDA (Compute Unified Device Architecture) programming model to accelerate the process of registration in hardware, using paral-lel methods to achieve pixel coordinate transformation, linear interpolation, and calculate the corresponding pixel gray value residuals. Results: The registration is up to the sub-pixel level and the GPU-based registration is dozens or even hundreds of times faster than CPU-based registration. Conclusions: This method greatly enhances the speed of rigid registration without changing the alignment accuracy.
8.An analysis of hospitalization rates of patients with goiter in Lianyungang City Jiangsu Province in 2002-2010

Objective To observe the changes of hospitalization rates of patients with goiter in Lianyungang City in a coastal area with mild iodine deficiency in 2002-2010.Methods Medical reports of patients with goiter from county hospitals were studied at county level with a retrospective method.In accordance with the National Iodized Salt Monitoring Program,nine townships(towns) were taken from each county (district) [total of five counties(districts)]; four villages were selected from each township (town); salt samples were taken from eight households in each village,and salt iodine was determined using direct titration of sodium thiosulfate.Results The hospitalization rate of patient with goiter increased from 1.87/10 million in 2002 to 7.05/10 million in 2009,and then fluctuated to 6.06/10 million in 2010(x2 =281.91,P< 0.01).Among them,female's hospitalization rates were significantly higher than that of male 's(The variation range of x2 values were 35.23-116.04,all P < 0.01),and the hospitalization rates of people over the age of 40 years were significantly higher than that of people less than 40 years of age(The variation range of x2 values were 33.04-263.04,all P < 0.01),and the hospitalization rates of people in urban areas were significantly higher than that of people in rural areas (The variation range of x2 values were 18.35-140.00,all P < 0.01).Average salt iodine was 27.61-30.13 mg/kg in 2002-2010.The coverage rate of iodized salt and the consumption rate of qualified iodized salt both increased year by year (x2 =183.75,211.99,P < 0.01).There was a positive correlation between hospitalization rate of patient with goiter and coverage rate of iodized salt(spearman correlation coefficient was 0.83,P < 0.05).There was a positive correlation between hospitalization rate of patient with goiter and the consumption rate of qualified iodized salt too (spearman correlation coefficient was 0.93,P < 0.05).Conclusions In Lianyungang City,the hospitalization rate of patient with goiter,the coverage rate of iodized salt and the consumption rate of qualified iodized salt all show upward trend in 2002-2010.The relationship among them is worth further study.

Human herpesvirus 7 (HHV-7) infection is dependent on the functions of structural glycoproteins at multiple stages of the viral life cycle. These proteins mediate the initial attachment and fusion events that occur between the viral envelope and a host cell membrane, as well as cell to cell spread of the virus. To characterize the HHV-7 glycoproteins that can mediate cell fusion, a cell-based fusion assay was used. 293T cells expressing the HHV-7 glycoproteins of interest along with a luciferase reporter gene under the control of the T7 promoter were cocultivated with SupT1 cells transfected with T7 RNA polymerase. HHV-7 glycoproteins gB, gH, gL and gO can mediate the fusion of 293T cells with SupT1 cells, and the fusion can be inhibited by anti-CD4 mAbs. Thus, the coexpression of HHV-7 gB, gO, gH and gL is sufficient and necessary for HHV-7 induced membrane fusion, and one of these glycoproteins or protein complex formed by these glycoproteins might be the ligand(s) of CD4 molecule.
10.Multi-slice spiral CT multiplanar reconstruction findings of localized fat collection adjaction to the subdiaphragmatic inferior vena cava

ObjectiveTo discusses the MSCT multiplanar reconstruction manifestation (MPR) of localized fat collection adjaction to subdiaphragmatic inferior vena cava (IVCfat).MethodsThe thoracic and abdominal MSCT scan data of 8246 patients were browsed,45 patients with presumed IVCfat on axial CT scans were further studied prospectively with MSCT MPR.The predisposing position of IVCfat and its relationship with IVC were observed.It was divided into two kinds of intraluminal type and extraluminal type according to the angle of IVCfat with respect of the wall of IVC.The other 50 patients without IVCfat were randomly selected as the control group.The sagittal inclination angle (SIA) and diameter ratio (DR) between supra- and sub-diaphragmatic IVC between the two groups were compared by using t test.Results The detection rate was 0.55% (45/8246).Of which hepatic vein lacuna 8 patients,subdiaphragmatic gap medial to IVC 28 patients and IVC groove 9 patients.The shape of IVCfat showed mainly for the round,oval and crescents on axial CT scans,of 15 patients intraluminal type,4 showed target signs .The shape of IVCfat showed mainly for half-moon at MPR.The SIA and DR at IVCfat group were 21.62° ± 8.42°and 2.01 ±0.84 respectively,at control group were 16.75° ±7.82°(t =1.594,P >0.05) and 1.31 ±0.28(t =2.341,P < 0.05 ) respectively.ConclusionThe round,oval or half of limited fat density shadow adjaction to subdiaphragmatic inferior vena cava which similar to in the lumen is the characteristic performance of IVCfat,it may be an anatomical variation.
