1.Treatment of big aera burn accompanying inhalation injury

Objective To review our clinical experiences in the treatment of big area burn accompanying in-halation injury to explore the optimal preventive measures and therapy. Methods To analyze the clinical cases con-ducted of 20 such admitted the big area burn aeeornpanying inhalation injury from 2004 to 2007. Results 18 patients (90%) were cured and 2 patients(2%) died. Conclusion The curative rate of big area burn accompanied by inha-lation injury can be enhanced by combining various treatments,including early treat,early presentive tracheotomy,ap-propriate tracheobronchial lavage,and clear traeheoobronchial secretion.
2.Prevention of prosthesis-patient mismatch during aortic valve replacement

Objective The prosthesis used for aortic valve replacement (AVR) can be too small in relation to body size,thus causing valve prosthesis-patient mismatch (PPM).The aim of this article was to summarize the preventive strategy of PPM during AVR.Methods A total of 357 patients [203 males,154 females; mean age (54.9 ± 18.7 ) years ] underwent AVR between February.2010 and December 2011.The weight and body surface area (BSA) of the group is( 60.1 ± 11.4 )kg and (1.67 ± 0.21 )m2 respectively.The aortic valve prosthesis effective orifice area (EOA) was divided by body surface area (BSA) to obtain the EOA index (EOAI).PPM was then defined as none or mild if EOAI was > 0.85 cm2/m2,as moderate for (0.65 - 0.85 ) cm2/m2 and as severe for < 0.65 cm2/m2.To avoid PPM,a simple three-step algorithm was applied:Step 1,Calculate the patient's BSA from weight and height;Step 2,Calculate the minimal valve EOA required based on the BSA to ensure an EOAI >0.85 cm2/m2 ; Step 3,Select the type and size of prosthesis that has reference values for EOA greater or equal to the minimal EOA value obtained in step 2.For patients with a small aortic root,the following three methods was used:( 1 ) Replace aortic valve with simple interrupted suture technique ; (2) Apply new type and high-performance prosthetic valves such as St.Jude Medical Regent mechanical valve ; (3) Enlarge the narrowed aortic root when necessary.Results Of all 357 patients,272 patients received mechanical AVR and 85 bioprosthetic AVR.Among the 49 patients who received AVR with simple interrupted suture technique.St.Jude Medical Regent mechanical valve was implanted in 38 patients and the aortic root enlargement was performed in 11 patients.The total prevalence of PPM was 6.4% and there was no severe PPM.The prevalence of PPM with mechanical AVR and bioprosthetic AVR was 1.8% and 21.2% respectively.There were 4 deaths during early period of operation,and the operative mortality was 1.1%.Conclusion Prosthesis-patient mismatch can be effectively prevented at the time of AVR with appropriate measurement.
3.Dynamic predictive modeling of extraction process for red ginseng using near-infrared spectroscopy.

It is the objective of this study to develop dynamic predictive model for the extraction process of red Ginseng using NIR spectroscopy. NIR spectroscopy was collected online and PLSR models were developed for total quantity of ginsenosides. The performance of NIR prediction model achieved R, RMSEC, RMSEP of 0.996 09, 0.018 9, 0.016 8, respectively. A first order dynamic mass transfer model was combined with NIR prediction of the quality indicator to predict the trajectory of the extraction process based upon the initial 3 or 4 data points. The results showed good agreement with actual measurements indicating reasonable accuracy of the predictive model. It could potentially be used for advanced predictive control of the extraction process.

4.Effect of Initial Substrate Concentration on Kinetics of Hydrogen Production by Photosynthetic Bacteria in Batch Culture

The effect of initial substrate concentration on the growth,metabolic activities of Photosynthetic bacteria (PSB) in the process of hydrogen production is studied.The empirical relation of the initial substrate concentration to the specific growth rate,specific substrate consumption rate and specific hydrogen production rate of PSB are developed based on the modified Monod model.It is found that the results given by the model are well agreed with the experimental data in growth phase and stationary phase of PSB.Meanwhile,the optimal substrate concentration in the process of growth,metabolism and hydrogen production is 50 mmol/L.Furthermore,when the initial substrate concentration deviates 50 mmol/L,the activation of PSB is limited or inhibited,and the inhibiting effect is more prominent than the limiting effect.In addition,it is demonstrated that initial substrate concentration has little effect on the specific substrate consumption rate.
5.The application and early result of rectangle titanium cage in the anterior cervical spine fusion.

OBJECTIVETo evaluate the value of rectangle titanium cage (SynCage-C) in the anterior cervical spine fusion.
METHODSNineteen patients underwent anterior cervical discectomy and interbody fusion using SynCage-C with average follow-up of 9 months.
RESULTSAll cases had cervical spondylosis with 13 cases of myelopathy and 6 cases of radiculopathy. One level fusion were 16 cases and two levels were 3 cases. The mean operation time was 40 minutes per level with average bleeding 60 ml. Patients were asked to wear a hard collar for average 8.5 weeks after the operation. The fusion was solid on lateral view X-ray films three months post-operation. Only 1 case had neck stiffness due to prolonged collar protection and another case had short term pain on bone dorner site. There were no other complications such as cage subsidence, displacement, milaligment and kyphosis of cervical spine.
CONCLUSIONThe SynCage-C can resort and maintain the intervertebral space effectively with the advantage of immediate stability, satisfactory fusion and easy to use.

6.Comparison of adjacent segment degeneration five years after single level cervical fusion and cervical arthroplasty: a retrospective controlled study.

BACKGROUNDCervical arthroplasty is indicated to preserve cervical motion and prevent accelerated adjacent segment degeneration. Whether accelerated adjacent segment degeneration is prevented in the long term is unclear. This trial compared adjacent segment degeneration in Bryan disc arthroplasty with that in anterior cervical decompression and fusion five years after the surgery.
METHODSWe studied patients with single level degenerative cervical disc disease. The extent of adjacent segment degeneration was estimated from lateral X-rays.
RESULTSTwenty-six patients underwent single level Bryan disc arthroplasty and twenty-four patients underwent single level anterior cervical decompression and fusion. All patients were followed up for an average of sixty months. In the Bryan arthroplasty group, nine (17.6%) segments developed adjacent segment degeneration, which was significantly lower than that (60.4%) in the anterior cervical decompression and fusion group. Eleven segments in the Bryan arthroplasty group developed heterotopic ossification according to McAfee's classification and two segments had range of motion less than 2°. In the heterotopic ossification group, four (19.5%) segments developed adjacent segment degeneration, similar to the number in the non-heterotopic ossification group (16.7%). Adjacent segment degeneration rate was 50% in grade IV group but 11.8% in grade II to III.
CONCLUSIONSAdjacent segment degeneration was accelerated after anterior cervical decompression and fusion. However, Bryan disc arthroplasty avoided accelerated adjacent segment degeneration by preserving motion. Patients with grade IV heterotopic ossification lost motion, and the rate of adjacent segment degeneration was higher than that in patients without heterotopic ossification.

7.Clinical Study on Mind-regulating Acupuncture in Treating Primary Trigeminal Neuralgia

Objective To observe the clinical efficacy of mind-regulating acupuncture in treating primary trigeminal neuralgia.Method Sixty-one patients with primary trigeminal neuralgia were randomized into a treatment group of 31 cases and a control group of 30 cases by using random number table method. The control group was intervened by oral administration of Carbamazepine, while the treatment group was additionally given mind-regulating acupuncture. The pain intensity, pain flare-up frequency and quality of life in the two groups were evaluated before and after the treatment, and the clinical efficacies were compared.Result The total effective rate was 90.3% in the treatment group, versus 70.0% in the control group, and the between-group difference was statistically significant (P<0.05). The pain score, pain flare-up frequency and quality of life score after the treatment were significantly different from those before the treatment in both groups (P<0.05); there were no significant between-group differences in comparing the pain score and flare-up frequency after the treatment (P>0.05); there was significant between-group difference in comparing the quality of life score after the treatment(P<0.05). The pain score, pain flare-up frequency and quality of life score at the 6-month follow-up were significantly different from those before and after the treatment in both groups (P<0.05); there were significant between-group differences in comparing the pain score, flare-up frequency and the quality of life score at the 6-month follow-up (P<0.05). Conclusion Mind-regulating acupuncture can produce a significant efficacy in treating primary trigeminal neuralgia and obviously enhance the quality of life.
8.Preservation of laryngeal function in surgical treatment of pyriform sinus carcinoma.

OBJECTIVETo explore the feasibility and the related surgical techniques of laryngeal function preservation in surgical treatment of pyriform sinus cancer.
METHODSTwo hundred and thirty cases (stage I, 6; stage II, 10; stage III, 91; stage IV, 123) with pyriform sinus cancer were treated surgically from 1978 to 1996 in the Department of Otorhinolaryngology of Qilu Hospital of Shandong University. The methods of removing tumor and repairing surgical defects were depended on the extension of lesions. The laryngeal and pharyngeal functions were rebuilt by normal tissue preserved with lesions entirely removed. One hundred and fifty-eight cases were surgically treated with laryngeal functions preserved and 72 cases total laryngectomy. The most of the cases received postoperative radiotherapy.
RESULTSThe overall 3 and 5 year survival rates were 67.4% (155/230) and 48.3% (111/230) respectively. For stage I, the survival rate was 5/6; stage 11, 70.0% (7/10); stage III, 57.1% (52/91) and stage IV, 38.2% (47/123); the 3 and 5 year survival rates in functionally preserved group were 67.7% (107/158) and 50.0% (79/158), while in none functional group were 66.7% (48/72) and 43.1% (31/72), respectively. 75.3% (119/158) patients have laryngeal functions (voice,respiration and deglutition) completely restored and 24.7% (39/ 158) partially restored(voice and deglutition).
CONCLUSIONSThe preservative surgery is feasible for the selected pyriform sinus cancer cases. Choosing and following optimum surgical methods is a prerequisite for improving the quality of life of the cases.

9.Activation of nuclear factor kappa B in newborn rats sepsis.

OBJECTIVEThe aim of the study is to explore the effect of NF-kappa B signal pathway in neonatal sepsis so as to provide the experimental base for corresponding clinical treatment of the sepsis, in which NF-kappa B is taken as the target.
METHODSThe sepsis model was established in newborn rats by giving Staphylococcus aureus subcutaneously: (1) The electrophoretic mobility shift assay (EMSA) was used to observe the activity of NF-kappa B in the lungs and the livers in newborn rats with Staphylococcus aureus sepsis. (2) Immunohistochemical method was used to observe the activity of NF-kappa B P56 in newborn rats with Staphylococcus aureus sepsis. (3) The anti-oxidant pyrrolidine dithiocarbamate (PDTC) was used to observe its effect on NF-kappa B activities of liver and lungs and on the activity of splenic NF-kappa B P56 in newborn rats with Staphylococcus aureus sepsis.
RESULTSIn newborn rats with Staphylococcus aureus sepsis, the NF-kappa B activity in lungs was enhanced at the 1st hour and reached to the peak level at the 3rd hour; then, it was weakened gradually and at the 24th hour faded away. The activity of the liver NF-kappa B was also activated and peaked at the 4th hour; then, it was gradually weakened and at the 24th hour faded away. The positive expression of splenic NF-kappa B P56 began to be intensified at the 1st hour (12.0 +/- 3.7), peaked at the 3rd hour (51.4 +/- 5.9) and showed insignificant differences at the 24th hour (3.4 +/- 1.4) as compared with the sepsis group. PDTC had an inhibitive effect on the activities of liver NF-kappa B and lung NF-kappa B and on the positive expression of splenic NF-kappa B P56 used in the dosage of 50-200 mg/kg. The larger the dosage was used, the more intensified inhibitive effect could be obtained. In the dosage of 200 mg/kg, the inhibitive effect was the most intensified.
CONCLUSIONS(1) In newborn rats with Staphylococcus aureus sepsis, the NF-kappa B of lungs, liver and spleen were activated, and all indicate a peak. (2) The anti-oxidant PDTC can inhibit NF-kappa B activity in a dose-effect fashion in newborn rats with Staphylococcus aureus sepsis.

10.Transnasal endoscopic repair of cerebrospinal fluid rhinorrhea

Objective To present the surgical technique and clinical effect of transnasal endoscopic repair of cerebrospinal fluid (CSF) rhinorrhea. Methods From 1996 to 2010,54 patients with CSF rhinorrhea were treated with intranasal endoscopic surgery,including 25 patients with traumatic CSF rhinorrhea,17 patients with spontaneous CSF rhinorrhea,and 12 patients with iatrogenic CSF rhinorrhea.The temporalis muscle,temporalis fascial,middle turbinate mucosa,nasal septum mucosa,inferior turbinate mucosa,fascia lata,leg muscle,abdominal fat,uncinate process mucosa and sinus mucosa were used to repair the fistulae.Results Forty-nine patients were successfully treated after the first operation,1 after the second attempt,1 after the third attempt,and 1 was successfully treated at the second operation in other hospital,1 stopped therapy after an unsuccessful repairing.One patient recurred within one and a half years after operation and stopped therapy.Seven patients developed complications after the operation ( high fever in 4,high fever and transient mild coma in 1,epilepsy in 1,pneumcephalus in 1 ) and were cured afterwards.Conclusions Transnasal endoscopic surgery is safe,effective and microinvasive treatment for patients with CSF rhinorrhea,it is the first choice for repairing of CSF rhinorrhea for its high successful rate.Accurate leakage site identification,selection of suitable approach and repairing method are critical to the success of operation.
