OBJECTIVETo investigate the clinical efficacy of Shexiang Injection (SI) on inflammatory reaction in patients with acute cerebral infarction (ACI).
METHODSForty-two patients with ACI were randomly divided into two groups, 21 in each group. The control group treated with conventional therapy and the SI group treated with conventional therapy plus SI. Besides, 21 healthy people were arranged in the normal group for control. Expression of CD54 of mononuclear cell (MC-CD54) and serum level of soluble vascular cell adhesion molecule-1 (sVCAM-1) were determined and the clinical efficacy was observed dynamically before treatment and on the 7th, 14th day of the course.
RESULTSLevels of MC-CD54 expression and sVCAM-1 in the ACI patients increased obviously (P < 0.01), reached the peak on the 7th day, and declined obviously on the 14th day in both groups, however, the lowering in the SI group was more significant than that in the control group (P < 0.01); and positive correlation was shown between these two indexes (P < 0.01). After treatment, score of neural defect was improved more significantly (P < 0.01), and the markedly effective and curative rate was higher in the SI group than those in the control group (P < 0.05), respectively.
CONCLUSIONSI could inhibit the subsequent inflammatory reaction, and thus improve the clinical efficacy of conventional therapy in treating patients with ACI.

Methylacetoacetyl-CoA thiolase deficiency (T2 deficiency) is a rare congenital and metabolic disease
affecting the ketone body and isoleucine metabolism. The typical symptoms are refractory metabolic
acidosis, in which large amounts of 2-methyl-3-hydroxybutyry1 carnitine, 2-methyl-3-hydroxybutyrate
and tiglylglycine are often detected in the blood and urine. We herein describe an atypical case of T2
deficiency with a high level of 3-hydroxybutyrate and a low level of 2-methyl-3-hydroxybutyrate in
the urine. Such a case was diagnosed by urinary organic analysis in combination with gene mutation
evaluation. Organic acids in the urine were measured using a gas chromatography mass spectrometer
and all exons were sequenced via deep sequencing. Molecular biology analysis confirmed the presence
of a homozygous mutation in the acetyl-CoA acetyltransferase 1 (ACAT1) gene. The patient received a
special diet of deeply hydrolyzed protein milk powder and raw corn starch. She was followed about 6
months. There were no ketoacidotic episodes and hypoglycemia even when she had fever. In conclusion,
patients with atypical features of T2 deficiency should also be investigated early. Gas chromatography
mass spectrometry and next-generation full exome sequencing may be helpful in diagnosis.

miR-200c has been shown to regulate the epithelial-mesenchymal transition (EMT) by inhibiting ZEB1 and ZEB2 expression in breast cancer cells. This study further examined the role of miR-200c in the invasion and metastasis of breast cancer that goes beyond the regulation on ZEB1 and ZEB2 expression. In this study, the bioinformatics software (miRanda) was used to predict the target gene of miR-200c and Renilla luciferase assay to verify the result. The metastatic breast cancer cells MDA-MB-231 were cultured and transfected with the miR-200c mimic or inhibitor. The expressions of miR-200c and HMGB1 were detected by RT-PCR and Western blotting, respectively. Transwell assay and wound healing assay were employed to examine the invasive and migrating ability of transfected cells. Target prediction and Renilla luciferase analysis revealed that HMGB1 was a putative target gene of miR-200c. After transfection of MDA-MB-231 cells with the miR-200c mimic or inhibitor, the expression of miR-200c was significantly increased or decreased when compared with cells transfected with the miR-200c mimic NC or inhibitor NC. Moreover, the expression of HMGB1 was reversely correlated with that of miR-200c in transfected cells. Tranwell assay showed that the number of invasive cells was significantly reduced in miR-200c mimic group when compared with miR-200c inhibitor group. It was also found that the migrating ability of cells transfected with miR-200c mimics was much lower than that of cells transfected with miR-200c inhibitors. It was suggested that miR-200c can suppress the invasion and migration of breast cancer cells by regulating the expression of HMGB1. miR-200c and HMGB1 may become useful biomarkers for progression of breast cancer and targets of gene therapy.

Objective To explore the application of endometriosis fertility index (EFI) in guidance after laparoscopic surgery of endometriosis patients combined with infertility and to explore methods to improve pregnancy rate in different EFI groups. Methods A prospective research was done in endometriosis patients combined with infertility in Beijing Obstetrics and Gynecology Hospital from January 2010 to June 2011, after laparoscopic surgery, these 146 patients were divided into 3 groups by EFI score. Using different pregnancy guidance, these patients had 5 years follow-up. Results (1) The 5 years overall pregnancy rate was 89.0%(130/146). The pregnancy rate was 95.7%(45/47) in EFI≥9 group, 92.8%(77/83) in EFI 5-8 group and 8/16 in EFI≤4 group, three groups were all reach satisfactory pregnancy rate;the rate of the first two groups had no statistically significance (P=0.498), but had significant difference with the last group (P<0.01). (2) In EFI≥5 patients, pregnancy rate was the highest in 6 months after operation;in EFI≥9 group, the pregnancy rate was 66.7%(30/45), and EFI 5-8 group was 50.6%(39/77). (3) EFI≥9 group had the highest natural pregnancy rate [83.6% (46/55)], natural pregnancy rate was significant statistical different in different EFI groups (P=0.001). Conclusions EFI score is a useful evaluation in predicting and guiding pregnancy in endometriosis patients combined with infertility after laparoscopic surgery. EFI score guidance, strict post-operation management and positive pregnancy scheme could significantly improve the pregnancy rate of endometriosis patients with infertility.

Objective To examine the pathological changes in the myocardial tissues such as inflammatory response and fibrosis in a rat model of acute radiation-induced heart damage (RIHD),and to explore whether NF-κB and its downstream pathway are associated with acute radiation-induced myocardial fibrosis. Methods Fourteen nale adult Sprague-Dawley rats were randomly divided into control group and radiation group.Local heart irradiation was delivered to the precordial region of rats to establish an RIHD model in a single fraction with a dose of 20 Gy generated by a 6 MV linear accelerator. At 14 days after irradiation, the histopathological changes in myocardial and interstitial tissues were examined by HE staining;the distribution of collagen fibers was observed by Masson staining, and collagen volume fraction (CVF) was used as a semi-quantitative evaluation for myocardial collagen deposition, which was defined as the percentage of collagen area occupied in total area, and was compared using the independent-samples t test. The protein and mRNA expression levels of the NF-κB members p50 and p65 and the downstream pathway members hypoxia-inducible factor 1α(HIF-1o),connective tissue growth factor (CTGF),and type I (COL-1) were quantitatively analyzed by Western blot and qPCR,respectively. Results At 14 days after local heart irradiation, the radiation group showed significant myocardial edema and derangement, rupturc of some myocardial ceils, mild nuclear pyknosis, darkened nuclear staining,a small number of irregular nuclei, and myocardial interstitial inflammatory cell infiltration accompanied by increased fibroblast,as compared with the control group. The Masson staining showed that the collagen fibers in radiation group were widely distributed at the interstitial tissue and increased significantly compared with those in the control group;normal myocardial cells were in disordered array and had a tendency to be replaced by collagen fibers. The semi-quantitative analysis showed that radiation induced a significant increase in CVF (22.05% vs.3.76%,P =0.003).Western blot and qPCR revealed that the protein and mRNA expression of p50,p65,HIF-1 α,CTGF,and COL-1 was significantly higher in the radiation group than in the control group (all P < 0.05).Conclusions The pathological features of acute RIHD include significant myocardial edema and myocardial interstitial inflammatory cell infiltration accompanied by increased fibroblasts and collagen fibers.Radiation exposure can activate NF-κB and cause the upregulation of HIF-1α and CTGF at both protein and mRNA levels, which may play an important role in the progression of radiation-induced myocardial inflammation to fibrosis.
