1.Risk factors of contrast-induced nephropathy in patients after coronary angiography

Objective To investigate the risk factors of contrast-induced nephropathy (CIN) in patients after coronary angiography.Methods Two hundred patients underwent coronary angiography were enrolled in this study.The patients were divided into CIN group and non-CIN group according to the occurrence of CIN after coronary angiography of 48-72 h,and then the related risk factors of CIN were analyzed.Results Thirteen cases of CIN were found in 200 patients,and the occurrence rate was 6.5%(13/200).Logistic regression analysis showed that risk factors of CIN included primary renal insufficiency,diabetes,contrast agent dose and advanced age (P < 0.05).Conclusion Primary renal insufficiency,diabetes,contrast agent dose and advanced age are risk factors of CIN in patients after coronary angiography.
2.Analysis of the Dietary Intake and Nutritional Status in 227 Pregnant Women

Objective To understand the dietary intake and nutritional status of pregnant women, and try to give a reasonable suggestion to promote the development of fetus. Methods 227 pregnant women in Changsha city were enrolled in this study. Their serum levels of vitamins were detected and their dietary intake were investigated. Results The ratio of the energy which was provided by dietary protein, carbohydrate and fat was appropriate. But the intake of dietary calcium, vitamin B1 and vitamin B2 was very low. With the development of gestation there were a remarkable rise in serum vitamin E level and decrease in serum folacin level, which was extremely obvious in the third trimester. It was about 40% of the pregnant women that the serum vitamin C level was lower than normal level. Conclusions Pregnant women tend to be lack of folacin, vitamin C and vitamin B 2.
3.Prognostic value of first documented rhythms in patients with in-hospital cardiac arrest

Objective To evaluate the prognostic value of the first documented rhythm in patients with in-hospital cardiac arrest.Methods The clinical data of 62 in-hospital cardiac arrest patients collected from May 2011 to April 2016 were reviewed.Multiple regression analysis was performed for determination of the factors relevant to ROSC.The patients were divided into defibrillative rhythm group [ventricular fibrillation (VF) /ventricular tachycardia (VT)] and non-defibrillative rhythm group [pulseless electrical activity (PEA) /asystole] according to the first documented rhythm after cardiac arrest.The differences in rate of restoration of spontaneous circulation,number of survival at hospital discharge and those discharged with favorable neurological outcome were compared between groups.Results Logistic regression analysis showed that non-defibrillative rhythm was related to low rate of ROSC.Rate of ROSC (x2 =6.90,P =0.01),number of survival at hospital discharge (x2 =10.49,P =0.00) and those discharged with favorable neurological outcome (x2 =14.70,P =0.00) were higher in the defibrillative rhythm group,while there was no difference in outcomes between the VF and VT groups.There was no difference in consequence found between the PEA and asystole groups.Conclusion Rate of restoration of spontaneous circulation,number of survival at hospital discharge and those discharged with favorable neurological consequence were higher with defibrillative initial rhythm.
4.Bacteria spectrum and prognosis of exit-site infection in peritoneal dialysis patients

Objective To investigate the bacteria spectrum and prognosis of exit-site infection in peritoneal dialysis patients. Methods Perfect exit, equivocal exit, exit-site infection (ESI) and tunnel infection (TI) were classified. Incidence of ESI, bacteria spectrum, treatment effect and prognosis were statistic. Results Sixty-nine patients' exits were examined monthly, twenty-one episodes of ESI were occurred during eighteen months. Main pathogens of exit-site infection were staphylococcus aureus (47. 6% ) and pseudomonas aeruginosa(28. 6% ) . Seventeen episodes of ESI were cured. Four episodes of ESI accompanied with tunnel infections. TI rate diagnosed by clinical observation and ultrasound was 0. 012/patient-year and 0. 036/patient-year respectively. Conclusions The outcome of catheters is associated with the regiment of bacteria. Tunnel infection often occurs in whom with a long time exit-site infection. Ultrasound examination can clarify the diagnosis simply and quickly.
5.Effect of high glucose on the expression of matrix metalloproteinase 2 and its inhibitors in human peritoneal mesothelial cells

Objective To study the gene and protein expression of matrix metalloproteinase 2 (MMP2) and its inhibitors TIMP1 and TIMP2 in human peritoneal mesothelial cells (HPMC), and the possible role of high glucose in submesothelial extracellular matrix (ECM) degradation during peritoneal dialysis (PD) . Methods Primary HPMC was isolated from spent peritoneal dialysis effluent collected from PD patients. After HPMC confluence, the cells were detached by trypsinization and passaged into 25 cm2 tissue-culture flasks. The effect of high glucose and hyperosmolarity on the gene expression of MMP2, TIMP1 and TIMP2 in HPMC was studied by semi-quantitative RT-PCR. Immunohistochemistry and zymography were used to measure the protein expression of MMP/TIMP in HPMC. 7.Efficient Secretion of Recombinant PEX in COS7 Cells Directed by MMP-9 Signal Peptide

With the rapidly development of the biotechnology industry,large quantities of recombinant proteins are needed for specific therapeutic and diagnostic applications.Bacterial cells are most often used for the production of recombinant proteins.However,recombinant proteins expressed in the cytoplasm of bacteria are often misfolded as insoluble inclusion bodies and therefore inactive.To circumvent this problem,several eukaryotic expression systems have also been developed over the years,ranging from yeast to mammalian cell-based technologies.For many mammalian proteins,especially those secreted and modified posttranslationally,a more compatible expression system is highly desirable because proper folding or modification can only be provided with closely related cells,i.e.,mammalian cells.Large scale transient transfection of mammalian cells is a recent and powerful technology for the fast production of milligram amounts of recombinant proteins.Transient expression by means of extrachromosomal replication in COS cells is frequently used to check the functional integrity of genes/plasmids and to produce small quantities of cell supernatants containing the protein of interest.As it is allowed for easy and efficient purification,many recombinant proteins used for therapeutic and structural studies are naturally secreted or engineered to be secreted.The use of a proper signal peptide is one of the major determinants for the efficient secretion of heterologous proteins from mammalian cells.The noncatalytic C-terminal hemopexin-like domain of MMP-2,PEX,can block angiogenesis and tumor growth in vivo.Large quantities of biochemically active recombinant PEX are required for the study of their functions and biochemical properties,as well as for their industrial applications.For this purpose,the rat growth hormone,mouse IgG? chain and MMP-9 signal peptides were used for expression of PEX in COS7 cells,and their secretion efficiencies were compared by Western blotting and ELISA.Western-blotting of PEX protein from culture media,resulted in detection of proteins with the predicted molecular mass,which indicate that all of the signal sequences could direct PEX secretion successfully.The MMP-9 signal peptide seems to be superior to the signal peptides from IgG and rGH both in terms of extracellular yield and in terms of secretion efficiency.Thus,expression of pM9PEX construct resulted in higher yields of extracellular PEX and the majority of the produced PEX was secreted and not trapped intracellularly.To examine whether the observed difference in secretion yields is promoted at the transcriptional level,a RT-PCR analysis was performed at 6 h after transfection.The presence of mRNA transcripts of PEX was observed in all the DNA constructs.Moreover,semiquantitative reverse transcription(RT-PCR)results show that there were no significant differences in the expression levels of PEX among the constructs at 6 h after transfection.Though there was no difference in the expression levels of PEX at an early time point after transfection,the presence of an ER-targeting signal peptide sequence in the expression vector affected the trafficking of expressed proteins in the cells.Hence,the described difference in exported yields is probably promoted at the secretion level,rather than at the transcriptional level.Chick chorioallantoic membrane(CAM)bioassay show that the PEX protein purified from cell culture had biological activity to inhibit the angiogenesis.The MMP-9's signal peptide is used for the first time as leader sequence for secretion of foreign proteins.The results revealed that higher amounts of secreted PEX were obtained when vectors containing MMP-9 signal peptide were used and it is also indicated that MMP-9 signal sequence could be effective on promoting the secretion of other heterologous proteins in eukaryotic cells.
8.Complications of femtosecond laser corneal small incision lenticule extraction

AIM:To investigate the safety and complications of femtosecond laser corneal small incision lenticule extraction(SMILE) procedure and discuss the prevention and treatment.METHODS: We retrospectively studied the complications of 403 patients(799 eyes) with myopia and myopic astigmatism treated by SMILE.RESULTS: All the patients underwent the operation successfully.Only 1 case(1 eye)suffered from dark spot and changed to femtosecond laser-assisted LASIK(FS-LASIK), 5 cases(5 eyes, 0.6%) suffered from the suction loss, 11 cases (17 eyes, 2.1%) developed opaque bubble layer.All patients gained perfect uncorrected visual acuity(UCVA) (20/20).The best corrected visual acuity(BCVA) did not decrease after operations.The incidence of haze and diffuse lamellar keratitis was low (0.3% and 0.4%, respectively)and no other complications were observed.There was 9 eyes in 6 patients (1.1%) found regression of refraction at 6mo after surgery, while the UCVA of rest patients reached 1.0 at 3mo after surgery.CONCLUSION: The SMILE procedure has high safety for myopia and myopic astigmatism.Effective prevention and management of the complications is the key to achieve the satisfactory visual acuity.
9.Effects of beraprost sodium on apoptosis in cultured mouse podocyte induced by angiotensin Ⅱ

Objective To explore the effects of beraprost sodium (BPS) on angiotensin Ⅱ(AngⅡ)-induced podocyte apoptosis and the relationship between BPS and peroxisome proliferator activated receptor δ(PPARδ). Methods Differentiated mouse podocytes were exposed to AngⅡ, further treated with BPS (or GSK0660) for 24 h. Podocyte apoptosis was detected by flow cytometry and AnnexinV-FITC staining. Bax , Bcl-2 and PPARδ mRNA were assessed by RT-PCR. Results AngⅡ promoted podocyte apoptosis and Bax mRNA expression significantly, down-regulated Bcl-2 and inhibited PPARδ mRNA (P < 0.05). Conversely, treatment with BPS reduced AngⅡ-induced podocyte apoptosis and Bax mRNA expression and promoted Bcl-s expression significantly resulting in an increase of PPARδ mRNA expression (P < 0.05). With the inhibition of PPARδ, podocyte apoptosis and Bax mRNA expression increased while Bcl-2 mRNA expression reduced. Conclusion BPS can effectively reduce AngⅡ-induced podocyte apoptosis , which is associated with the activation of PPARδand regulation of Bcl-2/Bax mRNA expression.

The paper introduced the practice of family doctor′s contracted service in Zhenjiang,Jiangsu province over the past six years.By exploring the tiered family doctor′s contracted services,namely 3+X teamwork responsible for household health,health insurance contracting with chronic disease patients,and personalized contracts,the city highlights the cornerstone role of primary care in the Healthy Zhenjiang 2015 Campaign.The campaign aims at providing fair,accessible,systematic and uninterrupted health services.Outcomes of the campaign have proved that the 3+X teamwork model helps building acquaintances relationship with residents by means of contracting,hence motivating both the sector and individuals in the health promotion and building a healthy Zhenjiang.
