1.Clinical Observation of Electroacupuncture for Gastrointestinal Dysfunction After Abdominal Operation

Objective To compare the therapeutic efficacy between conventional nursing and conventional nursing plus electroacupuncture in treating gastrointestinal dysfunction after abdominal operation, and their effects in recovering gastrointestinal function. Method Sixty patients with gastrointestinal dysfunction after abdominal operation were randomized into an electroacupuncture group and a control group according to their operation time, 30 cases in each group. The electroacupuncture group was intervened by conventional postoperative nursing plus electroacupuncture, by selecting bilateral Zusanli (ST36), Shangjuxu (ST37), Xiajuxu (ST39), Neiguan (PC6) and Taichong (LR3), connected to HANS electroacupuncture device, with frequency at 2 Hz and a tolerable intensity. The treatment was given once a day, for 5 d in total, till the recovery of gastrointestinal function. The control group was intervened by conventional postoperative nursing. The therapeutic efficacy was evaluated at the end of the intervention. The time of first bowel sound, first exhaust time, and first bowel movement time were observed. The gastrointestinal symptom score (GIS) was adopted to evaluate the effects of the two methods in improving gastrointestinal function at the end of the intervention. Result The general therapeutic efficacy of electroacupuncture group was better than that of the control group(P<0.05). In the electroacupuncture group, time taken for the first bowel sound, first exhaust and first bowel movement after the operation was shorter(P<0.05). The GIS dropped in both groups after the treatment, and the improvement was more significant in the electroacupuncture group(P<0.05). Conclusion Electroacupuncture helps promote the recovery of gastrointestinal function in patients with postoperative gastrointestinal dysfunction, and can produce a more significant improvement.
2.Lateral decubitus versus supine position in treatment of intertrochanteric fractures with proximal femoral nail antirotation

Objective To compare the lateral decubitus and supine position in the treatment of femoral intertrochanteric fractures with femoral proximal nail antirotation (PFNA).Methods A retrospective analysis was performed of the 85 patients with intertrochanteric fracture who had been treated at our hospital between September 2013 and September 2014.They were 24 men and 61 women,28 to 99 years of age (average,78.5 years).By Evans-Jensen classification,one case was type Ⅰ,15 cases were type Ⅱ,40 type Ⅲ,23 type Ⅳ,and 6 type Ⅴ.All the patients were treated with internal fixation with PFNA.Lateral decubitus position was adopted in 45 cases and supine position in 40.The 2 groups were compared in terms of operation time,intraoperative bleeding volume,difference between preoperative and postoperative hemoglobin values,tip-apex distance (TAD),calcar tip-apex distance (Cal-TAD),fracture healing time,and Harris score of the hip joint.Results The 85 patients were followed up for 9 to 21 months (average,13.6 months).Both 2 groups obtained fracture healing,with no screw cut-out.The lateral decubitus group had significantly less operation time (60.3 ± 17.5 min) and intraoperative bleeding volume (70.8 ± 37.8 mL) than the supine position group (72.7 ± 19.7 min and 90.3 ± 20.9 mL,respectively) (P < 0.05).The former also had a significantly smaller difference between preoperative and postoperative hemoglobin values (24.1 ± 5.7 g/L) than the latter (28.2 ± 8.5 g/L) (P < 0.05).There were no significant differences between the 2 groups in TAD,Cal-TAD,fracture healing time (3.7 ± 0.9 months versus 3.7 ± 1.0 months),or Harris hip score (73.4 ± 10.7 versus 75.5 ± 9.5) (P > 0.05).Conclusion In the treatment of femoral intertrochanteric fractures with PFNA,lateral decubitus position may be associated with shorter operation time and less intraoperative blood loss compared with supine position.
3.Percutanous microwave ablation for benign focal liver lesion: an initial study

Objective To discuss the principle and the value of pereutanous microwave ablation for benign focal liver lesions. Methods Fourteen patients with 15 benign focal liver lesions received percutanous microwave ablation with 15G cool-tip microwave antenna. The maximum diameter of lesion was (26.2±13.6)mm (10 - 57 mm). Results Treatment sessions and total treatment time for single lesion were 1.4±0. 5 (1- 2) and (837±574)s (300- 2057 s). Follow-up range was 1- 35 months (median 6 months). The hepatic lesions were covered by coagulation necrosis region of ablation completely in all cases on contrast-enhanced at 1 month after ablation. The area of coagulation necrosis of ablation decreased on follow-up imaging in patients with more than 3 months follow-up. No complications happened in all cases. Side effects include pain (12/14) and fever (8/14) lasting I -9 days. The increase of serum transaminase decreased to normal at 1 week after ablation. Conclusions Percutanous microwave ablation has the merit of safety,minimal invasion, good ablation result and low incidence of side effect. Under the precondition of eareful choice of indication,pereutanous microwave ablation can be a selective treatment method for benign focal liver lesions.
4.Application of pressure-controlled ventilation-volume guaranteed in obese patients with obstructive sleep apnea syndrome

Objective To explore the clinical effectiveness of pressure-controlled ventilation-volume guar-anteed(PCV-VG)in obese patients with obstructive sleep apnea syndrome(OSAS)during uvulopalatopharyngo-plasty. Methods 40 obese patients(BMI≥30 kg/m2)with OSAS scheduled for uvulopalatopharyngoplasty under general anesthesia were randomly divided into two groups of volume-controlled ventilation(group V,n = 20)and pressure-controlled ventilation-volume guaranteed(group P,n=20). The heart rate(HR),mean arterial pressure (MAP),arterial partial pressure of oxygen(PaO2)and arterial partial pressure of carbondioxide(PaCO2)were recorded before induction of anesthesia without oxygen inhalation(T0),30 min(T1)and 1 h(T2)after tracheal intubation,and 30 min after extubation(T3). The peak airway pressure(Ppeak),airway resistance(Raw),thoracic compliance (CL),oxygenation index (OI) and respiratory index (RI) were also calculated at T1 and T2 under observation of recovery. Results There were no obvious differences between the two groups of patients before anesthesia and after recovery. Compared with the group V ,PaCO2 ,PPEAK ,Raw at T1 ,T2 and RI at T1 ~ T3 of the group P decreased(P<0.05),while CL at T1,T2 and PaO2,OI at T1~T3 increased(P<0.05). There were no sig-nificant differences in HR ,MAP at the above time points. Conclusions Compared with volume-controlled venti-lation,PCV-VG can effectively enhance thoracic compliance,lower inspiratory pressure and airway resistance ,and decrease intrapulmonary shunt ,which is conductive to improve arterial oxygenation and gas exchange in obese patients with OSAS.
5.Protective effects of propofol preconditioning on myocardium against hypothermia ischemia normothermia reperfusion injury on isolated rat hearts

Objective To investigate the protective effects of propofol preconditioning on myocardium against hypothermia ischemia normothermia reperfusion injury on isolated rat hearts. Methods The Langendorff apparatus was used.Sixty SD rat hearts were divided randomly into 5 groups after 20-minute equilibrium(n=12): control(Con) group,hearts were continually perfused with K-H buffer for 175 min;ischemia/reperfusion(I/R) group,hearts were perfused with K-H buffer for 40 min,then subjected to global ischemia at 27 ℃ for 75 min,and followed reperfusion at 37 ℃ for 60 min;propofol preconditioning group 1(P1),group 2(P2),and group 3(P3),hearts were perfused with K-H buffer including 50,100,and 150 ?mol/L propofol for 10 min and followed reperfusion like I/R group,respectively.Heart rate(HR),left ventricular end-diastolic pressure(LVEDP), left ventricular developed pressure(LVDP) and ?dp/dtmax at the end of equilibration,pre-ischemia and at the end of reperfusion were recorded.The contents of creatine kinase(CK) and lactate dehydrogenase(LDH) in coronary effluent were measured at the end of equilibration and 1,10,20,30,and 60 min during reperfusion.The activity of superoxide dismutase(SOD) and the contents of maleic dialdehyde(MDA) were measured at the end of reperfusion.The area of infarct region was determined at the end of reperfusion. 7.Risk Factors for Plagiocephaly and Brachycephaly

Objective To investigate the factors related with plagiocephaly and brachycephaly. Methods 239 infants with plagiocephaly and brachycephaly were investigated, and analyzed with univariate analyses and multivariate Logistic regression analysis. Results The factors, such as gestational age birth (OR=0.636, P<0.001), birth weight (OR=0.095, P<0.001), time of hospitalization (OR=1.307, P<0.001), preterm birth (OR=2.649, P<0.001), stay in newborn intensive care unit (OR=4.456, P<0.001), change the position (OR=0.046, P<0.001), accepted early intervention guidance (OR=0.054, P<0.001), were significantly related with plagiocephaly and brachycephaly. Conclusion Preterm birth, low birth weight, and newborn complications are the risk factors for plagiocephaly and brachycephaly, while change the position and early intervention may prevent it.
9.Dynamic study and screening of new markers of spermatogonial stem cells by iTRAQ protein mass spectrometry.

OBJECTIVETo study the dynamic changes in the protein marker expression in the spermatogonial stem cells (SSCs) of mice at different ages by iTRAQ protein mass spectrometry and to screen new markers using the bioinformatic proteome database.
METHODSBased on the postnatal weeks, we divided 80 healthy male C57BL/6 mice into eight age groups of equal number, harvested their testicular tissues, extracted proteins following purification of the SSCs by compound enzyme digestion and magnetic-activated cell sorting. Then we analyzed and identified proteins using two-dimensional electrophoresis, protein mass spectrometry, and protein database information.
RESULTSTotally, 248,510 mass spectra were obtained from the MS experiment and 1132 proteins were identified. By the criteria of >1.2-fold for protein abundance difference and P value <0.05, we identified 298 differentially expressed proteins and 9 currently known makers of SSCs (PCNA, GFRalpha1, CDH1, Annexin A7, UCHL1, VASA, CD49f, CD29, and PLZf). Compara- tive analysis showed different expressions of the proteins in the SSCs of the mice of different ages, and the differences in the expressions of GFRalpha1, CD49f, and CD29 were consistent with the findings in other published literature. Ten proteins (P63, CD71, CD98, K19, ACE, K18, K15, K17, SH2, and SH3) were selected as SSC markers to be further studied.
CONCLUSIONThe proteins in SSCs are differentially expressed in mice of different ages. The technology of iTRAQ protein mass spectrometry can be used to analyze and compare the proteome information of mouse SSCs, obtain differentially expressed proteins in mice of different ages, and thus offers a new ap- proach to further analysis and study of the function and roles of these differential proteins.

10.Drinking-water type endemic fluorosis in Northern Jiangsu province in 2008 : an analysis of survey results

Objective To investigate the state of endemic fluorosis, running status of water improvement project to reduce fluoride in Jiangsu province, and to provide a scientific basis for prevention and control of endemic fluorosis. Methods In 2008, in the ten key counties of endemic fluorosis (zone), in Xuzhou,Lianyungang and Suqian, a stratified sampling method was employed to select 40 diseased villages according to their past water fluoride survey data. All children aged 8 to 12 were examined dental fluorosis, and all adults over 16 years were examined clinical skeletal fluorosis. Thirty per cent of the 40 diseased villages were selected, and 20 adults over the age of 16 in each selected village were examined by X-ray, respectively;50% of the 40 diseased villages were selected, and 30 any time urine samples of children aged 8 to 12 in each diseased village were tested urine fluoride. In each city, select a county, the status of water improvement project to reduce fluoride, water supply capacity and coverage in the county were investigated. Results A total of 3560 children aged 8 to 12 were examined, the detection rate of dental fluorosis was 38.51% (1371/3560), tooth defect rate was 5.34% (190/3560), and dental fluorosis index was 0.8. Seven hundred and eight urine samples were tested, the median urinary fluoride was 1.47 mg/L and the range was 0.08 ~ 10.08 mg/L. Clinical detection of skeletal fluorosis was 21.3% among adults over the age of 16, and X-ray detection rate of skeletal fluorosis was 39.2% (123/314).Investigated a total of 248 facilities of centralized water improvement projects, no funds to run or damaged 49, the water fluoride > 1.0 mg/L was 18. Conclusions Endemic fluorosis in Northern Jiangsu province has not been controlled completely, but has a rising trend, we should further strengthen the supervision of water fluoride reduction.
