1.Safety of CT-guided percutaneous lung biopsy in elderly and its risk factors

Objective To evaluate the safety of CT guided percutaneous lung biopsy in the elderly,and to analyze the risk factors for its complications.Methods Totally 204 elderly patients aged ≥65 years underwent transthoracic needle biopsy under CT guidance.Clinical data,complication and its risk factors were analyzed retrospectively.Results The complications included pneumothorax accounting for 15.2% (31/204),hemoptysis 21.6% (44/204),and no mortality was found.Logistic regression analysis showed that needle path ≥5.00 cm,lesion size ≤2.00 cm and emphysema around the lesion were the independent risk factors for postoperative pneumothorax (OR=4.05,2.54 and 3.97,all P<0.05).Meanwhile,the enhancement degree by CT >40 HU,needle path ≥5.00 cm and lesion size ≤2.00 cm were the independent risk factors for postoperative bleeding (OR=2.27,2.82 and 2.33,all P<0.05) in elderly patients undergoing lung biopsy.The sensitivity and specificity for postoperative pneumothorax and bleeding obtained by forecasting equation were 87.1% and 49.1%,54.5% and 80.8% respectively.Conclusions The length of needle path,lesion size and emphysema around the lesion are correlated with pneumothroax,and length of needle path,lesion size and the CT enhancement degree are correlated with bleeding after percutaneous lung biopsy in the elderly.
2.Effects of sulphated heparin on proliferation and apoptosis of human hepatocellular carcinoma cell line (HepG2)

AIM: To explore the effect and the mechanism of sulphated heparin on the proliferation and the apoptosis of human hepatocellular carcinoma cells. METHODS: The human hepatocellular carcinoma cell line (HepG-2) was used to identify the expression of ras gene protein and to study the effect of sulphated heparin on proliferation and the apoptosis in vitro . RESULTS: The sulphated heparin downregulated the ras protein expression and inhibited the cell growth in HepG2 cells. In the presence of sulphated heparin, the apoptosis rate of HepG2 increased. CONCLUSION:The data suggest that the effects of sulphated heparin on the proliferation and the apoptosis of the human hepatocellular carcinoma cell are correlated with the signaling transduction mediated by ras gene protein.
3.Effect of high intensity interval training on patients with cancer: a Meta-analysis

Objective:To evaluate the effect of high intensity interval training on cancer patients.Methods:Web of Science, EBSCO, PubMed, Ovid, EMbase, Cochrane Library, CBM, CNKI and Wanfang database were searched for randomized controlled trials of the effect of high intensity interval training on cancer patients, RevMan5.3 software was used for analysis.Results:Eleven articles with a population of 614 were included in the analysis, which involved 323 cases of the intervention group and 291 cases of the control group. The analysis showed that high intensity interval training could improve the total quality of life ( SMD values were 0.40, 95% CI 0.14-0.65, P<0.01) and peak oxygen uptake ( MD values were 3.00, 95% CI 2.19-3.80, P<0.01) of cancer patients, and reduce carotid intima-media thickness ( MD values were -0.06, 95% CI-0.08--0.03, P<0.01), but did not reduce the high-sensitivity C-reactive protein ( MD values were -0.61, 95% CI-1.23-0.01, P>0.05) and body weight ( MD values were 0.16, 95% CI-3.19-3.51, P>0.05). Conclusions:High intensity interval training has a positive impact on quality of life, cardiopulmonary function and carotid intima-media thickness with cancer patients, but has no significant effects on high-sensitivity C-reactive protein and body weight.

5.Expression of CIP2A, bcl-2 and p63 in papillary thyroid cancer and their significances

Objective To investigate the expression and significance of cellular inhibitor of protein phosphatase 2A (CIP2A), bcl-2 and p63 in papillary thyroid cancer (PTC). Methods Using immunohistochemistry to detect the expression of CIP2A, bcl-2 and p63 in 30 cases of nodular goiter (NG), 30 cases of thyroid adenoma (TA) and 57 cases of PTC [including classical PTC (cPTC) 20 cases, papillary microcarcinoma (PMC) 20 cases, follicular thyroid papillary carcinoma (FPTC) 7 cases]. Results In NG group, TA group and PTC group, positive rates of CIP2A were 0, 0 and 94.74 % (54/57), respectively. The differences were statistically significant. In NG group, TA group and PTC group, positive rates of bcl-2 were 16.67 % (5/30), 13.33 % (4/30) and 85.96 % (49/57), respectively. The differences were statistically significant. In each group, positive rates of p63 were 6.67% (2/30), 3.33% (1/30) and 5.26% (3/57), respectively, no significant difference among them. In PTC, expression of CIP2A and bcl-2 were significantly higher than in NG and TA (χ2 = 105.56, P= 0.00; χ2 = 58.95, P= 0.00). Furthermore, the expression of CIP2A and bcl-2 had correlation in PTC (r=0.94, P=0.00). The expression of CIP2A, bcl-2 and p63 had no significantly difference among all the PTC subtype (χ2 values were 2.02, 2.64, 1.85; all P> 0.05). The expression of CIP2A, bcl-2 and p63 was not associated with patients'age, sex, site, lymph node metastasis (all P>0.05). Conclusions High expression of CIP2A and bcl-2 is associated with PTC, and the expression of CIP2A and bcl-2 has correlation in PTC. The expression of p63 has no correlation with PTC.
6.Effects of rhubarb extracts on hyperexcitability of hippocampal CA1 neurons after fluid percussion injury

Objective To investigate the effects of rhubarb extracts, i.e. rhein and emodin, on the neuronal hyperexcitability and synaptic transmission, and to further reveal the mechanism of the secondary brain damage. Methods The fluid percussion injury (FPI) rat model and extracellular recording method were used. The evoked field potentials by stimulating Schaffer collaterals were collected from the ipsilateral (impact side) and the contralateral hippocampal CA1 areas of rat in vitro. And the field potentials, including the field excitatory postsynaptic potential and the population spike, were analyzed. Results After the impact was performed on the rat parietal cortex, the evoked field potentials in the ipsilateral hippocampus CA1 area were enhanced obviously. Rhubarb extracts reduced the slope of the field excitatory postsynaptic potential and the number of the population spike significantly while rhein and emodin increased the latency of the population spike obviously. Conclusion Rhubarb extracts, i.e. rhein and emodin, can depress the neuronal hyperexcitability, which suggests that rhein and emodin play an important role in protecting the central nervous system from neuronal damage after traumatic brain injury. FPI produces hyperexcitability of hippocampal CA1 neurons, probably by enhancing excitatory synaptic transmission.
7.Analysis of prescriptions for eradication of Helicobacter pylori in community health service centers of Shanghai Jing′an district

A total of 8 099 outpatient prescriptions for treatment of Helicobacter pylori (Hp) infection in 9 community health service centers of Shanghai Jing′an district were collected from the regional health information platform.The prescriptions were reviewed and analyzed according to the criteria of the Forth National Consensus Report on the Treatment of Helicobacter pylori Infection.The results showed that only 195 prescriptions met the criteria (2.4%).For those not meeting the criteria, 611 prescriptions (7.5%) gave the right therapeutic protocol but incorrect treatment course;6 893 prescriptions (85.1%) gave the incorrect protocol.It is uggested that incorrect treatment for Hp eradication is common in community health service centers consist in the Helicobacter pylori herapy prescriptions in the community health service cente in Shanghai, Jing′an District, which should be paid more attention to.
8.Effects of PMA(phorbol-12-myristate-13-acetate)and laminin on adhesion and proliferation of human hepatocellular carcinoma cell

AIM: To explore the effects of PMA(phorbol-12-myristate-13-acetate,a tumor promoter, mimicking the action of diacylglycerol on PKC)and laminin on the adhesion and the proliferation of human hepatocellular carcinoma cells, and provide a new clue to liver cancer treatment. METHODS: Human hepatocellular carcinoma cell line(BEL-7402)was used to identify the endogenous laminin and protein kinase C-?(PKC-?) expression, and the effects of laminin and PMA on the adhesion and the proliferation were also investigated in vitro. RESULTS: By the effect of exogenous laminin, human hepatocellular carcinoma cell (BEL-7402) possessed endogenous laminin expression and increased the adhesion and the proliferation, which was showed the synergistic action by the effect of PMA in combination. By the action of PMA alone,the proliferation and the PKC-? expression increased by exogenous laminin were decreased, and the adhesion and the endogenous laminin expression were increased. CONCLUSIONS: The finding suggested that the adhesion and the proliferation of human hepatocellular carcinoma cell were closely related to the effects of endogenous or exogenous laminin ,which were associated with cPKC-? activity. Therefore,the application of anti-laminin antibody in combination with PKC antagonist might be a new clue to find out the therapy for liver cancer.

10.Role of ERS in Astragaloside Ⅳ-induced cardioprotection against ischemia/reperfusion injury in rats

Aim To explore the role of endoplasmic re-ticulum stress( ERS) in Astragaloside Ⅳ-induced car-dioprotection against ischemia/reperfusion injury in rats. Methods A model of myocardial ischemia 30 min followed by 120 min reperfusion was made by liga-ting coronary artery in male Wistar rats. Rats were di-vided randomly into 4 groups: sham group, ischemia/reperfusion group, ERS inhibitor TUDCA group, As-tragaloside Ⅳgroup. Myocardial samples were collect-ed from the risk zones during ischemia and reperfu-sion, ERS was determined by measuring levels of glu-cose regulated protein 78 ( GRP78 ) , an established marker of ERS with Western blot. Immunofluorescence study was used to test GRP78 intensity with laser scan-ning confocal microscopy, TTC method was used to measure the infarct size,hematoxylin-eosin staining was used to observe the changes of morphological changes of myocardium. Results There was no statistical difference in GRP78 expression during ischemia com-pared to the sham group, but was markedly increased upon reperfusion. Astragaloside Ⅳ could mimic TUD-CA and significantly decreased the GRP78 expression, reduced infarct size and improved the morphology of myocardial tissue with a significant statistical difference compared with the control group ( P<0. 05 ) . Conclu-sions ERS is induced upon reperfusion but not during ischemia in isolated rat hearts. Astragaloside Ⅳ pre-vents myocardial reperfusion injury presumably by the inhibition of ERS.
