1.Multi-slice spiral CT and airway reconstruction in 33 patients with obstructive sleep apnea syndrome

This study was designed to investigate the application of multi-slice spiral CT(MSCT)and airway reconstruction in the patients with obstructive sleep apnea syndrome(OSAS).Using GE Lightspeed16 scanner,MSCT and airway three-dimensional reconstruction were performed on 33 patients with OSAS,who were admitted in the Chinese PLA General Hospital between 2004 and 2005.The sagittal diameter,coronal diameter and the section area of upper airway were measured.The results showed that the narrowest place of the airway in OSAS patients was the posterior soft palate region,in which the sagittal diameter,coronal diameter and the section area were 8.96 mm,16.64 mm and 164.62 mm2 respectively,and then was palatine uvula region.With MSCT scanning and post-processing,a precise measurement of airway can be gotten,and stenosis locus and scope are easy to be localized,which will provide reliable evidences for treatment scheme and surgical mode.
2.Application of AH Plus root canal filling paste in root canal therapy

To compare the postobturation pain and therapeutic effects in root canal therapy between two root canal filling materials: AH Plus and zinc oxide eugonol. Four hundred and twenty four teeth requiring root canal therapy were randomly divided into 2 groups: AH Plus ( trial group ) or zinc oxide eugonol ( control group) were used in treatment respectively. The pain intensity after treatment was evaluated by visual analogue scale. The therapeutic effects were evaluated by follow-up for 6 and 12 months. The pain intensity of trial group was significantly lower than that of the control group in 3rd and 7th day[8. 5% ( 18/212) vs. 15.6% (33/212),5.2% (11/212) vs. 14.6% (31/212),both P <0.05]. There were no significant differences in effictive rate between two groups after follow-up for 6 and 12 months[92. 5% ( 196/212) vs. 89. 6% ( 190/212 ) ,91.5% ( 194/212 ) vs. 88.7% ( 188/212 ), both P > 0. 05]. AH Plus root canal filling paste used in root canal therapy can significantly decrease postoperative pain.
3.Application of Electrical Stimulation in Regeneration of Peripheral Nerve(review)

Injury of peripheral nerve is a so common complication in trauma, that it is important to improve regeneration to recover the function of peripheral nerve. This paper has reviewed the pathology after injury of peripheral nerve, condition of successful regeneration of peripheral nerve, experiment and clinical study of electrical stimulation promoting regeneration of peripheral nerve and the possible mechanism. The effect of electrical stimulation on promoting regeneration of peripheral nerve has been gradually accredited. However, further study is still needed to discover the many unknown field.

6.Influence of inflammatory cells on early-stage reperfusion injury of canine lung allograft

Objective:To investigate the roles of donor alveolar maerophages and the recipient circulating neutrophils in early-stage reperfusion injury of lung allograft,and to study the interaction between the 2 kinds of cells.Methods:Twenty pairs of size-and weight-matched adult mongrel dogs were randomly assigned to 4 groups:C(control),D(leukocyte-depleted blood reperfusion),M(maerophage inhibition)and DM(leukocyte-depleted plus macropbage inhibition).The 20 cases of left lung transplantations were performed by the same surgeon.All procedures were identical,except that the donors in Group M and DM received the macrophage inhibitor gadolinium chloride(14 mg/kg)intravenously 24 h before operation,and that the recipients in Group D and DM underwent initial 10 min reperfusion with leukocyte-depleted blood collected from donors'inferior vena cava. All lung allografts were reperfused for 2 h.Results:Compared with Group D and C,macrophage inhibition ameliorated PO_2/FiO_2 and mean pulmonary arterial pressure(mPAP)consistently after 30 min reperfusion in Group M and DM;the parameters of lung reperfusion injury(malonaldehyde activity,wet/dry ratio)at 120 min after reperfusion were also significantly improved(P

Objective To explore the outcome of monoclonal gammopathy of undetermined significance (MGUS). Methods The data from 14 MGUS patients in our hospital including clinical features, outcome and change of M protein concentration were analyzed retrospectively. Results The MGUS didn't have the clinical manifestations of multiple myeloma (MM), the time of outcome from MGUS to MM was about 4-20 years (mean time, 10 years). The most types of MM were IgA and IgG, 6 cases were IgA type, 6 cases were IgG type and 2 cases were light chain type. The concentration of immune globulin in general showed an upward trend year by year. A few showed fold lines ascend. Conclusions The elevated monoclonal immunoglobulin may develop into MM after many years. We must follow up frequently to avoid error diagnosis and missed diagnosis.
9.Safety and efficacy of intra-arterial infusion neoadjuvant chemotherapy for local advanced bladder cancer

Objective To evaluate the safety and efficacy of intra-arterial infusion neoadiuvant chemotherapy in local advanced bladder cancer. Methods Nineteen cases with T2-T4a bladder cancer were enrolled in this study.Intra-arterial infusion chemotherapy with Gemcitabine and Cisplatin (GC)were performed for 1 to 3 cycles before radical cystectomy.Postoperative values of hematological parameters,maximum diameter of tumors,TNM(tumor,node and metastasis)stages and pathological grades were compared with preoperative parameters of the same case. Results Compared to the values before GC chemotherapy,WBC count showed no significant change post-operative,(6.63±2.58)×109/L vs(5.12±2.91)×109/L(P=0.13);RBC(4.41+0.52)×1012/L vs(3.92±0.42)×1012/L(P=0.00)and platelet count(220.50±59.86)×109/L vs(157.05±56.72)×109/L(P=0.001)showed significant decrease;ALT did not show significant decrease(20.00±8.31 vs 26.88±17.04 U/L,P=0.08);Creatltme also showed no significant change(95.82±14.57 vs 88.04±17.76μmol/L,P=0.06);Maximum diameter of tumors decreased significantly(3.72±1.23 vs 2.80±1.29 cm,P=0.02).Compared with clinical TNM stages,pathological TNM stages demonstrated significant decrease in 9 cases;While cell differentiation did not show decrease. Conclusions Intra-arterial infusion with GC regimen can reduce tumor size,decrease TNM stages,while not causing significant adverse impact to radical cystectomy.Bladder-spare treatment is an option for chemotherapy-sensitive cases.
10.Evaluation CT with MRI image fusion technique on delineation GTV for glioma

Objective To investigate the way to accurately delineate gross tumor volume (GTV) of high grade gliomas(HGG) for intensity modulated radiation therapy (IMRT) by using computed tomography (CT) and magnetic resonance imaging (MRI) image fusion technique. Methods CT and MRI images were fused from 19 patients. The GTV of each patient were independently delineated by one chief doctor and one resident doctor on CT and MRI image. The GTV contoured on CT (GTVCT), MRI (GTVMRI) were measured, and composite volumes (GTVCT+MRI) were the sum of CT-defined GTV and MRI-defined GTV. The differences of these volumes were compared. Results Whether chief or resident doctors delineated, all were GTVMRI >GTVCT(P <0.050). The percentages of GTVMRI on GTVCT+MRI were (98.57±7.00)% by chief doctors, and (97.84±10.00)% by resident doctors. Compared the difference between GTVCT and GTVMRI in postoperative patients and preoperative patients, P =0.046, and the difference between chief doctors and resident doctors was statistically significant for GTV defined by CT (P =0.020), but not by MRI and composite image (P >0.050).Conclusion The GTV of HGG patients must be delineated on both CT image and MRI image, including using CT and MRI image fusion. But the composite volumes(GTVCT+MRI) should be the sum of CT-defined GTV and MRI-defined GTV. Especially for the postoperative patients,delineating GTV should be taken more attention. And the GTV should be delineated by doctors with full experiences.
