1.Analysis of driving adaptability based on psycho-social factors

Objective To discuss the impact of relevant psycho-social factors on the driving adaptability. Methods A total of 870 vehicle drivers were screened out by stratified cluster sampling,and 620 drivers were finally selected for this study after eliminating those who had 1 -2 accidents. The 620 drivers were divided into accident group and non-accident group. Demographic questionnaire, life event scale, SCL-90 Scale and Eysenck Personality Questionnaire were applied to investigate their general condition, life event, mental health and personality. All data were analyzed by t test and stepwise Fisher discriminatory, and then the discriminatory equations of driving eligibility were established. Results The scores of factors including positive life events, negative life events, family events, work events and social events in accident group were 9.62 ± 11.36, 34.53 ± 30.01, 24.05 ± 23.09, 16.78 ± 17. 12 and 2.82 ± 5.07, respectively, which were higher than 4.67 ± 6.25, 6.38 ± 10. 15, 6.48 ± 9.43, 3.96 ±6.55 and 2.02 ± 5.43 respectively in the non-accident group ( P < 0. 01 ). The total SCL-90 score and the scores of all factors in the accident group were higher than those in the non-accident group ( P <0. 01 ). The Eysenck Personality Questionnaire scores of the accident group were significantly higher than the non-accident group in N dimension ( 56. 32 ± 8.77 vs. 45.23 ± 8.06 ) and P dimension (52.22 ±10.43 vs. 48.31 ± 9.35 ) ( P < 0. 01 ). Fisher discriminatory equations: Faccident group = 0. 258 × X6 + 0.119 ×X9 +0.637 ×X17 +0.043 × X5-7.476; Fnon-accident group =0. 137 × X6-1. 000 ×X9 + 1.423 ×X17 +0.003× X5-2.601. Total discrimination accuracy of verification was 88.3%, and the total accuracy of prospective discrimination was 88.0%. Conclusions There is clear correlation of driving adaptability with coercion, anxiety, emotion and negative factors. The discriminatory equations are useful for selecting drivers because of high accuracy, and may play an important role in reducing the occurrence of traffic accidents.
2.Clinical evaluation of rapid diagnosis of fungal keratitis by the combination of corneal scraping with laser scanning confocal microscopy

Background The rapid diagnosis can win more treating opportunities for patients with fungal keratitis.Even though the fungal culture is the gold standard for the diagnosis of fungal keratitis,it is difficult in early diagnosis due to the long duration of cultivation and false-negative rate.Objective This trial was to explore the clinical value in the rapid diagnosis of fungal keratitis by the combination of corneal scraping with laser scanning confocal microscopy.Methods Corneal scraping and laser scanning confocal microscopy were separately performed in 167 eyes of 167 patients with fungal keratitis.All the eyes were examined by the slit lamp,followed by laser scanning confocal microscope,and then the 10% KOH corneal smear was examined under the optical microscope.Results The positive rate of diagnosis was 75% (125/167) by corneal scraping,and that by laser scanning confocal microscopy was 91% (152/167).The positive rate of examining outcome was significantly higher in laser scanning confocal microscopy than that of corneal scraping (x2 =14.88,P =0.00).The positive results were 114 cases and negative results were 4 cases by two methods,with the concordance rate 70.7% (118/167).The hyphae or spore were seen in 32 cases by laser scanning confocal microscopy in 42 negative cases by corneal scraping,and in 15 negative cases by confocal laser scanning microscopy,11 positive outcomes were offered by corneal scraping.Conclusions The combined application of corneal scraping with confocal laser scanning microscopy can improve and speed up the diagnosis positive rate of fungal keratitis.
3. Research on mechanism of opioid receptors in pharmacological effects of ketamine

Ketamine has been used clinically as an analgesic and anesthetic since 1970. Recently, it has been concerned due to its rapid and effective antidepressant effect. However, the psychiatric adverse reactions caused by ketamine such as addiction and hallucination limit its clinical application, yet the mechanism of the adverse reactions is still unclear. At present, it is generally believed that the pharmacological effects of ketamine are mainly mediated by N-methyl-D-aspartate acid (NMDA) receptors, but increasing numbers of research evidences suggest that the opioid receptor also play an important role in the pharmacological effects of ketamine. This review, based on the relevant literature published publicly in the past 20 years, summarizes the mechanisms of opioid receptors in the pharmacological effects of ketamine, such as anesthesia, analgesia, anti-depressant, anti-addiction and addiction, providing reference for revealing the mechanism of ketamine, and making beneficial exploration for solving the clinical side effects of ketamine.
4.Randomized controlled trail on moxibustion for maintenance hemodialysis patients in deficiency syndrome.

OBJECTIVETo explore the effective therapy for maintenance hemodialysis patients in dificiency syndrome in end-stage renal disease.
METHODSNinety-seven cases were divided into an observation group (51 cases) and a control group (46 cases) randomly, and routine western medicine was used in both of them. On this base, moxibustion was used in Zusanli (ST 36) and Sanyinjiao (SP 6) in paper-tube-moxibustion equipment in the observation group. Evaluate the therapeutic effect on symptoms by comparing the symptom scores in two groups before and after treatment.
RESULTSAll the symptom scores in the observation group were improved after treatment, and the differences were significant (all P < 0.05). Among all symptoms, the most improved ones included lassitude and fatigue, short breath and aversion to talk, poor appetite, soreness and softness of waist and knees, aversion to cold, cold extremities and so on. In the aspect of therapeutic effect on symptoms, the total effective rate in observation group (64.7%, 33/51) was higher than that in control group (23.9%, 11/46), and the difference was significant (P < 0.05).
CONCLUSIONMoxibustion can improve the clinical symptoms of maintenance hemodialysis patients in end-stage renal disease, and can generate some therapeutic effect to the dificiency syndrome of this disease.

5.Effects of heparanase inhibition by RNA interference on proliferation, invasiveness and apoptosis of lung cancer cells.

OBJECTIVETo investigate the effects of heparanase expression inhibition on the proliferation, invasiveness and apoptosis of human lung adenocarcinoma cell line A549 cells.
METHODSRecombinant eukaryotic expression plasmid pshRNA-Hpa targeting human heparanase gene was constructed. A549 cells were cultured in DMEM and transfected with pshRNA-Hpa. The expression of heparanase mRNA and protein were examined by RT-PCR and Western blot. The proliferation, invasiveness and apoptotic rates of A549 cells were determined by MTT method, matrigel invasion assays and flow cytometry respectively.
RESULTSThe expression levels of heparanase mRNA and protein were down-regulated in A549 transfected with pshRNA-Hpa. The number of cells penetrating matrigel and the proliferation ability of A549 cells transfected with pshRNA-Hpa were reduced significantly compared to the control cells. The apoptotic rate of A549 cells transfected with pshRNA-Hpa was 12.53% +/- 0.34%, being significantly higher than that of the control cells (both P < 0.01). Western-blot showed that inhibition of heparanase expression led to reduced Akt phosphorylation.
CONCLUSIONSThe recombinant plasmid pshRNA-Hpa effectively inhibited the expression of heparanase, thus suppressing the proliferation and invasion and inducing apoptosis of A549 cells. The effects may be due to the down-regulation of Akt phosphorylation level.

6.Improvement of survival quality of the patients with hemodialysis treated with moxibustion for regulating spleen and stomach functions: multi-central randomized controlled study.

OBJECTIVETo discuss the impacts of moxibustion for regulating spleen and stomach function on the survival quality of the patients of end stage renal disease (ESRD) with maintenance hemodialysis (MHD).
METHODSOne hundred and nine cases of uremia with MHD from 3 hemodialysis centers were randomized into an observation group (58 cases) and a control group (51 cases). The regular hemodialysis and conventional medication were used in the two groups. In the observation group, on the basis of the common treatment, moxibustion was applied to Zusanli (ST 36) and Sanyinjiao (SP 6), 2-3 times a day, the treatment of 4 weeks made one session. Totally, 3 sessions were required and the follow-up lasted for 3 months. KDQOL-SF (kidney disease quality of life short form,KDQOL-SFTM 1. 3) was adopted for the questionnaire investigation on survival quality before treatment, after treatment and at the end of follow-up separately in the two groups.
RESULTSAfter treatment, the survival quality scores in terms of physical functioning (83.62+/-13.27 vs 79.32+/- 22. 17), general health (58. 88+/- 20.24 vs 48.82+/-20.89) and vitality (77.07+/-15.56 vs 70. 59+/-22.61) in the observation group were higher than those in the control group (all P<0. 05). In comparison before and after treatment in the same group, the survival quality scores in terms of physical functioning, general health, vitality and symptoms/problems were all improved in the observation group (all P<0. 05). At the end of follow-up, the survival quality scores in terms of physical functioning, general health, mental health, social functioning, vitality, effects of kidney disease and cognitive function were higher in the observation group as compared with those in the control group (all P<0. 05). In comparison of the results at the end of follow-up with those before treatment, the survival quality scores in terms of vitality, symptoms/problems and cognitive function in the observation group were improved (all P< 0. 05). The differences were not significant in all of the 19 fields of survival quality evaluation before and after treatment, and after follow-up in the control group (all P>0. 05).
CONCLUSIONMoxibustion for regulating spleen and stomach function improves the survival quality of the patients with hemodialysis in terms of physical functioning, general health and vitality, which benefits the psychological condition of the patients, resulting in the improvements of the survival quality in the fields of mental health, social functioning, effects of kidney disease and cognitive function.

7.Association of urinary albumin excretion rate and hyperuricemia with macrovascular atherosclerosis in type 2 diabetic patients.

OBJECTIVETo investigate the association of urinary albumin excretion rate (UAER) and hyperuricemia with macrovascular atherosclerosis in type 2 diabetic patients.
METHODSNinety-seven type 2 diabetic patients were divided into two groups according to the UAER, namely group A with UAER between 20 and 200 microg/min (n=63) and group B with UAER > or = 200 microg/min (n=34); the patients were also classified into hyperuricemia group (group C, n=59) and normal blood uric acid (BUA) group (group D, n=38). The disease course, BUA, fasting blood glucose (FBG), triglyceride (TG), total cholesterol (TC), low-density lipoprotein (LDL), high-density lipoproteins (HDL), UAER and arteria carotis intima-media thickness (IMT) were determined in these patients. The relationship of UAER and hyperuricemia with carotid arterial IMT was analyzed statistically.
RESULTSThe levels of TG, TC, LDL and HDL showed no significant differences between the 4 groups (P>0.05). The disease course, BUA, UAER, and FBG levels and IMT in groups A and C were significantly higher than those in groups C and D (P<0.05), but no such differences were found between groups A and C or between groups B and D (P>0.05). Arotid arterial IMT was independently correlated to the disease course, BUA and UAER (r=0.201, 0.1999, 0.211, respectively, P<0.05), and a significant positive correlation was noted between BUA and UAER (r=0.221, P<0.05).
CONCLUSIONMacrovascular atherosclerosis in type 2 diabetic patients is significantly correlated to the disease course, BUA and UAER levels, which can be used to evaluate and predict macrovascular atherosclerosis in type 2 diabetic patients.

8.Effects of zinc on biological behavior of hepatocellular carcinoma cell line BEL-7404

AIM:To observe the effects of exogenous zinc on the biological behavior of hepatocellular carcino -ma ( HCC) cell line BEL-7404.METHODS: BEL-7404 cells were cultured with zinc sulfate at various concentrations . The intracellular concentration of zinc , cell viability , cell cycle , cell apoptosis and migration and invasion abilities were measured by TSQ fluorescent probe , MTT assay, DNA ploid analysis, acridine orange/ethidium bromide fluorescence stai-ning and Transwell assay , respectively .The mRNA and protein expression levels of albumin in the BEL-7404 cells were determined by real-time PCR and Western blot , respectively .RESULTS: With the elevated concentration of zinc in cul-ture condition, the concentration of zinc in the BEL-7404 cells was increased (P<0.05).The cell viability and migration and invasion abilities were decreased , while the apoptotic rate was increased (P<0.05).The cells in G0/G1 phase were decreased, while the cells in G2/M phase were increased.Additionally, the mRNA and protein expression of albumin also increased (P<0.05).CONCLUSION:The zinc ion inhibits the cell viability as well as migration and invasion abilities , blocks the cells in G 2/M phase , and may reduce cell malignant phenotype .
9.A survey on the quality of life among disabled people in Hangzhou City

Objective To learn the quality of life for people with disabilities in Hangzhou City, and to provide the basisfor targeted interventions. Methods Using the health survey list (SF-36) and the self-compiled population learningvolume, 350 disabled people were investigated face to face in Xiaoshan, Jiande and Fuyang District of Hangzhou City,and the quality of life of different disabled people with demographic characteristics were compared. Results A total of 335disabled people were investigated. The SF-36 score of the 335 disabled people was (62.36±18.40) . Compared with thehealthy people, the scores were lower in the survey of 335 people with disabilities(P<0.001) . The scores of the maledisabled in VT, MH, and PF were lower than the female. In terms of PF, RP, BP, scores of people above 48 years oldwere lower than people under the age of 48 years old, and in terms of mental health, scores of people above 48 years oldwere higher than people under the age of 48 years old. Score in physiological function of the physical disabled people waslower than other types of disabled people(MD=-19.447, -18.509, -19.792, P<0.008) . In terms of PF, score ofgrade 2 disabled people was lower than grade 1 and 4 disabled people (MD=-17.925, -18.162, P <0.008) .Conclusion The quality of life of disabled people in Hangzhou is low, which is affected by gender, age, and disabilitytype and disability grade.
10.Study on the pharmacokinetics and cardiac distribution of tetramethylpyrazine hydrochloride in mice

Objective To investigate the pharmacokinetics and cardiac distribution of tetramethylpyrazine ( TMP ) hydrochloride in mice by re-versed phase high performance liquid chromatography ( RP -HPLC ) method.Methods The plasma and heart homogenate of mice were trea-ted with acetonitrile and a RP -HPLC method was developed.The 66 mice were injected intraperitoneally with a single dose of 80 mg? kg -1 . The concentration of TMP in plasma and heart was determined .The pa-rameters of the pharmacokinetics were calculated .Results This RP-HPLC method had a good linearity , recovery rate and precision.The standard curve of TMP was linear in the range of 0.2-20.0 mg? L-1 ( r=0.999 7 ).The major parameters of TMP were as follow: AUC0-t was (671.20 ±31.49) mg? min? L-1,Cmax was(17.54 ±0.71) mg? L-1, tmax was 10 min, t1/2 was (25.47 ±14.85 ) min.Conclusion A sim-ple, accurate and sensitivity RP-HPLC method is developed and valida-ted for the determination of TMP concentration in plasma and heart of mice.TMP has a rapid absorption and higher concentration in the heart , which was beneficial for the treatment of heart disease .
