OBJECTIVE:To observe the clinical efficacy of acupoint application of antiemetic cream in the treatment of che-motherapy-induced nausea and vomiting. METHODS:90 cases of non-small cell lung cancer(NSCLC)(stage Ⅱ to Ⅲ)treated by chemotherapy(taxol+cisplatin)were randomly divided into antiemetic cream group,Weifuan group and control group, with 30 cas-es in each groupby drawing lots. Control group was given intravenous injection of ondansetron hydrochloride 8 mg 30 min before chemotherapy;antiemetic cream group was additionally given acupoint application of antiemetic cream 30 min before medication on first-forth day on the basis of control group;Weifuan group was additionally given acupoint injection of metoclopramide(brand name:Weifuan) 30 min before medication on first-forth day on the basis of control group. The clinical efficacy of 3 groups were observed during and 3 days after chemotherapy. RESULTS:After chemotherapy, the total control rates of antiemetic cream group (93.33%)and Weifuan group(90.00%)were apparently higher than the control group(66.67%),with statistical significance(P<0.01 or P<0.05);The total effective rates of TCM syndrome in antiemetic cream group (90.00%) and Weifuan group(83.33%) were apparently higher than in control group(60.00%),with statistical significance(P<0.01 or P<0.05). Therapeutic efficacy of antiemetic cream group was similar to Weifuan group, without any statistical significance(P>0.05). However,8 patients had obvi-ously pain at the injection site in Weifuan group, but tolerated to the drugs basically. CONCLUSIONS:The acupoint application of antiemetic cream combined with intravenous injection of ondansetron is effective and easy to operate in the treatment of nausea and vomiting induced by chemotherapy for stage Ⅱ to Ⅲ of NSCLC without pain, scar and sequela, and easy to be accepted by patients.

Acupuncture clinical trials are designed to provide reliable evidence of clinical efficacy, and SCI papers is one of the high-quality clinical efficacy of acupuncture research. To analyze these papers published in high impact factor journals on acupuncture clinical trials, we can study clinical trials from design to implementation, the efficacy of prevention and cure, combined with international standard practices to evaluate the effectiveness and safety of acupuncture. That is the core of acupuncture clinical trials, as well as a prerequisite for outstanding academic output. A scientific and complete acupuncture clinical trial should be topically novel, designed innovative, logically clear, linguistically refining, and the most important point lies in a great discovery and solving the pragmatic problem. All of these are critical points of papers to be published in high impact factor journal, and directly affect international evaluation and promotion of acupuncture.

Abstract: Objective To explore the correlation between the levels of silent information regulator 1 (SIRT1) and forkhead box protein O3 (FOXO3) in peripheral blood mononuclear cells of patients with active pulmonary tuberculosis (APTB) and macrophage-related cytokines-inducible nitric oxide synthase (iNOS) and arginase-1 (Arg-1). Methods A total of 64 APTB patients who were treated in Yubei Hospital, the First Affiliated Hospital of Chongqing Medical University from January 2020 to December 2021 were gathered as the APTB group, 59 people with latent tuberculosis infection (LTBI) were gathered as the LTBI group, and 62 healthy people were gathered as the control group. Quantitative real-time PCR (qPCR) method was performed to measure the levels of SIRT1 mRNA and FOXO3 mRNA in peripheral blood mononuclear cells. The enzyme-linked immunosorbent assay (ELISA) was performed to measure serum iNOS and Arg-1 levels; ROC curve was used to analyze the value of SIRT1 mRNA and FOXO3 mRNA levels in the differential diagnosis of LTBI and APTB; Pearson correlation was performed to analyze the correlation of SIRT1 mRNA and FOXO3 mRNA in peripheral blood mononuclear cells of APTB patients with serum iNOS and Arg-1 levels. Results The levels of SIRT1 mRNA, FOXO3 mRNA and serum iNOS in peripheral blood mononuclear cells decreased in control group, LTBI group and APTB group, and the level of serum Arg-1 increased in turn (P<0.05). The AUCs of SIRT1 mRNA and FOXO3 mRNA in differential diagnosis of LTBI and APTB were 0.876 and 0.887, respectively, the sensitivity was 71.2% and 76.3%, and the specificity was 96.9% and 90.6% respectively. The levels of SIRT1 mRNA and FOXO3 mRNA in peripheral blood mononuclear cells of APTB patients were positively correlated (r=0.500, P<0.05), and they were positively correlated with serum iNOS and negatively correlated with serum Arg-1 (P<0.05). The SIRT1 mRNA, FOXO3 mRNA and serum iNOS in peripheral blood mononuclear cells of APTB patients after 6 months of treatment were higher than those before treatment, and serum Arg-1 was lower than before treatment (P<0.05). Conclusions The levels of SIRT1 mRNA and FOXO3 mRNA in peripheral blood mononuclear cells of APTB patients are low, and they are positively correlated with macrophage-related cytokine iNOS and negatively correlated with Arg-1.
