1.Malaria surveillance in Shanghai from 2005 to 2012

Objective To understand the status of malaria prevalence in Shanghai,so as to provide the evidence for evaluat-ing and promoting malaria elimination. Methods The data of population,malaria endemic and monitoring in Shanghai from 2005 to 2012 were collected and analyzed. The malaria positive rates in febrile patients and normal population,the malaria antibody lev-els of the floating population and location pupils,and the species and density of mosquitoes were investigated and the results were analyzed. Results The blood examples from 400 177 febrile patients with 0.36%of the total population were examined for malar-ia,and the annual rates of blood examination varied from 0.15%to 0.83%in the period of 2005 to 2012. Before 2010,the annual blood examination rate was lower than 0.20%,but increased significantly since 2010 because of the implementation of the Action of Malaria Elimination in Shanghai from 2010 to 2020. Totally 604 malaria cases were found and the positive rate of blood examina-tion was 0.15%. During the period of 2006 to 2008,more than 100 malaria cases were found every year,but the number of malaria cases decreased since 2009,and only 26 cases was found in 2012. Since 2011,all the malaria cases have been found infected out of China according to the questionnaire investigation. The malaria positive rate of blood examinations decreased from 0.61% to 0.03%yearly. The proportion of local residents who had the blood examination was 79.02%,but that of the malaria positive in float-ing population was 83.44%. The active case detection was carried out in 15 759 persons and five cases were found in 2007 and 2008,respectively. The level of malaria antibody was tested in 1 239 440 floating population with the average positive rate of 0.04%. The level of malaria antibody was also tested in 7 065 local pupils but there was no positive for malaria. Only Anopheles si-nensis was found in Shanghai suburb,but no other Anopheles mosquitoes were found at the city or towns. The peak of Anopheles den- sity was from the end of July to August. Conclusions Malaria surveillance is carried out efficiently in Shanghai,and the result shows Shanghai is in the phase of malaria elimination. The more attention should be paid to persons coming from malaria endemic areas for the malaria blood examination and the general and advanced hospital should maintain the ability of malaria blood exami-nations simultaneously.
2.Application of palliative care led by oncology professional nurses in the elderly patients with late tumor

Objective To explore the effect of palliative care led by oncology professional nurses in the elderly patients with late tumor. Methods Ninety patients who were diagnosed with late tumor were randomly divided into control group and intervention group. Routine nursing care was conducted in the control group and in the intervention group, palliative care lead by professional nurses was conducted besides the routine nursing care for 8 weeks. The two groups were compared in view of life, symptom distress, and self-efficacy intervention effect. Result The quality of life and self-efficacy were significantly higher than those of the control group (P<0.05) and the symptom distress was significantly lower than that of the control group eight weeks after the intervention (P<0.05). Conclusion The multi-disciplinary intervention led by the oncology professional nurses can improve the quality of life and self-efficacy of the patient and improve the symptoms of the patient.
4.Enzymatic Synthesis of D-D4FC Using Intact Cells

D-D4FC (?-D-2′,3′-didehydro-2′,3′-dideoxy-5-fluorocytidine),a new anti-HIV drug,is on its PhaseⅡ clinical trials in America,France and Germany. Our lab has synthesized D-D4FC successfully using N-deoxyribosyltransferase from Lactobacillus helveticto catalyzing the ribose transfer from D4T (?-D-2′,3′-unsaturated thymidine) to 5-FC (5-fluorocytidine).The yield of D-D4FC reached 25%.We discovered the reaction could also be done by using intact cells.The yield could increase to 50% in 12.5 hours and more convenient to industrial continuous process.In this paper,the conditions including pH,buffer,substrates concentration,cells amount,reaction time and a possible catalytic mechanism were studied and discussed.
6.Expression and significance of heat shock protein 70 in maternal serum, umbilical cord blood and placenta of patients with hypertensive disorders complicating pregnancy

Objective To investigate the expression of heat shock protein 70 (HSP70) in maternal serum,umbilical cord blood and placenta of patients with hypertensive disorders complicating pregnancy (HDCP) and to discuss its role in the pathogenesis of HDCP.Methods Totally 90 patients with HDCP were recruited as HDCP group,and were devided into three subgroups,including gestational hypertension group (30 cases),mild preeclampsia group(30 cases) and severe preeclampsia group(30 cases).A totally of 30 cases of healthy pregnant women were defined as the control group.All of them were admitted to Xuzhou Maternity and Child Health Care Hospital from August 2011 to December 2012.ELISA was used to detect the expression of HSP70 in maternal serum and umbilical cord blood.Immunohistochemistry streptavidin peroxidase(SP) was used to detect the protein in placenta,and semi-quantitative reverse transcription (RT)-PCR was used to detect the expression of HSP70 mRNA.Results (1) The levels of HSP70 in maternal serum and cord blood of mild preeclampsia group were (2.61±0.98) and (0.78±0.27)μg/L,respectively; and were (3.10± 1.18) and (0.96±0.28)μg/L in severe preeclampsia group.The levels of HSP70 in mild and severe preeclampsia groups were significantly higher than those in the control group [(1.88±0.79) and (0.61±0.15) μg/L,respectively] and gestational hypertension group [(2.13 ± 0.71) and (0.64 ± 0.18) μg/L,respectively;P<0.05].The level of HSP70 in severe preeclampsia group was significantly higher than that in mild preeclampsia group (P<0.05).And the level of HSP70 in gestational hypertension group was higher than that in the control group,but there was no statistical difference (P>0.05).(2)The positive rate of placental HSP70 in gestational hypertension group,mild and severe preeclampsia group [83%(25/30),90% (27/30) and 100%(30/30)],respectively were significantly higher than those in the control group(43%,13/30;P<0.05).The positive rate of placental HSP70 in severe preeclampsia group was significantly higher than that in gestational hypertension group and mild preeclampsia group(P<0.05).(3)The expression of placental HSP70 mRNA in gestational hypertension group,mild and severe preeclampsia group (0.82±0.27,0.92± 0.26 and 1.36±0.29,respectively) were significantly higher than that in the control group (0.45±0.18),with statistically significant differences (P<0.05).The expression of placental HSP70 mRNA in severe preeclampsia group was significantly higher than that in gestational hypertension group and mild preeclampsia group(P<0.05).Conclusion The expression of HSP70 increased significantly in maternal serum,umbilical cord blood and placenta of patients with HDCP,and it had positive correlation with the severity of the disease,indicating that HSP70 may play a role in the pathogenesis of HDCP.
7.Relationship between tumor necrosis factor-αgene-308 promoter polymorphism and susceptibility to development of bronchial asthma in Uighur and Kazakh population in Xinjiang

Objective To investigate the relationship between the polymorphism of the tumor necrosis factor-αgene-308 promoter and bronchial asthma (BA)in Uighur and Kazakh population in Xinjiang.Methods PCR product sequencing method was used to detect the polymorphism distribution of TNF-αgene in 60 BA patients and 60 controls among Xinjiang Uighur and Kazakh population.Then we analyzed the association between different TNF-αgenotypes and BA in Uighur and Kazakh population.Results The distribution of GG,GA and AA genotype was significantly different between the two groups.Statistical analysis showed that BA group had a significantly higher TNF-αA allele frequency than that of the control one (P <0.05).After adjustment for sex and age,we found the A allele was a risk factor for BA pathogenesis (P <0.05).Conclusion The polymorphism of TNF-α gene-308 may be associated with the susceptibility to BA in Xinjiang Uighur and Kazakh population,so TNF-α gene-308 may be considered as a genetic marker for early identification of individuals at high risk for BA,which may play an important role in preventing the development of BA in clinical practice.
8.Factors for the recurrence after endoscopic resection of early gastric cancer

Objective To investigate the related factors of recurrence of early gastric cancer (EGC) after endoscopic resection. Methods Clinicopathologic data of 169 patients with EGC who underwent endoscopic resection and periodically followed up by the Chinese PLA General hospital were analyzed retrospectively. Results During a follow-up of 13-57 months (median time 24. 5 months), 12patients had gastric cancer again and the recurrence rate was 7. 1% (12/169). The recurrence time varied from 3 to 36 (28 ± 23)months and the median time was 18 months. The recurrence rates of 0.5 year, 1st year, 2nd year and 3rd year were 1.18% (2/169), 3.55% (6/169), 9.91% (11/111) and 12.24%(12/98), respectively. Eleven patients had gastric cancer again within 2 years after resection.Undifferentiated histology (including poorly differentiated carcinoma and signet ring cell carcinoma),submucosal infiltration and lymphovascular invasion of the primary lesion of EGC were related to thepostsurgical recurrence ( all P < 0. 05). Conclusion Most recurrence of EGC occurred within 2 years afterendoscopic resecton and is related with undifferentiated histology, submucosal infiltration andlymphovascular invasion. It is important for these patients to receive endoscopy follow up.
9.Relationship between ambulatory arterial stiffness index and extent of coronary atherosclerosis

Objective To study the relationship between ambulatory arterial stiffness index and extent of coronary atherosclerosis.Methods All subjects were underwent coronary CT angiography.Ambulatory arterial stiffness index(AASI) was measured before coronary CT angiography.AASI value between different periods in each group were calculated and compared.Results AASI between different periods in the normal control group and coronary heart disease group existed statistical difference (P < 0.05).The extent of coronary sclerosis and AASI between different periods in different coronary artery lesions groups were statistically different(P <0.05),AASI was proportional to the degree of coronary sclerosis,24 h AASI,DAASI,NAA SI were related to extent of coronary atherosclerosis lesions,the correlation coefficient between NAASI and extent of coronary atherosclerosis was the largest.Conclusion AASI night is related to coronary atherosclerosis most closely,namely the higher the value of the nighttime AASI,the greater the degree of coronary atherosclerosis.
10.Initial exploration of clinical value for diagnosing in tracranial aneurysms with multi-slice helical CT three-dimensional angiography

Objective To evaluate the clin ical value of Multi-slice helical CT three dimensional angiography (3D-MSCTA) as first method for diagnosing intracr anial aneurysms. Methods We studied patients with clinical suspected intracranial aneurysms (13 patients with subarachnoid hemorrhage among cases). All these patients under went 3D-MSCTA and Digital Subtraction Angiography (DSA), 16 patients of them accepte d operation treatment. Row data was acquired by Multi-slice helical CT-AQUILION (Toshiba): scan speed 0.5 s/rot, image slice thickness 1.0 mm, helical pitch 3 .0/5.0. Contrast media (Angiografin) was injected intravenously (1.0-2.0 ml/kg) at spee d of 2.5-3.0 ml/s, delay time was 15-23 sec, reconstruction interval 0.5 mm, recons truction slice thickness 1.0 mm. Source images were processed using a workstation SGI-O2 , images post-processing software was ALATOVIEW,Version 1.21. The reconstructed images were then processed into shaded volume rendering (SVR) and maximal intensity projection (M IP) and Fly-through images. Entire brain DSA was performed obtaining anterioposteri or, lateral, and oblique images. Images of 3D-MSCTA and DSA were analysed by 3 radiologists and 2 neurosurgeons. Results 25 aneurysms were d etected by 3D-MSCTA. Aneurysms′s body, neck, source vessel and the relationship between the aneurysm and surrounding structures was clearly and surely displayed. 22 of 25 aneurysms were detected by DSA,another 3 were (1 anterior communicating artery aneurysm and 2 left middle cerebral artery aneurysm) was not detected. Sixteen of patients un derwent operation treatment, and the results of 3D-MSCTA corresponded very well to those of operation. Maximal diameter of aneurysms body was 14.0 mm and minimal diameter w as 1.7 mm. Conclusion 3D-MSCTA is a high sensitivity and rapid and no ninvasive method for detecting intracranial aneurysms. We suggest that 3D-MSCTA may be the first cho ice for diagnosing intracranial aneurysms.
