1.Protective effects of 3-methyladenine on acute lung injury caused by multiple trauma in rats.

OBJECTIVETo investigate the role of autophagy in acute lung injury (ALI) caused by multiple trauma in rats via pretreat with 3-methyladenine (3-MA).
METHODSForty-five Sprague-Dawley male rats, with age of 4 months and body weight of 250-300 g,were randomly divided into three groups. In the sham group, the rats received sphenotresia only;in the control group, the rats were made model of femur shaft fracture combined with brain injury, and treated with physiological saline by abdominal cavity at 1 hour before making model; in the 3-MA group, the rats were made model of femur shaft fracture combined with brain injury,and treated with 3-MA of 10 mg/kg by abdominal cavity at 1 hour before making model. Histologic changes and the concentration of related inflammatory factors in the damaged lung tissue were examined at 48 h after opteration, at the same time, the effect of 3-MA on the expression of LC-3 II and Beclin-1 was examined through reverse transcriptase polymerase chain reaction technique (RT-PCR).
RESULTSCompared with sham group, LC-3 II and Beclin-1 level in control group at 48 h after operation were obviously increased (P < 0.01). Compared with control group, LC-3 II and Beclin-1 level in 3-MA group at 48 h after operation were obviously decreased (P < 0.01). Compared with sham group, the level of proinflammatory cytokines (TNF-α and IL-6) in control group obviously enhanced (P < 0.01). Compared with control group, above items in 3-MA group was obviously lower (P < 0.01). Compared with control group,the histopathological damage of lung in 3-MA group obviously reduced (P < 0.01).
CONCLUSIONAutophagy can aggravate the acute lung injury caused by fracture of shaft of femur combined with brain injuries,but 3-MA can reduce tissue damage by inhibiting the autophagy and inflammatory response.

3.Propofol in aged patient the application in painful gastroscope inspection

Objective Discuss propofol in aged patient do not have painful gastroscope inspection in calm ease pain effect and safety.Methods Inspect gastroseope divide into observation group(propofol group) with compare group(conventional gastroscope group).Observe each group of record to receive the gastroscope operating time.nar- cotic thing role time and reaction in skill,before inspecting in rear BP,HR,SpO_2 and skill rear questionnaire investi- gation.Results The case owned is inspected all consummately,observe group to have negative role less,patient is more easy to accept.Conclusion It is rapid to have effect in propofol inspects without painful gastroscope,recovery time short,negative role lose,is comfortable and safe.
4.Transosseous osteosynthesis with annular external fixator for the treatment of long bone defect after tibial traumatic.

OBJECTIVETo investigate the efficacy of the application of transosseous osteosynthesis with annular external fixator for the treatment of traumatic large bone defects of the tibia.
METHODSFrom 2002 to 2011,17 cases of traumatic tibial long bone defect were treated by transosseous osteosynthesis with annular external fixator including 11 males and 6 females with an average age of (31.00+/-2.35) years old ranging from 22 to 45 years. The length of defect was from 5 to 13.1 cm with an average of (7.50+/-1.01) cm. The fixed time,bone healing time and the extend length of the affected limb were observed, and the degree of recovery of limb function after surgery were assessed ccording to the Paley's limb function evaluation.
RESULTSAll patients were followed up from 12 to 48 months with an average of (30.00+/-2.35) months, the bone defects of 17 patients achieved bone healing. The average bone healing time was (10.10+/-0.59) months (8 to 15 months); the average time of fixed time was (14.10+/-0.65) months (12 to 16 months); the average extend length of the affected limb was (7.50+/-1.01) cm. The results of Paley's functional evaluation was excellent in 9 cases,good in 6,fair in 2.
CONCLUSIONTransosseous osteosynthesis with annular external fixator is an effective method for treatment of large bone defects of the tibia. It is successful reconstruction of limb length, simple operation, less soft tissue injury,and is clinical worthy of promotion.

5.Comparison of mini-probe endoscopic ultrasonography with computed tomography in preoperative staging of esophageal cancer.

OBJECTIVETo compare mini-probe endoscopic ultrasonography (MCUS) with computed tomography (CT) in preoperative T and N staging of esophageal cancer, and to find out the MCUS parameters to judge lymph node metastasis for esophageal cancer.
METHODSThirty-five patients received both MCUS and CT preoperatively, on both of which the T and N stages were determined. The accuracy, sensitivity, specificity, positive predicting value and negative predicting value were compared with the postoperative pathological results.
RESULTSThe accuracy of MCUS was 85.7% in T staging and 85.7% and 80.0% in N staging by the two different methods, which were 45.7% and 74.3%, respectively, by CT.
CONCLUSIONMCUS is better than CT in preoperative staging for esophageal cancer. The ratio of short to long axis (S/L) combined with short axis is a useful way to determine lymph node metastasis.

6.Minimally invasive transforaminal lumbar interbody fusion versus posterior open-surgery in treatment of lumbar spondylolisthesis

ObjectiveTo compare the clinical results between minimally invasive transforaminal lumbar(mini-TLIF) and posterior open surgery in treatment of lumbar spondylolisthesis.MethodsFrom March 2008 to August 2010,a total of 49 cases with lumbar spondylolisthesis underwent surgical intervention were retrospectively analyzed,including 23 cases with mini-TLIF and 26 with open surgery.Operation time,intra-operative bleeding,and radiation exposure times were recorded.Pre- and postoperative back pain was assessed by visual analogue scale(VAS),and lumbar function was evaluated by Oswestry disability index (ODI).The clinical results were assessed by Macnab criterion,and the pre and postoperative radiologic parameters were compared.ResultsThe mean follow-up time was 11 months(ranged,9-22).Both groups got good clinical results and satisfactory radiologic parameters.The group of mini-TLIF was superior to the group of open surgery in intra-operative bleeding,VAS of the second day postoperatively and the willingness of reoperation(P<0.05).The ODI in the patients with open surgery were decreased from 31.2%±8.2% to 16.1%±6.8% corresponding to the pre-oporation and the final follow-up.The ODI in the patients with mini-TLIF were decreased from 34.4%±11.7% to 15.3%±4.3% corresponding to the pre-operation and the final follow-up.There is no significant difference of the change of ODI between two groups (t=0.673,P=0.412).The group of mini-TLIF need more operation time and were exposed to more X-ray when compared to the open surgery group(P<0.05).ConclusionMini-TLIF and open surgery can both get satisfactory clinical outcomes in treatment of lumbar spondylolisthesis.Mini-TLIF was superior to open surgery in intra-operative bleeding and VAS of the second day postoperatively,but it needs more operation time and radiation exposure.
7.Efficacy of adaptive servoventilation in patients with congestive heart failure and Cheyne-Stokes respiration.

BACKGROUNDCongestive heart failure (CHF) is associated with Cheyne-Stokes respiration (CSR), which may hasten CHF. Adaptive servoventilation (ASV) is a novel method of ventilatory support designed for removal of CSF in CHF patients. This study compares the efficacy of ASV in patients with CHF and CSR with the efficacy of oxygen therapy.
METHODSFourteen patients with CHF and CSR were recruited. During sleep, nasal oxygen therapy and ASV treatment were each performed for two weeks. Comparison before and after each treatment was made for the following items: a) parameters of sleep respiration, sleep structure and quality; b) left ventricle ejection fraction (LVEF) and 6-minute walk distance.
RESULTSCompared with the baseline levels of apnoea hypopnoea index of 34.5 +/- 6.1 before treatment, the apnoea hypopnoea index significantly decreased following oxygen therapy to 27.8 +/- 8.2, P < 0.05 and further reduced following ASV treatment to 6.5 +/- 0.8, P < 0.01. The minimal pulse oxygen saturation markedly increased following oxygen therapy from a baseline of (84.3 +/- 2.6)% to (88.6 +/- 3.7)%, P < 0.05 and further increased following ASV treatment (92.1 +/- 4.9)%, P < 0.01. Stages I + II sleep as percentage of total sleep time decreased from (81.9 +/- 7.1)% to (78.4 +/- 6.7)% following oxygen therapy and further to (72.4 +/- 5.0)% following ASV treatment. Stages III + IV sleep as percentage of total sleep time decreased from (8.4 +/- 5.5)% to (6.0 +/- 3.0)% following oxygen therapy and but increased to (11.9 +/- 5.4)% following ASV treatment. The arousal index of 30.4 +/- 8.1 before treatment significantly decreased following oxygen therapy to 25.6 +/- 5.7, P < 0.05 and further declined following ASV treatment to 18.2 +/- 6.1, P < 0.01. No significant difference was shown in above percentages between day 14 of oxygen therapy and before treatment (P > 0.05). LVEF was significantly higher on day 14 of ASV treatment (37.2 +/- 4.1)% than on day 14 of oxygen therapy (33.2 +/- 5.1)% and before treatment (30.2 +/- 4.6)% (all P < 0.05). Six-minute walk distance was the shortest before treatment (226 +/- 28) m, longer on day 14 of oxygen therapy (289 +/- 26) m, and the longest on day 14 of ASV treatment (341 +/- 27) m (all P < 0.01).
CONCLUSIONASV treatment is of better efficacy and greater clinical significance in improvement of CHF by eliminating CSR than oxygen therapy.

8.Cancer incidence and mortality of children in urban districts of Guangzhou between 2000 and 2004.

BACKGROUND AND OBJECTIVECancer is one of the most primary causes of death for children. The study was to analyze the cancer incidence and mortality of children in urban districts of Guangzhou between 2000 and 2004, to explore the incidence regularity of pediatric cancers and to provide a reference for prevention and treatment of pediatric cancers.
METHODSThe data of cancer incidence and mortality of children during 2000-2004 were collected from Guangzhou Population-based Cancer Registry, and were calculated and analyzed.
RESULTSThe cancer incidence of children between 2000 and 2004 in Guangzhou was 17.91 per 100,000 (18.92 per 100,000 in males, 16.70 per 100,000 in females); the cancer mortality was 4.73 per 100,000 (4.65 per 100,000 in males, 4.83 per 100,000 in females). The incidence of lymphoid leukemia ranked first followed by cancer of central nervous system and non-Hodgkin's lymphoma. Cancer incidence was 77.52 per 100 000 in children of less than one year old, 21.49 per 100,000 in 1-4 years, 9.66 per 100,000 in 5-9 years and 17.11 per 100 000 in 10-14 years, with significant difference among the four groups (Chi(2) = 307.602, P < 0.001).
CONCLUSIONLymphoid leukemia, cancer of central nervous system and non-Hodgkin's lymphoma are the most common cancers in children. The children of 0-4 years old are the population with high cancer incidence.

9.Correlation analysis between nutritive components of Whitmania pigra and Bellamya purificata.

The dried Whitmania pigra is used for the treatment of cardiovascular and cerebrovascular diseases in traditional Chinese medicine. Bellamya purificata is widely distributed in the Chang Jiang River basin, it is natural diets of W. pigra. Current study was conducted to compare and analyze the nutritional ingredient in W. pigra, body fluid and flesh of B. purificata. Results showed that the contents of protein, crude fat and total sugar in W. pigra, body fluid and flesh of B. purificata were significantly different (P < 0.05). Protein content in W. pigra accounts up to 65.01%. The contents of inorganic elements and amino acid were abundant in W. pigra, body fluid and flesh of B. purificata. The content of essential amino acids in them were 32.6, 221.59, 40.78 mg x g(-1), respectively. The content of flavor amino acid in them were 27.51, 14.5, 32.03 mg x g(-1), while the coresponding content of antioxidant amino acid were 8.81, 5.91, 9.73 mg x g(-1), respectively. The individual amino acids of high content in them were Glu, Asp and Leu. Macro elements Ca, P, Mg and trace elements Zn, Si, Fe were abundant. It could be speculated that W. pigra may be a promising novel food, and the present results provide a foundation to develop artificial feed for W. Pigra.

10.Application of cumulative sum control chart algorithm in the detection of infectious disease outbreaks

In recent years, for improving the ability of early detection on infectious disease outbreak, many researchers study the disease outbreak detection algorithms, based on many disease surveillance data, expecting to detect the abnormal increasing and cluster of disease and symptom at an early stage by adopting appropriate algorithm. This paper introduces a cumulative sum control chart method, one of statistical process control algorithms widely used in foreign countries and describes its basic principle and characteristic, key points of design, typical examples in application of disease outbreak detection of cumulative sum method, with expect to provide reference for its application in studies of disease outbreak early warning in China.
