1.Character of drug targeting lymph nodes after regional injection of carboplatin-activated carbon in patients with breast cancer

Purpose: To examine the drug concentration in axillary nodes after regional injection of carboplatin-activated carbon suspension (CP-CH) in patients with breast cancer, and compared the result with what happened after regional injection of carboplatin solution ( CP-Sol). Methods: 32 patients with breast cancer were divided into 2 groups randomly with 16 patients in each group. One group received regional subcutaneous injection of CP-CH adjacent to the primary tumor. The contrast group received CP-Sol under the same conditions. Modified mammectomy was done 1, 12, 24, 72 hours later. The carboplatin concentration in axillary lymph nodes was examined by atom absorbing spectrometry. Results: At 1, 12, 24, 72 hours after injection of CP-CH, the carboplatin concentration in lymph nodes was 11.23?5. 66u,g/g, 26.40?11. 18u.g/g, 18.72?7. 14u.g/g, 15. 44?6. 92u,g/g, respectively. At 1, 12, 24 hours after injection of CP-Sol, the carboplatin concentration was 0. 24? 0. 06ug/g, 0. 13? 0. 08ug/g, 0. 12? 0. 04ug/g, respectively. 2.Effect of bag-1 expression on the occurrence of breast cancer and its 5-year survival rate

BACKGROUND: Bag-1 is a multifunctional and anti-apoptotic gene. Its anti-apoptotic ability is enhanced when binding to bcl-2 to form a complex.Now it is considered as a predictive biomarker for the early diagnosis of breast cancer. However, whether it is useful in the assessment of the prognosis of breast cancer is still elusive.OBJECTIVE: To explore the expression of bag-1 in breast cancer and its role for prognosis.DESIGN: A controlled study with breast cancer, benign breast tumor and normal breast tissues as subjects.SETTING: The Vascular and Endocrine Surgery Department of Xijing Hospital of the Fourth Military Medical University of Chinese PLA.MATERIALS:Totally 100 breast cancer specimens were obtained form May 1995 to May 2000. Ten benign breast tumor and 10 normal breast tissues were used as control. All the specimens were paraffin-embedded and came from the Pathological Department of Xijing Hospital Affiliated to Fourth Military Medical University of Chinese PLA.METHODS: Immunohostochemical strept avidin-biotin complex(SABC) method was adopted to detect bag-1 expression in these specimens.pression levels of bag-1.RESULTS: The positive expression rate of bag-1 in breast cancer (85%) was significantly different form those of benign breast tumor (10%) and normal breast (10%) (χ2= 29.98, P = 0.00). While the positive expression rates in breast cancer of different stages (stage Ⅰ, stage Ⅱ and stage Ⅲ ) were 88%, 82% and 88%, respectively, which has no significant difference (χ2 = 0. 61, P = 0.75) . In duct carcinoma, lobular carcinoma and special carcinoma, bag-1 positive expression rate was 86%, 85% and 80%,which was also no significantly different (χ2 =0.16, P =0.95). In the 94followed patients, the 5-year survival rate of positive bag-1 expression was 79% and that of negative bag-1 expression was just 9%. The difference was significant (χ2 = 0. 07, P = 0.04).CONCLUSION: High bag-1 expression exists in breast cancer and its level is not associated with the clinical stages or pathological types of the cancer.Therefore, bag-1 may be used as a predictive marker for the prognosis of breast cancer.
3.Treatment of Peripheral Arterial Extensive Occlusive Disease by One Stage Arterialization of Posterior Tibial Vein

Objective To investigate the effect of one stage arterialization of posterior tibial vein in treatment of peripheral arterial extensive occlusive disease.Methods Forty-six cases(56 limbs) of patients with peripheral arterial extensive occlusive disease were treated with one stage arterialization of posterior tibial vein.Results The symptom of pain disappeared right after one stage arterialization of posterior tibial vein in all patients.Skin temperature went up.The long-term results were satisfactory during the period of 3 months to 7 years follow-up,except two limbs were amputated and two limbs were reoperated with pedicle omental transplantation.Conclusion The technique of one stage arterialization of posterior tibial vein has advantages of one-stage procedure,various indications,little influence to venous return and rapid relief of ischemic symptoms.
4.Impact of ethyl pyruvate on the cognitive function after splenectomy in aged rats

Objective To investigate the impact of ethyl pyruvate (EP) on cognitive function and the expression of high mobility group box 1 (HMGB1) and receptor for advanced glycation end products (RAGE) after splenectomy in aged rats.Methods Eighty-four male aged Sprague-Dawley rats, 18 months old, weighing 500-600 g, were randomly divided into 4 groups (n=21 each) by random number table method: control group (group C), surgery group (group S) ethyl pyruvate group (group E) and solution without EP group (group R).Morris water maze test was performed to evaluate cognitive function 5 days before surgery and 1, 3, 7 days after surgery.Group E was injected with EP 40 mg/kg intrapertoneally after splenectomy, group S and group C were injected with equivalent normal saline after splenectomy, group R was injected with equivalent solution without EP.Rats were killed after Morris water maze test, and the expression of HMGB1 and RAGE protein and mRNA in hippocampus were measured by Western blot and RT-PCR methods.Results Compared with group C, the escape latency and swimming distance were significantly prolonged in groups S, E and R 1 and 3 days after surgery, as well as the expression of HMGB1 and RAGE in hippocampus were significantly up-regulated (P<0.05).Compared with group S, the escape latency and swimming distance were significantly decreased and the expression of HMGB1 and RAGE were down-regulated in group E 1 and 3 days after surgery (P<0.05).Compared with the preoperative group, the escape latency and swimming distance were significantly prolonged in groups S, E and R 1 and 3 days after surgery (P<0.05).Conclusion EP may improve cognitive function in aged rats by down regulating the expression of HMGB1 and RAGE in the hippocampus.
5.Application of MR Fat Saturation Sequence in Acute Spinal Trauma

Objective To further study the value of MR T2WI fat saturation sequence in acute Spinal Trauma. Methods 109 cases of acute Spinal Trauma underwent MR imaging were analysed retrospectively. The detection rate of lesions with bone marrow edema, Vertebral fracture and spinal cord injury was compared in T2WI and FS T2WI. Results FS T2WI could detect more lesions of obscure fracture and bone marrow edema than that in T2WI,and it could accurately show the extent and the feature of anatomic structure in acute injury of bone. Conclusion FS T2WI can improve the diagnostic sensition and accuracy in Spinal Trauma, and it should be used as a routine MR sequence for detecting Spinal Trauma.
7.Correlation between CD34+ cell and immature granulocyte in donor's peripheral blood by rhG-CSF

Objective To investigate the correlation between immature granulocyte and CD34+ cells, mononuclear cells (MNC) in donor's peripheral blood by recombinant human granulocyte colony-stimulating factor (rhG-CSF). Methods The stem cell were mobilized by rhG-CSF 7.25-10 μg·kg-1·d-1 from 122 allo-PBSCT donors. Before and after mobilization, to test CD34+ cells of peripheral blood stem cell graft and number of MNC, immature granulocyte, CD34+ cell per patient' weight were calculated. Results White blood cell count and immature granulocyte gradually increased, and reached the peak on the frith day. There was a good relationship between increased immature granulocytes and increased CD34+ cells. The patients all achieved completed donor engraftment and achieved hematopoietic recovery. The chromosome, blood type and HLA type were transformed to be donor's type. Ph1 changed to be negative in CML patients. Conclusion rhG-CSF (7.25~10 μg·kg-1·d-1) had a good effect to mobilize PBSC. There was a good relationship between in-creased immature granulocytes and increased CD34+ cells after mobilization by rhG-CSF. The number of immature granulocytes can reflect indirectly the count of stem/progenitor, so the MNC and immature granulo-cytes can become a threshold of dosage standard.
8.Pharmacokinetic Studies of Sinomenine by Blood Microdialysis Technique

[Objective] To explore the feasibility and advantages of blood microdialysis technique for the pharmacokinetic study of sinomenine. [Methods] In this study, rats served as the experimental subjects and sinomenine was administered to the rats by intravenous injection. With microdialysis technique for blood sampling, the blood concentration of sinomenine was determined by high performance liquid chromatography (HPLC) and the associated pharmacokinetic parameters were analyzed by 3p97 program. [Results] The pharmacokinetics process of sinomenine in rats presented as the two-compartment model. The half-life of distribution phase was 10.98min and that of elimination phase was 44.71 min. [Conclusion] It is feasible to study pharmacokinetic parameters of sinomenine by using blood microdialysis technique. In comparison with the traditional technique, blood microdialysis technique is superior to the examination of drug blood concentration .
9.Effect of Medical Ethics on the Quality of Life and Treatment Compliance for Breast Cancer Patients

Objective:To analyze and discuss the influence of medical ethics on the quality of life and treatment compliance for breast cancer patients in clinical application, and to expound the positivehidden value of medical ethics to dealing with the doctor-patient relationship. Methods:176 cases of breast cancer patients in a hospital from December, 2012 to December, 2014 were selected to participate in this study. They were divided into experi-mental group (n=88) and control group (n=88) using the sequence number. The subjects in control group were treated according to the NCCN guidelines for breast cancer, while the experimental group was treated with medical ethics on the basis of the routine methods. The effect and safety were explored. Result:In evaluation of the quality of life, it was found that the excellent rate of patients in the experimental group was significantly higher than those in the control group (89. 8% vs. 68. 2%, P<0. 05). The incidence of complications in the study group was sig-nificantly lower than the control group ( P<0 . 05 ) . The complication incidence in the experimental group was sig-nificantly lower than that of the control group (73. 9% vs. 96. 6%,P<0. 05). Conclusion:Medical ethics has a great influence on the quality of life and treatment compliance for breast cancer patients, and its application has a positive indirect and clinical utility.
10.Establishment of nursing measures for patients with hemiplegia after stroke in various functions based on Nursing Interventions Classification

Objective To identify the nursing interventions and activities for hemiplegic patients in various functions suffered from stroke,and guide clinical practice.Methods Review rehabilitation of evidence-based clinical practice and literature of hemiplegic patients,stroke patients in different functional states were continuously cared and recorded by a researcher.Nursing program was identified according to patients' functions and the sixth edition of Nursing Interventions Classification,8 experts were consulted by a questionnaire survey,which was based on the initial screened nursing interventions and activities,30 nurses and 41 experts were consulted then.Results The established nursing program included 20 interventions and 307 activities.80%-100% nurses' opinions culminated in consensus on the selected interventions.The selected activities scored 4.00-5.00 in average,with the coefficient of variations ranging from 0.00 to 0.25.Conclusions The established nursing program is convenient to communication and could be a tool to guide the clinical nursing practice.
