Objective:To evaluate the characteristics of fusion using biomimetic n-HA/PA66 interbody cage with rhBMP-2 and discuss the feasibility of it.Methods:Thirty adult female goats underwent C_(3-4) discectomy and interverbral fusion performed with nHA/PA66 interbody cage without rhBMP-2(n=10).nHA/PA66 interbody cage with rhBMP-2(n=10),autogenous iliac crest bone graft(n=10).X ray was carried out at 4 and12 weeks after surgery to evaluate fusion quality.The goats were killed respectively on the 12th week.Nondestructive biomechanical testing for stiffness,un-decalcified histology were performed.Result:The goats in the n-HA/PA66 with rhBMP-2 group and the autogrft group all showed complete fusion at 12th week.there was no statisti- cally significant differience between the n-HA/PA66 with rhBMP-2 group and the autogrft group upon biomechanics,radiography and histomorphology.Conclusion:The interverbral fusion cage with n-HA/PA66 has good hiocompatibifity and osteoconductive ability and it may be good osteoionductive with rhBMP-2,so it was one ideal interbody fusion cage.
3. The mechanism and research progress of obeticholic acid in the intervention of non-alcoholic fatty liver disease

Non-alcoholic fatty liver disease(NAFLD)is a relatively common chronic liver disease with imbal- anced lipid metabolism, and its incidence is increasing year by year. NAFLD is closely related to bile acid excretion, ab- normal bile acid synthesis and liver fibrosis. Farnesolid X receptor(FXR)regulates bile acid transport, inhibits bile acid synthesis and resists liver fibrosis. Obeticholic acid is a targeted drug for the treatment of NAFLD targeting the farnesol X receptor, which can effectively regulate fat metabolism and blood sugar balance. This review summarizes the mechanism and research progress of obeticholic acid intervention in NAFLD, and provides a theoretical basis for its foundamental re- search and clinical application.
4.Application of aortic root internal drainage in the operation of type A aortic dissection

Objective To review the experience of aortic root internal drainage in the operation of type A aortic dissection.Methods This was a prospective and observational study.Clinical data were available from 20 cases of type A aortic dissection patients (A group) from March 2003 to March 2008,and anothcr 36 cases of type A aortic dissection patients (B group) from March 2008 to May 2013.All of patients received the operation of replacement of ascending aortic aneurysm and aortic arch replacement and descending aortic stented elephant trunk implantation in our hospital.The additional inside drainage were made between the aortic root aneurysm sac and right atrial appendage in B group.Perioperative and postoperative data including the operation time,the amount of bleeding,cardiopulmonary bypass time,the amount of red cell transfusion,number of postoperative cases re-operated for hemostasis,total drainage volume in 24 hours,amount of postoperative red blood cell transfusion,number of cases of postoperative pulmonary complications,and postoperative mortality rate were analyzed retrospectively.The data was analyzed using SPSS version10.0 software.The chi-square test was used for constituent ratios,whilest was applied to analysis of differences in above variables betweens two groups.Results There were significant differences in the amount of bleeding during surgery,cardiopulmonary bypass time,the amount of red cell transfusion,rate of postoperative re-operation,total drainage volume in 24 hours,amount of postoperative red blood cell transfusion,number of cases of postoperative pulmonary complications and postoperative mortality between the two groups (x2/t =2.658,2.381,2.265,3.056,6.862,2.896,2.316,7.215,7.668,P =0.012,0.034,0.007,0.016,0.013,0.032,0.008,0.008,respectively).Conclusions The method of aortic root internal drainage in the operation of type A aortic dissection could reduce the amount of bleeding and improve the success rate of operation.
5.Calcium phosphate cement with rhVEGF and rhBMP-2 promotes revascularization of big section bone allograft in dogs

Objective To explore the ability of revascularization of big section deproteinized bone (DPB) allograft combining calcium phosphate cement (CPC) with recombinant human vascular endothelial growth factor (rhVEGF) and bone morphogenetic protein-2 (rhBMP-2). Methods Totally 36 adult hybrid dogs were inflicted to demi-articular defect models by exsecting 30 mm right femur inferior extremity near articular surface,and then randomly divided into 3 groups, group A, receiving material CPC/rhVEGF/rhBMP-2/DPB transplantation into the demi-articular defect, group B, material rhVEGF/rhBMP-2/DPB transplantation, and group C with simple material DPB transplantation. At 4, 8, 12 and 16 weeks after operation, vasculogenesis and new bone formation were assessed by X-ray examination, histological examination, transmission electron microscopy, vas capillare analysis by vaso-filling with ink and radionuclide bone imaging examination (ECT). 7.Prognostic factors of low-and undifferentiated gastric cancer with negative metastasis of lymph nodes

Objective To investigate the prognostic factors of low-and undifferentiated gastric cancer with negative metastasis of lymph nodes.Methods The clinicopathological data of 270 patients with low-and undifferentiated gastric cancer and negative metastasis of lymph nodes who were admitted to the Tianjin Cancer Hospital from January 2002 to December 2007 were retrospectively analyzed.All the patients received radical gastrectomy.One hundred and sixty-one patients had the number of lymph nodes dissected under 15,53 patients had 15-20 lymph nodes dissected,33 had 21-30 lymph nodes dissected and 23 patients with the number of lymph nodes dissected more than 30.Patients were followed up via out-patient examination,phone call and mail till October 2013.The prognosis analysis was done by COX step-wise regression.The survival curve was drawn by KaplanMeier method,and the survival was analyzed using the Log-rank test.Results All the patients were followed up for a median time of 63 months (range,2-103 months).The median survival time was 63 months (range,2-103 months),and the 1-,3-,5-year overall survival rates were 93.0%,69.5% and 58.5%,respectively.The median survival time of the 161 patients with the number of lymph nodes dissected under 15 was 58 months (range,2-103 months),and the 1-,3-,5-year survival rates were 91.4%,59.3% and 48.8%,respectively.The median survival time of the 53 patients with the number of lymph nodes dissected between 15 and 20 was 68 months (range,4-95 months),and the 1-,3-,5-year survival rates were 94.3%,84.9% and 71.7%,respectively.The median survival time of the 33 patients with the number of lymph nodes dissected between 21 and 30 was 68 months (range,34-94 months),and the 1-,3-,5-year survival rates were 100.0%,97.0% and 87.9%,respectively.The median survival time of the 23 patients with the number of lymph nodes dissected more than 30 was 60 months (range,2-84 months),and the 1-,3-,5-year survival rates were 87.5%,62.5% and 54.2%,respectively.There was significant difference in the prognosis between the 4 groups (x2 =25.077,P < 0.05).There was significant difference in the prognosis between patients who had 21-30 lymph nodes resected and those had 15-20 lymph nodes resected (x2=3.924,P <0.05).Significant difference was also observed in the prognosis between patients who had 15-20 lymph nodes resected and those had more than 30 lymph nodes resected (x2=4.454,P < 0.05),while no significant difference was observed in the prognosis between patients who had lymph nodes resected more than 30 and those had lymph nodes resected less than 15 (x2=0.450,P >0.05).The results of univariate analysis showed that gender,age,location and diameter of the tumor,serosal invasion,Borrmann classification,range of gastric resection and the number of lymph nodes dissected were factors influencing the prognosis of patients with low-and undifferentiated gastric cancer and with negative metastasis of lymph nodes (x2 =4.143,12.607,23.665,11.549,26.350,8.059,5.222,25.077,P <0.05).The results of multivariate analysis showed that the tumor diameter longer than 5 cm and serosal invasion were the independent factors influencing the prognosis of patients with low-and undifferentiated gastric cancer and with negative metastasis of lymph nodes (HR =1.842,3.084,95% confidence interval:1.286-2.638,1.956-4.861,P < 0.05).Lymph nodes dissected for 21-30 was the protective factor of prognosis (HR =1.533,95% confidence interval:1.229-2.248,P < 0.05).Conclusions Lymph nodes dissected for 21-30 during radical gastrectomy may improve the long-term survival of patients.The tumor diameter longer than 5 cm,serosal invasion and the number of lymph nodes dissected (less than 15,15-20 and more than 30) were the independent factors influencing the prognosis of patients with low-and undifferentiated gastric cancer and with negative metastasis of lymph nodes.
8.Bone morphogenetic protein-2 carrier materials in the spinal fusion

BACKGROUND: There are some adverse reactions when bone morphogenetic protein-2 is applied independently in clinic; therefore, it is urgent to develop an ideal carrier material to reduce the incidence of adverse reactions induced by bone morphogenetic protein-2. OBJECTIVE:To comprehensively understand the performance of different kinds of bone morphogenetic protein-2 carrier materials, and to summarize their research progress in the field of spinal fusion. METHODS:The literatures about bone morphogenetic protein-2 scaffolds in spinal fusion were retrieved by the first author from PubMed and Wanfang datebase durings July 1, 1995 to July 1, 2015 with the key words of“tissue engineering, bone morphogenetic protein-2, spinal fusion, scaffold” in English and Chinese, respectively. Repetitive studies were excluded. RESULTS AND CONCLUSION:Bone morphogenetic protein-2 carrier materials mainly include four categories: natural biological materials, synthetic organic materials, synthetic inorganic materials and composite materials, each of which has several representative scaffold materials. Each category has their own shortcomings when applied in animal spinal fusion, for instance, the mechanical properties of natural biological materials are generaly poor; synthetic organic materials may cause inflammatory reactions; the mechanical strength of synthetic inorganic materials is poor and the production process of composite materials is relatively complicated. Therefore, the selection of bone morphogenetic protein-2 carrier materials stil needs further experimental study.
